This film and media risk assessment form identifies potential hazards for a film production shooting in two campus halls. It lists six hazards: 1) electrical items could electrocute actors, 2) furniture could cause tripping, 3) light stands could fall over and hit people, 4) microphone wires could be trip hazards, 5) a slap stunt could injure an actor, and 6) a rolling tripod and camera could trip people or hit objects. For each hazard, it rates the severity, likelihood, and provides measures to reduce the risks, such as securing equipment, being aware of placements, and controlling wire paths.
