4. PHC 1st Quarterly Partner Meeting
Welcome
On March 17, 2016 the PHC hosted its first
quarterly meeting of 2016. During this
meeting we updated our community
partners on what's in store for the PHC as we
continue moving toward building a more
integrated preventive health system for
children birth to five and their families.
The PHC is a collaborative built for impact.
This meeting helped set the tone for 2016
with over fifty partners ready to embark on
systems change, systems building, and
community transformation efforts.
Lisa Leighton, Arizona Executive Director
myCommunity Connect, kicked off the day
with her presentation on the myCommunity
Connect integrated health model that is
being piloted in Maryvale. This model joins co
-location of family and medical support
services with an electronic infrastructure that
helps connect our families with the resources
and services they need to thrive.
Michele Schmidt, Senior Evaluator for LeCroy
and Milligan, presented the progress on the
PHC’s Prevention Plan. The PHC Steering
Committee and the LeCroy and Milligan
team is hard at work developing indicators,
baselines and evidence based practices for
each of the Health Concern Areas we re-
developed in 2015.
Our day ended with the launching of four
Action Learning Teams (ALTs) and two
Workgroups. These workgroups and ALTs will
be the driving forces behind the PHC and
they got off to a great start in identifying
their Aim Statements and gelling as teams!
Thank you to all who attended and helped
make our first quarterly meeting a success!
We hope to see you all at our next Quarterly
PHC Community Partner meeting in July 21,
2016.

OBJECTIVE 1: Access to Care
The PHC is working with LeCroy & Milligan Associates, Inc. to develop a PHC Prevention
Plan. The PHC Prevention Plan will provide an overview of the objectives, goals, baselines,
and indicators of success for each of our Health Priority Areas that our collaborative refined
in 2015. The Prevention plan will also provide a list of Evidence Based Strategies and
Promising Practices for each Objective. Lastly, we will include descriptions of the
Workgroups and Action Learning Teams that are currently working on policy, systems, and
community transformation efforts so that you can easily see how you and your organization
can help us Create a Healthier Tomorrow, Today. A sample of what this might look like is
below (not actual objectives).

7. PHC Structure
The PHC is structured for impact. Each member of our Steering committee leads a community
based work group or PHC operations workgroup. The inner circle are the PHC operations
workgroups and the outer circle represent the policy, systems and community transformation
workgroups and Action Learning Teams.

8. Organizing
Our Work
Workgroups:
Workgroups are led by Steering Committee Members and PHC backbone staff to convene
PHC Partners and develop an annual action plan designed to address one of the PHC’s
Health Concern Areas. The workgroups develop goals, indicators of success and work with
the PHC as a whole to move our work forward.
Action Learning Team (ALTs):
Action Learning Teams are composed of PHC partners from multiple community sectors
and an ALT Coach who together work to address a community based/ systemic challenge
to one of the PHC’s Health Concern Areas. The ALTs devise a strategic plan and the ALT
coach helps implement that plan. The ALT Coach also helps the team actively reflect on
the team’s progress, successes, and barriers through an action-learning process. These
teams will help all of us develop promising practices that will raise the capacity of our entire
PHC community.
