 Initiated integration process of a 600-physician hospital organization to develop health interventions at the
physician and community levels, involving: physicians, four hospitals, and a data vendor to develop evidence-
based clinical practice guidelines, collecting and measuring outcomes over time from inpatient, outpatient,
emergency room, and ancillary services.
 Developed PCMH NCQA program for 53 practices within the PHO, submission to NCQA 2012-2013 achieving
level 2 and level 3 recognition.
