Health has become very expensive in the United States. A very big percentage of people are finding it hard to fund their medical expenses. To deal with this problem, the reduction of healthcare costs and the increase in performance management are two major components that are required. The management and accounting practices of the health industry have been highly affected by the need to reduce the costs and improve the performance of the sector. Activity based accounting, which is often borrowed from the private sector, is one of the main strategies that most institutions in the health industry are trying to apply to improve the performance of the sector. This research paper intends to analyze the case of the health industry and how it has applied activity based accounting. The research will also analyze the process of implementation of activity based accounting in the industry and the benefits that it has had in the areas within which it has been implemented.
INTRODUCTION
Healthcare systems in almost all countries face the pressure of delivery effective but cost-efficient care in the face of increasing demands. Healthcare has faced a great challenge of providing affordable healthcare services. These pressures have led to the development of various initiatives and strategies to improve the quality of healthcare delivery in the country. Activity based accounting is one strategy that has been applied by various US healthcare organizations to improve the efficiency of the care they provide. This strategy assigns costs according to the activities performed; making it easier to track the consumption by each. Context
Community/public health nursing is nursing care that is population-focused and occurs in non-hospital settings. Numerous nursing theories can provide the structure for community/public health nursing; however, nursing theories that incorporate components of the general systems theory frequently provide the framework for the practice of community/public health nursing.
Historically, nurses have made significant contributions to the field of public health. From providing maternal-child care to poor women in the late 1800s, to promoting hygiene among school aged children in the early 1900s, to providing environmental and safety care to industrial workers during World War I, nurses have been instrumental in shaping health policies (Maurer & Smith, 2013). Today, community/public health nurses have a key role in identifying and developing plans of care to address local, national, and international health issues.
Many factors influence the health of communities and populations, including national policies that focus on health promotion. Healthy People, a major national health promotion program issued by the U.S. Surgeon General, identifies major health problems of the nation and sets national goals and objectives targeting health promotion (Maurer & Smith, 2013). If adopted, activities that target health promotion can result in disease prevention.

The first phase of this Quality Initiative documented the serious and pervasive nature of the nation's overall quality problem, concluding that "the burden of harm conveyed by the collective impact of all of our health care quality problems is staggering" (Chassen et al., 1998).
IOM Definition of Quality
The degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge.
This phase built on an intensive review of the literature conducted by RAND to understand the scope of this issue (Schuster) and a framework was established that defined the nature of the problem as one of overuse, misuse and underuse of health care services (Chassen et al). More specifically, the report Ensuring Quality Cancer Care (1999) documented the wide gulf that exists between ideal cancer care and the reality many Americans with cancer experience.
During the second phase, spanning 1999-2001, the Committee on Quality of Health Care in America, laid out a vision for how the health care system and related policy environment must be radically transformed in order to close the chasm between what we know to be good quality care and what actually exists in practice. The reports released during this phase—To Err is Human: Building a Safer Health System(1999) and Crossing the Quality Chasm: A New Health System for the 21st Century(2001)—stress that reform around the margins is inadequate to address system ills.
The series of IOM quality reports have included a number of metrics that illustrate how wide the quality chasm is and how important it is to close this gulf, between what we know is good quality care and what the norm is in practice.
To Err is Human put the spotlight on how tens of thousands of Americans die each year from medical errors and effectively put the issue of patient safety and quality on the radar screen of public and private policymakers. The Quality Chasm report described broader quality issues and defines six aims—care should be safe, effective, patient-centered, timely, efficient and equitable—and 10 rules for care delivery redesign.
Phase three of the IOM's Quality Initiative focuses on operationalizing the vision of a future health system described in the Quality Chasm report. Payments
INTRODUCTION
Payment design, coverage policies, reimbursement rules, and other financial incentives and
disincentives are powerful motivators when attempting to steer the healthcare system toward
more desirable care patterns (Guterman et al., 2009). Experiments with payment design and
coverage and reimbursement policies are currently going on in both public and private healthcare
sectors, with varying results. Speakers in this session of the workshop explored current payment
design experiments and discussed the efficacy of utilizing these reimbursement tools to improve
the value received from health care.
In this chapter, Carolyn M. Clancy details the pay-for-performance (P4P) model, an effort to
more explicitly link provider payments to quality of care. She highlights the lack of coherent
approaches to P4P and the variable success this approach has had in fundamentally changing
provider practice patterns. For example, while financial incentives for individual physicians have
shown that P4P can induce quality improvements for diabetic patients (Beaulieu and Horrigan,
2005), group-level incentives have had no impact on mammography screening or hemoglobin A
testing rates (Rosenthal et al., 2005). After underscoring that the current incentive system and
healthcare infrastructure fail to accommodate the achievement of real efficiency and quality, she
outlines recommendations for rethinking medical training, measurement, system design, and the
reward system.
Building on Clancy’s recommendations, Donald A. Sawyer identifies how the current healthcare
system stymies innovation in product development. He suggests refocusing the myopic view of
innovation on the horizon of long-term health improvements and financial savings. Reed V.
Tuckson discusses the alignment of manufacturers, technologists, payers, patients, and providers
necessary to establish a system that continues to provide incentives for innovation and maintains
an open market for the development of promising but unproven interventions. He elaborates
specifically on a joint effort between UnitedHealth Group and the American College of
Cardiology to develop appropriateness criteria for cardiac single-photon emission computed
tomography myocardial perfusion imaging—a new and very expensive technology—based on

