In order to solve the serious problems of traditional surgical
orthodontic treatment, a new approach was adopted:
the first step is OGS, and this is followed by orthodontic alignment. This approach is named as Surgery First Orthognathic Approach (SFOA)
Used in the right conditions, SFOA is highly successful and has a positive impact on the patients psychosocial status.A combined orthodontic and orthognathic surgery approach is accepted as the standard of care for patients who have a severe skeletal jaw discrepancy with facial asymmetry.
But some disadvantages have been recognized.
One drawback is the long presurgical treatment time that typically worsens facial appearance and exacerbates the malocclusion. In some countries, these disadvantages have caused patients to seek plastic surgeons who are willing to perform orthognathic surgeries without collaboration with orthodontists or consideration for the final occlusion.
Recently, to address patient demand and satisfaction, the surgery-first approach was introduced to overcome some disadvantages associated with the conventional surgical orthodontic approach.1991-Brachvogel et al. suggested the potential advantages of a surgery-first approach.
In that article the advantages of post-surgical orthodontics are outlined as follows:
1) Orthodontic movement does not interfere with compensatory biological responses.
2) Dental movements can be based on an already corrected skeletal pattern.
3) Some surgical relapse can be managed during treatment.
2009: Nagasaka et al., popularized SFOA54. Nagasaka et al1 were among the first to actually carry out SFOA using miniplates for post-surgical orthodontic treatment
The 2011 symposium presented the surgery‑first approach and created broader interest in the complete elimination of time‑consuming preoperative orthodontic treatment

- The clinician must treat the patient virtually using the computer images long before touching even a single tooth.
- The lnvisalign System consists of mindset, software, impressions, working with the computer, understanding physical properties and behavior characteristics of the plastic and ensuing force systems acting on teeth, as well as patient management.
- Impressions must be taken with poly–vinyl siloxane (PVS) due to its superior accuracy and stability. The patient’s impressions, PVS bite registration, photographs (intraoral and extraoral) and radiographs are submitted to Align Technology in addition to the completed treatment form. (CT)
Each aligner is prescribed to be worn for 2 weeks and
is only removed for eating, drinking, brushing and flossing.
CT images of PVS impressions are transferred to a special software called "Treat" software.

38. If segmental movements are involved, a set of models is
sectioned at the osteotomy sites. Care should be taken when
sawing not to damage teeth other than those which are going
to be extracted at the time of surgery.

40. ANATOMICALY ORIENTED MODEL
SURGERY
In complex cases, especially where multiple bimaxillary
movements are required, it is essential to use a more
refined technique
In this technique, in addition to the impressions and sqash
bite, a face-bow recording is taken.
The working models are anatomically trimmed and
articulated on the semi adjustable articulator using the
face-bow recording and then the standard squash bite

43. Horizontal and vertical reference lines are drawn on the
mounting plaster to register the post-operative position of
each maxillary and mandibular segments before surgery.
Two sets of parallel horizontal lines A/A and B/B are
drawn on the upper and lower models. These are easily
done by rotating the detached model with the felt pen.
The B lines should be just clear of the apices of the teeth,
and not less than 15mm from the A lines. The actual
distance between the A and B lines is then recorded on the
plaster. These lines will be used to plan the vertical
movements

45. Three vertical lines VC, VB, VM are drawn from upper
base line (A) to the lower baseline (A) on each buccal
segment. These lines pass through the buccal surfaces of
the upper cuspid, bicuspid and the distal cusp of the last
upper molar tooth., and they are extended to their
occluding partners.
These will help to indicate the anteroposterior movements
achieved by the model surgery.

47. The vertical distances from the buccal cusp tips of the
three reference teeth to their A base lines are recorded to
help calculate any vertical movements.
Transverse changes are recorded by the inter-canine and
inter-molar distances measured across the palate and
recorded by taking reference points on the canine tips and
the mesiobuccal cusp of the first molars.

49. When all the reference lines have been drawn and the
measurements completed, the osteotomy lines are drawn
between A and B lines to correspond with the bone cuts.
The plaster mounting assembly is then sectioned at the
osteotomy sites with a saw or large abrasive disc and the
whole arch or segments are repositioned in the planned
post-operative position
