The addition of a vaccine patrol team at Fairview Hospital led to an increase in pneumococcal vaccination rates among adult inpatients. During a 4-week period, the vaccination rate for patients over 65 increased from 4% to 9%. The vaccine patrol team identified 117 eligible patients and was able to administer vaccines to 75% of them. Common barriers to vaccination identified were nurses forgetting to order vaccines and difficulties navigating the electronic medical record to determine vaccination needs. The team recommends modifications to the electronic record like automatic vaccination orders to help nurses better identify and vaccinate eligible patients.
