Bone remodeling is bone regeneration
process.
Osteoclast a bone eating cell in bone
resorption process.
Osteoblast a bone forming cell in bone
forming process.
RANKL Receptor activator of nuclear
factor kappa-Β ligand.
PTH(parathyroid horomone)

It is of two form;
 Bone resorption
 Bone formation

 In paget disease there is
abnormality in bone
remodeling process
 Osteoclast break down bone
first.
 Osteoblast then build up bone
rapidly forming abnormally large
and deformed bone.
 Etiology is unknown; possibly
viral
 Localized process involving
one or more bones; does not
involve the entire skeleton
 Usually seen in late adulthood
(average age > 60 years)

 Osteolytic(lytic) phase.
 Mixed phase.
 Osteoblastic(scleolitic)
phase.

 Early on; typically no
symptom.
 Over time, the deformed
bone.
 Impinging on nerve couse
pain.
 Impingination on CN VIII
lead to hearing loss.
 Vision loss if CN II is
impinginated.
 Lion-like facies’leontiasis’
 High-output cardiac failure - due to
formation of AV shunts in bone

Drug
 Pain reliever.
 Bisphosphonates:
 Alendronate.
 Risedronate.
 Rankl inhibitor
 Donesumab.
 The use of VIT D have not yet
approved.
 Teriparatide(PTH) has box
warning of osteosarcoma hence
not used.
 Surgery is also essential to
 Decompress the impanginated
nerve.
 Collect bone deformity.
 Reduce associated fracture.
