1. A woman named Hasmiah Rewa brings her sick child, Abulkhair, to the hospital to see a doctor.
2. At the hospital, nurse Amrah Arief examines the child but says the doctor is busy and the child will need to come back tomorrow.
3. The next day, Hasmiah Rewa returns to the hospital with Abulkhair and they see doctor Anugrah, who examines the child and determines he has been coughing for 2 days. The doctor prescribes medicine and tells them to return if the cough does not improve in 3 days.
The doctor rushed to the hospital to perform emergency surgery on a boy after being called in. The boy's father yelled at the doctor for taking too long to arrive. During the surgery, the doctor remained calm and reassuring despite the father's anger. After the successful surgery, the doctor quickly left for another commitment. The nurse then informed the father that the doctor's own son had died the day before in an accident, and that the doctor had left the burial to save the boy.
The document describes the narrator's experience getting multiple rescheduled mammogram appointments before finally getting tested. When she gets her results, she is told further testing is needed. She worries it may be cancer. She prays and continues performing. When she gets home, she feels a mass in her breast, confirming her fears. She schedules an appointment for an ultrasound and compression films to get confirmation. Throughout, she reflects on family members lost to cancer and her determination to fight if she receives a cancer diagnosis.
We suggest triggering shifts in the healthcare experience through the introduction of a new phrase, coupled with intervention design
The report is based on two core insights into the healthcare system :
1. The healthcare systems is a self-optimizing, learning system manned by human beings.
We take the position that shifting the "system" is a matter of opening a window of empathy so the key actors can come face-to-face with the unintended consequences of their actions.
2. Experiences are a collection of moments.
Some moments have more power than others. Moments of pain carry the most power and the possibility to evoke empathy and the desire to change. These moments of mediation ( suggesting that shift is needed) represent slices through the healthcare system. Examining one reveals an entire chain of constraints responsible for the pain faced by the patient or caregiver, by omission or commission.
Paired with an intervention design approach: each moment is an entry point for shifting the experience within the healthcare system
This document documents 13 separate journeys the patient or caregivers took and 60 different moments of mediation where they felt helpless or powerless.

Tim experienced what it was like to be disabled for 24 hours by taking on the role of a nursing home patient with multiple sclerosis. He found it frustrating to be dependent on others for all his needs and unable to perform even simple tasks independently like reading or using a computer. While some nurses were helpful, he observed that staff were often too busy with other patients to give him individual attention. The experience gave him insight into how people in nursing homes feel and perspectives on providing more compassionate care.
