Intervention
The capstone change proposal is effects of disproportionate nurse to patient staffing ratios on the quality of patient care. Patients can be exposed to several safety issues if proper care is not given to them. These problems include falls, hospital-acquired infection due to poor hand hygiene by the healthcare workers, medication administration errors, poor health education to the patients, and negligence in attending to the spiritual needs of the patients. Interventions includes presenting the safety concerns to the management team of the facility to enable them to hire more nurses to deliver adequate care to the patients. In-service training of the nurses on fall prevention, proper application of fall precautions and identification of patients who are at risk of falls are another important intervention. Proper hand hygiene is an intervention that will prevent hospital-acquired infections and nurses should form the culture of doing it (Sands, & Aunger, 2020). Medication errors can lead to complications or death of patients. Nurses should check the medications properly and identify the patients before administration of the medications.

Decreasing the nursing staff is the only logical way to combat the increased health costs, however, it partly kills the working enthusiasm of the staff.
But, the decreased staff is birthing more problems, as there is an array of patients that come to this center for care and cure and the decreased number of nurses cannot put up with all of them.

The nursing profession is faced with many challenges like any other. The nurse is tasked with the duty to take care of patients by visitation and to have the ability to study the problems to satisfy their patient’s .to to give the nurses the ability to know their work well; they are taking with the work of identification of duties that they are supposed to handle. There have been many challenges facing the nurses in their fold of work .such has been due to the shortage in the workforces as compared to the patient workload. (Lu, et al. 2015). Thus comes the need for the nurse to work for long hours without getting time to rest. When the nurses are overworked, they end up being over-exhausted in which in turn will result in the nurses underperforming.
The problem is crucial because the nurse is needed to take care of the patients. Thus the lesser the nurse who is present the healthcare facilities, the lower the work output that the facilities will meet with. The understaffing of the nursing facilities results in the problem that there are also other factors then come to hand. The lack of the nurses will make the experts present within the facility lack time to do their research due to lack of time.
Time is need for analysis of the data that has been obtained for patients to be done, if the nurse is not enough within the facilities, then that ability of the health care facilitates to allocate the needed number of the nurse to get the analysis of the result will be in lack. Therefore the facility will fail to give the satisfaction required to the public and the patient who are in the facilities. Consequently, the number of nurses who are within many organization are concerned with healthcare needed to be checked. This will help in the elevation of the care with the patients and the wards. (Lu, et al. 2015). He challenges that nurses face in the line of duty need to be taken care of.
The government and the concerned partied need to recruit enough nurses to help in the allocation of the services and the care that is needs by the patients within the diverse and spread health care facilities. (Wang, et al. 2017).
Importance of the research
Researches have been done to try and know the problem that the nurses face when in line with their duty. The study is intended to see the question of the nurse’s face and help in the environment of the issues because then nurses play a care role in the society in the healthcare if the patient and the people in general. Also, the study will help many organizations to have a clear picture of the problems and the challenges that the nurses in the course of doing their work. The shortage in the nursing profession has been an issue for over several years. Especially following COVID-19 suddenly gotten worse. St. Mary Elizabeth Hospital is no exception to this growing issue. Nurses make up the majority of medical practitioners and are essential to the industry. There remains a demand for more skilled educators in the perioperative environment and less even workforce distribution. Many serious factors cause the lack of nurses. As the age increases, there is a greater necessity for medical coverage. The authenticity is that, instead of taking just one illness, senior adults typically have illnesses and founders that necessitate professional care. Overall, individuals exist lengthier, a growing ultimatum for well-being care. Many chronic illnesses that were previously fatal are now treatable (Mar et al., 2019). The baby boom generation is still at a stage where they might need more medical attention as society ages. Today, more incredible Americans than ever previously time in history are above the age of 65.
According to Haddad et al. (2022, disclosed Nursing employment is anticipated to increase by 6% during the following ten years, according to the Bureau of Labor Statistics Number Of simulations 2021–2031. The number of Nurses working in the profession is expected to rise by 195,400 from 3.1 million in 2021 to 3.3 million in 2031. The nursing shortage has been a long-standing problem in the United States and spans eight decades (National League for Nurses, 2017).
Newly licensed nurses (NLNs) are graduate RNs who
have passed the National Council Licensure Exam-RN
(NCLEX-RN®) and are employed for the !rst time in
the role as a professional nurse. NLN turnover has been
reported in recent years to a"ect patient safety and com-
pounds the global nursing shortage (Boamah & Las-
chinger, 2015; Bradbury-Jones, 2015; Kovner, Brewer,
Fatehi, & Katigbak, 2014; Spence Laschinger, Zhu, &
Read, 2016; #omas & Kellgren, 2017; World Health
Organization, 2017). #e American population is liv-
ing longer with chronic diseases and expanding disabili-
ties; more well-prepared RNs are needed as health care
is ever-advancing and technology is at the forefront to
help solve health care problems and improve quality of
life (Academy of Medical-Surgical Nurses, 2018; Ghe-
breyesus, 2018; National Academy of Medicine, 2017;
Reinhard, 2014).
PROBLEM IDENTIFICATION AND SIGNIFICANCE

