A wound is a break in the integrity of the skin or tissues, which may be associated with disruption of the structure and function.
Healing is the body’s response to injury in an attempt to restore normal structure and functions.
Healing can occur in two ways: Regeneration and Repair
There are 4 phases of wound healing: hemostasis, inflammation, proliferation, and remodeling. This document also describes the mechanism of wound healing. Factors that affect healing include infection, uncontrolled diabetes, poor nutrition, age, anemia, the presence of foreign bodies, etc.
Complications of wound healing like infection, hyperpigmentation of scar, contractures, and keloid formation.

- 4. Who is allowed to shut off the Oxygen on the unit? • In the event oxygen needs to be shut off to our unit, the first action is to ensure all patients needing oxygen are secure • The charge nurse can notify the house supervisor or the unit supervisor once all patients have been secured and Oxygen may be shut off
- 5. How often should patients on our unit be weighed? • EVERY DAY! • Our night shift PCAs should be weighing the patients everyday around the same time in their last rounds. • This should be documented in Meditech daily • Why is this important? • Our CHF and Dialysis patients need to be weighed to ensure that the treatment plan we are providing is working. • As of ....... Dialysis will no longer be picking up patients if there is no weight documented. Please do not delay patient care by not having this documented.
- 6. When should you take a verbal (not telephone) order? • Only in the case of an emergency • Orders are to be written in the chart as verbal only when the patient is in distress or an emergency has arisen • Telephone orders are to be timed and dated
- 7. Where can you find a list of materials located on the crash cart? • In each crash cart binder there is a detailed list of the items that you are able to find in the crash cart.
- 8. When should you document on the education screen? • Anytime that you educate your patient on ANYTHING! • New medications • Diet • Isolation education needs to be documented on any patient placed in isolation.
