Problem Statement: MATHEMATICS Classes experiencing low students’ performance that lead to students in failing the
subject
Project Objective Statement: To increased students’ performance and attained 100% passing rate in each quarter
Root Cause: The poor academic performance may result from the ff. causes
 Low Numeracy Result
 Low MPS in any Mathematics assessment