(2017) reported that 1.2 million RN positions will be
vacant between 2014 and 2022 and that approximately
700,000 nurses will retire or leave the workforce by 2024.
Cline, La Frentz, Fellman, Summers, and Brassil (2017)
abstract
Background: Lack of resiliency contributes to grow-
ing dissatisfaction among newly licensed nurses (NLNs)
and often leads to clinical errors and job resignations.
Method: An integrative review synthesized current re-
search investigating NLNs’ resiliency within their first
year of hire and interventions that may affect their re-
siliency. Results: Key database searches (2008 to 2018)
yielded 16 studies. Insufficient resiliency among NLNs
has been correlated with intentions to leave current
jobs and decreased job satisfaction. Residency pro-
grams, well-prepared preceptors, and peer support
promoted NLN resilience and enhanced patient safety.
Lack of coworker support has led to NLNs’ intentions
to leave their current jobs or the profession entirely.
Conclusion: NLN turnover has been interpreted to be
an outcome of poor NLN resilience. The first year of
practice is stressful and affects NLNs’ mental health
and cognitive reasoning, thereby risking patient safe-
ty. Resiliency should be measured using a resiliency

Introduction
Nursing just like any profession faces numerous challenges daily. Nurses are usually tasked with the duty to ensure that they work diligently and professionally to ensure that their clients are satisfied with the services that they deliver. They are usually tasked with the duty to ensure that the patients that visit their premises get quality services that they deserve. However, these services come at a cost; they face a myriad of challenges that they have to work fully despite these challenges to satisfy their clients. Therefore, this research paper analysis some of the problems that nurses face in the daily execution of their duties.
Identification of the Problem
Nurse faces a myriad of challenges in their daily execution of duties. To ensure that they discharge their duties perfectly all these challenges must be identified and effective measures are put in place to ensure that the challenges are taken care of. The major problems that most of the medical facilities are the issue of understaffing. Nurses have faced challenges in terms of duties assigned to them in comparison to their numbers. Studies conducted have also portrayed that there are shortages of nurses in the medical facilities. However, the challenges that these nurses face are varied in different nations and towns, there are those nations that have surplus while there are those towns and nations that have a high shortage of these important service providers in the healthcare facilities.
The impact of these shortages has also resulted in further challenges for the nurses. In this regard, one of the major challenges that these nurses face is the need to work for long hours due to the shortage of employees at the medical facilities. The few available nurses must work in shifts and endure long working hours to ensure that they meet the desired goals of the institution are met. The work of nurses calls for one to be sober and cognitive in decision making (McLelland et al. 2015). However, with a situation where nurses are required to work for long hours, work in shifts such that they can sleep for a while and resume their duties. This is a worrying trend for the nurses because their cognitive ability and judgment as normal human beings are likely to decline when overworked. Therefore, the chances of making poor decisions are also likely to increase with time as they work overtime.