ABSTRACT. Research studies demonstrate wide varia-
tion in how physicians diagnose and treat patients with
similar medical conditions and suggest that at least some
of the variation reflects inefficiencies and unnecessary
medical costs. Health care researchers are actively exam-
ining ways to reduce variations in practice through
standardization of medicine to reduce the cost of treat-
ment and ensure the quality of outcomes. The most
widely accepted form of this standardization is Evidence
Based Best Practices (EBBP). Furthermore, financial
health care providers such as hospitals and managed care
organizations are investigating methods to tie resource
usage to medical protocols in their efforts to monitor and
control health care costs. Such proposals are contentious
because they report on physicians’ medical practice
behaviors (such as the number of tests ordered, use of
specific therapies, etc.) and such reports could potentially
be used to influence their clinical behaviors. The intent of
this exploratory study was to examine physicians’
perceptions about linking a standard costing system to
EBBP guidelines. The authors interviewed nine practic-
ing physicians asking each physician to respond to the
question, ‘As a physician working in a hospital environ-
ment, what are your reactions to and concerns with
combining standard costing techniques with EBBP?’ The
interviews were in-depth and free form in nature. The
physicians’ responses were recorded and analyzed using
Grounded Theory Methodology. Using this methodol-
ogy the field data was categorized into two major themes.
The most important theme centered on ethics and the
second theme was concerned with the implementation
and use of a standard cost system in regard to EBBP. If
physicians’ worries about ethical dilemmas and imple-
mentation issues are not resolved, then it is likely that
doctors would be unwilling to participate in any efforts to
develop or use a standard cost-reporting system in med-
icine. While this study was exploratory in nature, it
should provide future guidance to accountants, health
care researchers and health care providers about physi-
cians’ issues with the use of standard costing methods in
medicine.

Context
Community/public health nursing is nursing care that is population-focused and occurs in non-hospital settings. Numerous nursing theories can provide the structure for community/public health nursing; however, nursing theories that incorporate components of the general systems theory frequently provide the framework for the practice of community/public health nursing.
Historically, nurses have made significant contributions to the field of public health. From providing maternal-child care to poor women in the late 1800s, to promoting hygiene among school aged children in the early 1900s, to providing environmental and safety care to industrial workers during World War I, nurses have been instrumental in shaping health policies (Maurer & Smith, 2013). Crossing the Quality Chasm: The IOM Health Care Quality Initiative
In 1996, after releasing America's Health in Transition: Protecting and Improving Quality, the IOM launched a concerted, ongoing effort focused on assessing and improving the nation's quality of care.
The first phase of this Quality Initiative documented the serious and pervasive nature of the nation's overall quality problem, concluding that "the burden of harm conveyed by the collective impact of all of our health care quality problems is staggering" (Chassen et al., 1998).
IOM Definition of Quality
The degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge.
This phase built on an intensive review of the literature conducted by RAND to understand the scope of this issue (Schuster) and a framework was established that defined the nature of the problem as one of overuse, misuse and underuse of health care services (Chassen et al). More specifically, the report Ensuring Quality Cancer Care (1999) documented the wide gulf that exists between ideal cancer care and the reality many Americans with cancer experience.
During the second phase, spanning 1999-2001, the Committee on Quality of Health Care in America, laid out a vision for how the health care system and related policy environment must be radically transformed in order to close the chasm between what we know to be good quality care and what actually exists in practice. The reports released during this phase—To Err is Human: Building a Safer Health System(1999) and Crossing the Quality Chasm: A New Health System for the 21st Century(2001)—stress that reform around the margins is inadequate to address system ills.
The series of IOM quality reports have included a number of metrics that illustrate how wide the quality chasm is and how important it is to close this gulf, between what we know is good quality care and what the norm is in practice.
To Err is Human put the spotlight on how tens of thousands of Americans die each year from medical errors and effectively put the issue of patient safety and quality on the radar screen of public and private policymakers. The Quality Chasm report described broader quality issues and defines six aims—care should be safe, effective, patient-centered, timely, efficient and equitable—and 10 rules for care delivery redesign.
Phase three of the IOM's Quality Initiative focuses on operationalizing the vision of a future health system described in the Quality Chasm report. Payments
INTRODUCTION
Payment design, coverage policies, reimbursement rules, and other financial incentives and
disincentives are powerful motivators when attempting to steer the healthcare system toward
more desirable care patterns (Guterman et al., 2009). Experiments with payment design and
coverage and reimbursement policies are currently going on in both public and private healthcare
sectors, with varying results. Speakers in this session of the workshop explored current payment
design experiments and discussed the efficacy of utilizing these reimbursement tools to improve
the value received from health care.
In this chapter, Carolyn M. Clancy details the pay-for-performance (P4P) model, an effort to
more explicitly link provider payments to quality of care. She highlights the lack of coherent
approaches to P4P and the variable success this approach has had in fundamentally changing
provider practice patterns. For example, while financial incentives for individual physicians have
shown that P4P can induce quality improvements for diabetic patients (Beaulieu and Horrigan,
2005), group-level incentives have had no impact on mammography screening or hemoglobin A
testing rates (Rosenthal et al., 2005). After underscoring that the current incentive system and
healthcare infrastructure fail to accommodate the achievement of real efficiency and quality, she
outlines recommendations for rethinking medical training, measurement, system design, and the
reward system.
Building on Clancy’s recommendations, Donald A. Sawyer identifies how the current healthcare
system stymies innovation in product development. He suggests refocusing the myopic view of
innovation on the horizon of long-term health improvements and financial savings. Reed V.
Tuckson discusses the alignment of manufacturers, technologists, payers, patients, and providers
necessary to establish a system that continues to provide incentives for innovation and maintains
an open market for the development of promising but unproven interventions. He elaborates
specifically on a joint effort between UnitedHealth Group and the American College of
Cardiology to develop appropriateness criteria for cardiac single-photon emission computed
tomography myocardial perfusion imaging—a new and very expensive technology—based on

