PSYCHOLOGICAL TEST REPORT
,[object Object],Name:Chloe Beatrice D. Tiu
Age:5 years and 10 months old
Date of Birth:June 15, 2004
Place of Birth:Quezon City
Address:Unit 1a Montessori Mansion, Xavier St. Greenhills
Referred By:Sr. Dina Ang, M.I.C. , ICA Directress
Date Referred:March 12, 2010
TEST ADMINISTERED:DATE ADMINISTERED:
House Tree Person TestApril 29, 2010
Stanford Binet Intelligence ScaleApril 29, 2010
Children Apperception ScaleApril 29, 2010
,[object Object],Chloe was garb with a pink sleveless whole dress and has a pony tailed shoulder length hair. She has white complexes and an average plump. She was proficient in terms of oral expression and exceeds her vocabulary beyond her age because she speaks English fluently with intonation. The volume of voice was normal which she was hesitant in giving information. She was gregarious, friendly which she get easily close to people, and relate given topics to her experiences in a collaborative manner. Furthermore, she was well oriented in terms of time, person which was interacting, and the place where the examination is conducted.
,[object Object],From the Stanford-Binet Intelligence Scale, Chloe achieved an I.Q. of 117 which signifies her above average mental ability. Her mental age of seven years and two months exceeds her chronological age of five years and ten months which denotes that she has an advance intellectual ability. Evaluating her intellectual performance, Chloe shows an adequate motor proficiency and attention. She has a good motor process which can easily recall factual information and has a huge storehouse of information. Besides, she has an adequate recalls in terms of visual information, auditory information, differentiates and established relationship between unrelated concepts and ideas. In terms of personal information, she is highly curious, self-motivated and highly flexible which she can easily adapt to new circumstances.
,[object Object]
House Tree Person TestThe quality of figures in terms of house, tree, and person was satisfactory for Chloe’s present age. Test result shows the relationship between the person and an animal which suggest fear of revealing self to conceal her problems or conflicts. Moreover, it emphasizes hostility or anger towards environment. Probable cause of this was over control from her parents. When a child dominated her expression of feelings and actions from parent’s discipline, he or she tends to exclude this by acting opposite behavior from external environment. Such as in school, she uses her intellect by reciting and obtaining good grades by studying or hard or playing with other children. This appears as a placement of house at right side in the figure.
,[object Object],Almost all of Chloe’s responses indicate usual patterns of daily activities. The quality time with her family such as waking up in the morning, preparing breakfast, going to school or work is probably the result of absence of parents. These repetitive patterns might affect Chloe’s attitude and behavior because she only achieve basic needs such as food, clothes, and necessities at school. She felt inadequate every time she handle herself doing own assignments or preparing herself going to school which appears at Card No. 9 as self-caring and helpless.
,[object Object]
Fear of Revealing Self – Chloe needs to establish close relationship from her parents. Her parents must ask if there’s any problem at her school, or in their family so that she can express her burden feeling.

 • 3. Fear of Revealing Self – Chloe needs to establish close relationship from her parents. Her parents must ask if there’s any problem at her school, or in their family so that she can express her burden feeling.
 • 4. Using intellect as defense for Unexpressed Anger – Parents must decrease dominance or over controlling of Chloe’s action and expression of her feeling. It may cause her to detach her emotion and lack of empathy when she grew older. It suggests that let enjoy her childhood life which open new opportunities to interact with other children because her age was more engage in playing rather than going to school.
 • 5. Inadequacy – Needs to have more quality time with her parents because she felt sometimes, its hard handling herself. This will enable her to feel that she was love, nurture, and support.Thank you for referring,
Serafina P.