Medication administration errors(MAE) can threaten patient outcomes and are a dimension of
patient safety directly linked to nursing care. Children are particularly vulnerable to medication errors because
of their unique physiology and developmental needs.
Aims: The present study aims to examine types, stages and causes of medication errors. Barriers of medication
administration errors reporting and its facilitator at pediatric University hospitals from nurses point of view.
Methods: A descriptive study was conducted in Pediatric intensive care units, medical, surgical and urology
ward of children's university hospital at Mansoura University, intensive care units, kidney dialysis at
Abouelrash pediatric hospital and general wards of Elmonaira at Cairo University Hospitals. 80 nurses were
included in the study after fulfilling the criteria of selection. A structured interview questionnaire that consists
of four sections was used.
Results: The highest types of medication errors as reported by studied nurses occurred when the medication is
delivered by the wrong route, the highest stage of medication errors done by nurses was missing of medication
then patient monitoring and administration and the highest cause of medication errors was due to heavy
workload. The results of this study indicated that the strongest perceived barriers to medication administration
errors reporting were fear from consequences of reporting, then managerial factor and then the process of
reporting from the nurse's viewpoint. The nurses agree that identifying benefits of reporting followed agree that
feeling safe about working environment, and agree that good professional relationship with physicians was the
most facilitating factors of reporting medication errors.
Conclusions: It was concluded that medication errors result from interrelated factors, the strongest perceived
barriers to medication administration errors reporting were fear from consequences of reporting, and good
relationship with nurse managers and physicians were the most facilitators of reporting medication errors.
Recommendation: The study recommended that the assessment of medication errors should be done
periodically and in- service training program about medication administrations should be applied

The shortage in the nursing profession has been an issue for over several years. Especially following COVID-19 suddenly gotten worse. St. Mary Elizabeth Hospital is no exception to this growing issue. Nurses make up the majority of medical practitioners and are essential to the industry. There remains a demand for more skilled educators in the perioperative environment and less even workforce distribution. Many serious factors cause the lack of nurses. As the age increases, there is a greater necessity for medical coverage. The authenticity is that, instead of taking just one illness, senior adults typically have illnesses and founders that necessitate professional care. Overall, individuals exist lengthier, a growing ultimatum for well-being care. Many chronic illnesses that were previously fatal are now treatable (Mar et al., 2019). The baby boom generation is still at a stage where they might need more medical attention as society ages. Today, more incredible Americans than ever previously time in history are above the age of 65.
According to Haddad et al. (2022, disclosed Nursing employment is anticipated to increase by 6% during the following ten years, according to the Bureau of Labor Statistics Number Of simulations 2021–2031. 4. participate in the study. Study instrument This study adapted the survey questionnaire
from the Massachusetts Nurses Association Survey on Workplace Violence/ Abuse, with a
permission. The questionnaire was developed in English language, validated and used in a
previous study conducted by the Massachusetts Nurses Association Congress 228 H. A. E.
Alkorashy and F. B. Al Moalad on Health and Safety and Workplace Violence and Abuse
Prevention (MNA 2008). The questionnaire contains 40 structured multiple-choice
questions related to the prevalence of WPV, forms and perpetrators as well as precipitating
factors for WPV. As the official language in the work setting is the English language, the
decision was no translation was required and the questionnaire was used in its original
language. A pilot study on 30 participants had been conducted. Two words were changed:
“Union representative” in question 13, was changed to “hospital security office” because
there is no union in KSA and in question 20, “Employer” was changed to “hospital” and
“tolerating undesirable behaviour, such as violence” instead of “zero tolerance policy”
because the latter was not clear for the most of the subjects in the pilot study. The reliability
for the tool was determined by Cronbach’s alpha (a = 0.7615). Data collection The self-
administered questionnaires were distributed to the participants after receiving approval
to conduct the study from the research/ethical committee and the administration of the
university hospital. The data were collected from March to May 2011. Perceptions of WPV
by violence experience and job category The results of this study revealed that more nurses
who had experienced WPV perceived it as verbal abuse (v2 = 27.838, P < 0.001), and
consider it as a serious problem (v2 = 107.484, P < 0.001) as compared with those who had
not experienced WPV. On the other hand, more nurses who had no experience with WPV
perceived it as physical attacks compared with those who had experienced WPV (v2 =
4.885, P = 0.027) (Table 1). Perpetrators and forms of violence Participants who had
experienced WPV in the 12 months prior to data collection (n = 175) were asked to indicate
the form and source of the violence. From all violence incidents (n = 1103), all the
participants identified patients as the main perpetrators (f = 204), followed by the patients’
relatives (f = 195). Verbal abuse was the most frequently reported forms of violence 435
(38.5%) exhibited by the 99 (56%) physicians, 89 (50.9%) patients and 82 (47.0%)
patients’ relatives. Followed by verbal or written threats, 289 (27.4%); and threatening
behaviours, 226 (19.6%) (Table 2). Precipitating factors of WPV Data analysis The data
were analysed using descriptive and inferential statistics. Spearman’s chi-square test had
been used to compare variables across groups and to investigate the relationship of selected
demographic characteristics with the perceptions of WPV. Several factors contribute to the
emergence of WPV against nurses. These include organizational, individual and situational