ABSTRACT. Research studies demonstrate wide varia-
tion in how physicians diagnose and treat patients with
similar medical conditions and suggest that at least some
of the variation reflects inefficiencies and unnecessary
medical costs. Health care researchers are actively exam-
ining ways to reduce variations in practice through
standardization of medicine to reduce the cost of treat-
ment and ensure the quality of outcomes. The most
widely accepted form of this standardization is Evidence
Based Best Practices (EBBP). Furthermore, financial
health care providers such as hospitals and managed care
organizations are investigating methods to tie resource
usage to medical protocols in their efforts to monitor and
control health care costs. Such proposals are contentious
because they report on physicians’ medical practice
behaviors (such as the number of tests ordered, use of
specific therapies, etc.) and such reports could potentially
be used to influence their clinical behaviors. The intent of
this exploratory study was to examine physicians’
perceptions about linking a standard costing system to
EBBP guidelines. The authors interviewed nine practic-
ing physicians asking each physician to respond to the
question, ‘As a physician working in a hospital environ-
ment, what are your reactions to and concerns with
combining standard costing techniques with EBBP?’ The
interviews were in-depth and free form in nature. The
physicians’ responses were recorded and analyzed using
Grounded Theory Methodology. Using this methodol-
ogy the field data was categorized into two major themes.
The most important theme centered on ethics and the
second theme was concerned with the implementation
and use of a standard cost system in regard to EBBP. If
physicians’ worries about ethical dilemmas and imple-
mentation issues are not resolved, then it is likely that
doctors would be unwilling to participate in any efforts to
develop or use a standard cost-reporting system in med-
icine. While this study was exploratory in nature, it
should provide future guidance to accountants, health
care researchers and health care providers about physi-
cians’ issues with the use of standard costing methods in
medicine.

Leaders in low- and middle-income countries (LMICs) require timely and compelling evidence about how to strengthen their health systems to improve the health and well-being of their citizens. Yet, evidence on how to strengthen health system performance to achieve sustainable health improvements at scale, particularly toward Ending Preventable Child and Maternal Deaths (EPCMD), fostering an AIDS-Free Generation (AFG), and Protecting Communities against Infectious Diseases (PCID) is limited. The evidence that does exist is scattered, insufficiently analyzed, and not widely disseminated. Without evidence, decision-makers lack a sound basis for investing scarce health funds in health systems strengthening (HSS) in an environment of competing investment options.
USAID is committed to advancing the evidence base on HSS and this commissioned report clearly demonstrates that HSS can improve health in LMICs.

Making decisions on who delivers health services and where and how these services are organized is important to achieve priority health goals such as EPCMD, AFG, and PCID. The findings of this report document the value of investing in HSS.
