Capstone Project Topic Selection: Improving Workflow for Nurses Working in Primary
Care Settings
Capstone Project Topic Selection and Approval
Problem or Issue
One of the major problems that affect nurses is improving workflow in a busy environment. When nurses are few and, healthcare professionals often face work overload. This has the potential to affect the quality of care given to patients in busy environments. When workload is high, efficiency reduces, thus making nurses vulnerable to errors (Cain & Haque, 2009). The healthcare sector has often faced a lot of demands to develop or reestablish its workflow. In many situations, the desire for evaluating workflow is the need to respond to new ways in which tasks are completed. There are various workflow issues that continue to face nurses in their working environments. They include challenges related to providing services to critically ill patients, and emergence of multidisciplinary teams in care. In such environments, all healthcare professionals struggle with the need to implement change that makes the care team more patient-centered.
Setting/ Context
The context under which this problem is common is primary care setting. Primary care settings are those that provide medical and psychological diagnosis and treatment. Primary healthcare settings are also involved in the provision of personal support for patients of all backgrounds, and in all stages of illness (Goroll & Mulley, 2012). In primary healthcare settings, nurses and other professionals are involved in the communication of information about prevention, diagnosis, treatment, and prognosis, as well as the prevention and care of chronic disease and disabilities through risk evaluation, health education, and early disease detection.
High-Level Detail of the Problem
Nurses operating in primary healthcare settings are often overburdened with many tasks that interfere with the workflow. This is particularly true when they are working in multidisciplinary teams that require coordination and cohesion. In such cases, it might be difficult to clearly determine each professional’s roles and responsibilities (Hickey & Kritek, 2011). At the same time, when the number of nurses is few, the available one’s face stress and physical strain that might interfere with the quality of care given to patients and increase likelihood of errors. In an environment where technological interventions do not sufficiently meet the objectives of healthcare groups, it might also cause workflow issues. Such alternative flows arouse worries since the non-formal mechanisms depend upon the health professionals’ memory and may overlook the safety systems that might be offered.
Impact of the Problem
Workflow issues often generate vinous negative impacts on both the nurses and the patients. For instance, poor coordination among healthcare professionals can result in errors that pose many harms to the pa ...

The capstone change proposal is effects of disproportionate nurse to patient staffing ratios on the quality of patient care. Patients can be exposed to several safety issues if proper care is not given to them. These problems include falls, hospital-acquired infection due to poor hand hygiene by the healthcare workers, medication administration errors, poor health education to the patients, and negligence in attending to the spiritual needs of the patients. Interventions includes presenting the safety concerns to the management team of the facility to enable them to hire more nurses to deliver adequate care to the patients. In-service training of the nurses on fall prevention, proper application of fall precautions and identification of patients who are at risk of falls are another important intervention. Proper hand hygiene is an intervention that will prevent hospital-acquired infections and nurses should form the culture of doing it (Sands, & Aunger, 2020). Medication errors can lead to complications or death of patients. Nurses should check the medications properly and identify the patients before administration of the medications.
Evidence Based Literature
The articles reviewed have different research aims and questions, but they are all centered into the idea of the effects of nurse-to-patient ratios on patient outcomes. The research questions of these articles are divided into three categories: definition of nursing staffing, effects of nursing-to-patient ratio on patient outcomes and nursing characteristics that hinders the delivery of care. The study by (Cho et al., 2020), defines the term nursing staffing in terms of the nursing care needs of the patients.
Nurses are essential in the provision of quality care in acute units, and their staffing levels have an impact on patient outcomes. (Cho et al., 2015), examine the link between nursing staffing and patient outcomes, specifically the mortality rate. Comparing to (Driscoll et al., 2018) and (Shin et al., 2018), the articles examine the effects of nursing staffing ratios on the patient outcomes in acute specialist units. Besides, (Needleman, 2016) reviews the studies that examine the effects of nursing skill mix on the patient outcomes such as patient ratings of hospitals, mortality, adverse health outcomes, and nurse burnout and dissatisfaction.
