This is my somewhat "wasted" innovation project proposal. I intend to share it here for academic purposes. This is just a draft however and should not be used in citations or referencing. I decided to put it here in this platform finally because I no longer want to engage the pleasures of the higher ups. Otherwise, this will just be a matter of compliance and not love for genuine learning and academic freedom. The whole research and innovation process in DepEd has become too tedious, restrictive, and red-taped. If you write too long, they may want your paper to be concise or shorter and then if you cut it short, they then want it to be much longer. I still cannot see how some people become so obsessed with editing or proofreading nowadays when we don't even have the perfect English in this country and we don't have the same wavelengths to understand each other. Pants down.

2. Signed Standard Letter of Application for Government Permit/Recognition
3. Inspection Report of the Division Inspection Team/Supervisor In-Charge of Private Schools
4. Board Resolution (certified by the Corporate Secretary) stipulating the intention to apply for Permit/ Recognition of specific Basic

5. Philosophy and goals of the course
• Pictures/Lists of prospective enrollees (by class) and school personnel
6. Securities and Exchange Commission (SEC) Registration
• Registration is in the name of the school
• Course applied is mentioned in the Articles of Incorporation
• Incorporators/owners are citizens of the Philippines (except those established by religious groups or mission boards)
• At least 60% of Capital Contribution (Php1,000,000.00 for Preschool , Php1,000,000.00 for Elementary, Php1,500,000.00 for Secondary) is
owned by the owners/incorporators
7. School Governance
• Organizational Chart indicating the administrative, teaching, and non-teaching personnel
• Transcript of Records of the school head (Masters Degree holder), teachers, nurse, physician, dentist, librarian
• PRC Rating/License of the school head and teachers
• Notarized Employment Contracts of the school head, members of teaching and non-teaching staff
8. School Site ,school buildings, classrooms, ancillary facilities, furniture and fixtures
• Copies of documents showing ownership of school site and buildings (OCT, TCT, Deed of Donation, Copy of payment of Real Property Tax for
School Buildings) or Contract of Lease that shall stand 10 years from when application is filed
• Copy of the Certificate of Occupancy
• Comprehensive Floor Plan indicating area, dimension, maximum and actual number of learners that classrooms and non-academic rooms
(computer room, library, home economics/TLE room, science laboratory, etc.) accommodate
• Pictures of principal’s office, guidance office, computer room, library, home economics/TLE room, science laboratory, comfort rooms, canteen,
clinic (with cot/bed, medicine cabinet, first aid kit, etc), playground, hand washing areas/facilities, fire extinguishers, fire escapes, fire exits,
ramps for learners with disabilities/special needs,
• Pictures of classrooms showing chairs, desks, tables, cabinets/shelves, black/white boards, display boards, bulletin boards, etc.
9. Curriculum
• Copy of DepED Curriculum Learning Competencies for each level (and school’s special curriculum if available)
• Copies of comprehensive class programs indicating the year level and section, time, name of subject, name of teacher who handles the
subject, and number of minutes for each subject
• List and pictures of textbooks and supplementary materials
• Lists and pictures of teaching aids/instructional materials, library resources/materials/equipment, athletic/sports supplies and equipment,
laboratory supplies/materials/equipment, TLE tools and utensils, curriculum materials
• List of co- and extra-curricular activities and special programs/projects
• Copy of the School Handbook
• Copy of Staff Training/Professional Development Plan
10. Others
• Copy of school budget indicating the source of funds
• Copy of proposed tuition and other school fees
• Copy of Latest Financial Statement certified by an external CPA with Independent Auditor’s Opinion
• Copy of receipt of payment of school bond (payable to Regional Office), inspection fees (payable to the Division Office)
• Copies of receipts of latest remittances to SSS, Philhealth, PAGIBIG (and Retirement Plan)
