General Instructions
General Instructions
a. Letter of intent addressed to the Head of Office, or to the
highest human resource officer designated by the Head of
Office
b. Duly accomplished Personal Data Sheet (PDS)
(CS Form No. 212, Revised 2017) and Work Experience
Sheet, if applicable
c. Photocopy of valid and updated PRC License/ID, if
applicable
d. Photocopy of Certificate of Eligibility/Report of Rating, if
applicable
The Teacher Applicant shall submit the following:
e. Photocopy of scholastic/academic record such as
but not limited to Transcript of Records (TOR) and
Diploma, including completion of graduate and
post-graduate units/degrees, if available
f. Photocopy of Certificate/s of Training, if applicable
g. Photocopy of Certificate of Employment, Contract of
Service, or duly signed Service Record, whichever
is/are applicable
h. Photocopy of latest appointment, if applicable
i. Photocopy of the Performance Ratings in the last
rating period(s) covering one (1) year
performance prior to the assessment, if applicable
j. Checklist of Requirements and Omnibus Sworn
Statement on the Certification on the Authenticity
and Veracity (CAV) of the documents submitted and
Data Privacy Consent Form (Annex C)
k. Other documents as may be required for
comparative assessment:

Means of Verification (MOVs) showing Outstanding
Accomplishments, Application of Education, and
Application of Learning and Development
reckoned from the date of last issuance of
appointment
Photocopy of Performance Rating obtained from the
relevant work experience, if performance rating in
Item (i) is not relevant to the position to be filled
School Screening Committee
1. Receive and verify application documents
2. Submit List of Applicants in the School and forward the documents
of the teachers applicants
3. Conduct Demonstration Teaching
a. The schedule will set by the school head of the school
where the applicants applied.
b. The observers shall be composed of the School Head
and a Master Teacher/Head Teacher (preferably of the
learning area for secondary ). In case, there is no Master
Teacher in the School, the School head of the nearby school
will be one of the observers.

c. The actual lesson of the day will be given a day before
the demonstration teaching.
d. Follow the class demonstration procedure prescribed
in DO 07, s. 2023, Enclosure 2 page 7-9)
e. Administer the Non- Classroom Observable Indicators
through Teacher Reflection and submit to the Division
Sub-committee for Rating. Prompt Questions shall
given by the HRMPSB through the Division Sub-Committee.

3. Submit to the Division Sub-committee the result of the
Demonstration Teaching including COT Rating Sheet, Inter-
observer and Agreement Form.
4. Submit to the Division Sub-committee the coded Teacher
Reflection Form of the applicants

1. Receive and application documents from the School Screening
Committee
2. Conduct initial evaluation of applicants qualifications
3. Conduct assessment on the following
a. Education
b. Training
c. Experience
d. PPST Non-Classroom Observable Indicators (NCOI) measured
through Teacher Reflection
Division Sub-Committee
4. Receive prompt questions from the HRMPSB and distribute to the
school committee for administration.
5. Rate the NCOI / Teacher Reflection following Enclosure No. 2 page
10-12 of DO 07, s. 2023.
6. Prepare and submit soft and hard copies of the Initial Result of the
Assessment of Teacher Applicants to the Division Committee/
HRMPSB.
• Submission of checked and validated Initial Assessment Results to
the Division Office (Separate Results for Kindergarten, Elementary
and Junior and Senior High School) in hard and soft copy

  • 7. School Screening Committee 1. Receive and verify application documents 2. Submit List of Applicants in the School and forward the documents of the teachers applicants 3. Conduct Demonstration Teaching a. The schedule will set by the school head of the school where the applicants applied. b. The observers shall be composed of the School Head and a Master Teacher/Head Teacher (preferably of the learning area for secondary ). In case, there is no Master Teacher in the School, the School head of the nearby school will be one of the observers.
  • 8. School Screening Committee c. The actual lesson of the day will be given a day before the demonstration teaching. d. Follow the class demonstration procedure prescribed in DO 07, s. 2023, Enclosure 2 page 7-9) e. Administer the Non- Classroom Observable Indicators through Teacher Reflection and submit to the Division Sub-committee for Rating. Prompt Questions shall given by the HRMPSB through the Division Sub-Committee.
