5. The School-Sub Committee shall perform the initial screening, evaluation and validation
of submitted documents by the applicants. The said committee shall be chaired by the School Head
with two to three (2-3) designated teachers as members.
6. The School Sub-Committee shall transmit all the documents and the List of Applicants
per Specialization to the Division Secretariat through the Records Section on or before April 12,
2024. They are also requested to submit the hard copy and soft copy of the List of Applicants per
area of specialization via electronic mail at sdoligaocity.personnel@deped.gov.ph.

12. DIVISION SELECTION SUB-COMMITTEES
DUTIES AND FUNCTIONS
a. Conduct orientation to applicants
b. Review and evaluate the submitted documents of the
applicants based on criteria
c. Evaluate the applicants' competencies on non-classroom
observable indicators (NCOI) using the Teacher Reflection Form
(TRF)
d. Evaluate the demonstration teaching performance of
applicants and rate them based on the standard rubrics
e. Review, validate and assign the equivalent ratings to
trainings attended by the applicant based on presented
documents/certificates
f. Other related duties and functions
13. ASSESSMENT PROCEDURES
a. The schedule of assessment for teacher-applicants shall be posted in the
division bulletin board and online, days before the assessment.
b. On scheduled date, the applicant shall be assessed based on the criteria
set in demonstration teaching, document evaluation and teacher reflection form per
DepEd Order 7, s. 2023.
c. A plenary orientation on the procedure shall be conducted by the Division
Sub-Committee Chairman to all teacher-applicants scheduled for the day. All health
protocols shall be strictly observed by all teacher-applicants and sub-committee
members.
d. After the orientation, the teacher-applicants shall proceed to their
designated rooms where they shall sign up the attendance sheet and write their
lesson plans for an hour according to their specialization.
e. The participants are expected to bring raw materials in preparing audio
visual aids such as pentel pens, manila paper, colored papers and laptops. No
ready-made lesson plans shall be allowed and bringing such shall be a ground for
disqualification.
14. ASSESSMENT PROCEDURES
f. After an hour, all teacher-applicants shall submit the lesson plans to the sub-
committee member of the group where they belong and shall go out of their rooms to stay
at the designated holding areas and wait for their turn for the demonstration teaching
where they will be rated according to the PPST Classroom Observation Indicators (COIs),
except for the first applicant who will be given at least five minutes to go over his/her
lesson plan for the demonstration teaching.
g. The teacher applicant shall be given at least 15-20 minutes of demonstration
teaching in front of actual learners.
h. Immediately after the demonstration teaching, the sub-committee shall agree on the
rating using the COT-RSP rating sheet and the COT-RSP Inter-Observer Form, both
of which shall be signed by the observer and the applicants.
i. Right after the demo teaching of the first applicant and while the committee is
deliberating on his/her rating, the lesson plan of the next applicant shall be given for
him/her to be ready for his/her turn. The process shall be repeated until the last teacher-
applicant has undergone demonstration teaching and has been rated.
j. The results of COI’s shall immediately be submitted to the Secretariat for
inclusion in the CAR-RQA.
15. ASSESSMENT PROCEDURES
k. Applicants who are waiting for their turn to demo teach and those
who have already undergone demonstration teaching shall proceed to the
assigned room/s of the Division Sub-Committee on ETE/ PBET/ LET/ LEPT for
the paper/ document evaluation.
l. To ensure transparency of results, a projector shall be set up where
the corresponding points in ETE/ PBET/ LET/ LEPT shall be reflected.
m. The Teacher Reflection Form (TRF) of teachers scheduled for the
day shall be evaluated by the Division Sub-Committee in charge of this who
shall agree on the individual ratings that shall be reflected in the Teacher
Reflection Inter-Evaluation Agreement Form for submission to the Secretariat.
n. The result of the assessment shall be reflected by the secretariat in
the CAR-RQA and in the Individual Evaluation Sheet (IES) which shall be
signed by the applicant and attested by the Chairman of the HRMPSB or
his/her authorized representative.
16. DOCUMENTS TO BE SUBMITTED BY
APPLICANTS
a. Letter of intent addressed to the Schools Division Superintendent
which shall indicate the following information:
• Statement of Purpose/ Expression of Interest
• Subject group he/she intends to teach.
• Preferred school(s), if any
b. Duly Accomplished Personal Data Sheet (CSC Form No. 212,
Revised 2017) with the latest passport size ID picture and with Work
Experience Sheet;
c. Photocopy of valid and updated PRC License/ID, if applicable;
d. Photocopy of Certificate of Eligibility / Report of Rating, if applicable
e. Photocopy of scholastic/academic record such as but not limited to
Transcript of Records (TOR) and Diploma, including completion of graduate
and post-graduate units/degrees, if available;
f. Photocopy of Certificate/s of Training, if applicable;

j. Checklist of Requirements and Omnibus Sworn Statement on the
Certification on the Authenticity and Veracity (CAV) of the documents
submitted and Data Privacy Consent Form pursuant to RA 10173 (Data
Privacy Act of 2012) using the attached form and notarized by authorized

k. National Certificates issued by TESDA, if applicable
l. Copy of the Voter’s ID and/ or any proof of residency as deemed
acceptable by the HRMPSB.

18. INSTRUCTION FOR APPLICANTS
7. Applicants should indicate in their application letter whether they are applying in the
Kindergarten, Elementary or Secondary level (JHS/SHS) indicating their
major/specialization/ track.
8. Applicants who wish to apply in different levels should submit documents including
the letter of intent separately following the color coded folders: YELLOW for
Kindergarten; BLUE for Elementary, GREEN for Junior High School and RED for
Senior High School.
• Reminder: Elementary LET eligibility holders can only apply in the Elementary
Level while Secondary LET eligible can apply both in the Elementary &
Secondary level.
9. All applicants shall undergo the assessment process and shall submit the required
CHECKLIST OF REQUIREMENTS AND OMNIBUS SWORN STATEMENT.
11. Applicants with special needs (e.g., PWD) should submit to the Human Resource
Management Promotion and Selection Board a letter request for any assistance that they
might need during the evaluation and ranking process.

School Division Office may call for recruitment if all teachers to the CAR-RQA are already

the Official Memorandum /Call for Applications shall warrant exclusion from the pool of official
applicants
