This document outlines strategies for implementing a patient portal at Hospital A to increase patient engagement. It proposes a three-phase plan: 1) Communicate the vision to employees, physicians and patients, and form a management committee; 2) Select and implement a user-friendly patient portal that integrates with their EMR system and provides education materials in multiple languages; 3) Roll out the portal over 6-12 months and continuously evaluate utilization and impact on outcomes.