Some of the factors such as nursing skills, staffing methods, and working environment affects the nursing staffing ratio, which hinders the quality of care. The article by (Bridges et al., 2019), explores the relationship between nursing staffing skills and the quality and quantity of their interactions with patients in hospital wards. Annotated Bibliography
We have seen that in a health-care organization there are multiple responsibilities
including assessments of priorities in areas that require attention in an organization. The
following annotated bibliography compiles a summary of the references that will be used
to prepare a final paper that will identify nursing leadership priorities at Fantastic
Medical Center (FMC) and proposed resources for addressing the priorities.
Previously identified nursing leadership priorities at FMC that will be the focus of
the literature review are (a) nursing staff turnover, (b) nursing staff competency, and (c)
medication errors of omission. Accordingly, the literature and available resources were
searched utilizing terms that included the above listed priorities and terms to reflect any
proposed resources for addressing the priorities in order to support strategies with
evidence-based references.
Additional references will include readings from the course that involve systems
leadership, organizational structure, and use of a systems-based approach for analysis of
organizational issues. These may include Lewin’s change theory, or the diffusion of
innovation theory. The synthesis of these references will help me build the foundation of
to address strategies that can be used within FMC, both as an analysis for the project.

Abstract— If job satisfaction is there in employees, work done by these employees is usually of better quality in comparison where the employees are not satisfied with their jobe. So this study to assess job satisfaction and influence of demographic variables on job satisfaction, this study was carried out on 105 doctors of teaching hospitals. Questionnaire method of data collection was adopted. Job satisfaction was measured by six domains: Organizational functioning, Interpersonal relationship, Financial incentives, Non-financial incentives, Physical facilities and Working conditions. Study observed that over all, doctors were moderately satisfied with their job. Domains such as Interpersonal relationship and Working conditions, doctors were highly satisfied, whereas rest of the domains: Organizational functioning, Financial incentives, Non-financial incentives, and Physical facilities doctors were moderately satisfied. It is important to note that even though overall satisfaction is moderate, there were few components, where doctors were highly satisfied were - Communication system between patients and doctors, Involvement in decision making in the department, Rules and regulations of the institution, relationship between the department colleagues and other department colleagues, Provision for leave encashment, reward given for research work, workload of clinical aspect and workload of teaching aspect. Age and sex both shown significant association on level of satisfaction where as experience, designation and marital status of the doctors have not shown significant association.
This document discusses the importance of evidence-based practice in nursing. It begins by explaining how evidence-based practices have reverberated across nursing practice, education, and science. The need for improved healthcare calls for evidence-based practices to be incorporated into health systems to increase effectiveness, safety, and efficiency. New practice approaches should be evidence-based to help move healthcare in the desired direction. The document also provides an example of how one facility successfully reduced hospital-acquired pressure ulcers through implementing evidence-based skin assessment and wound care protocols. It concludes by stating the importance of evidence-based practices in tackling issues like hospital-acquired pressure ulcers.
The document discusses assessing an organization's culture and readiness for implementing an evidence-based practice project to prevent CLABSI in a NICU. An organizational assessment tool was used to evaluate readiness. It found strengths like leadership support but also weaknesses such as lack of time and EBP knowledge. The author recommends strategies to improve readiness like recognizing top performers and aligning with available resources. Key stakeholders for the project include nurses, doctors, patients, and insurance companies. Information technologies could help implementation and care delivery if integrated properly.
Nurse Burnout and Quality of Care Word Review Paper HW.pdfbkbk37
This document provides a literature review on nurse burnout and quality of care. It discusses several research articles on this topic. The key findings are:
1) Higher levels of nurse burnout correlate with lower levels of quality patient care.
2) Nurse burnout can lead to job dissatisfaction, which is associated with missed or reduced nursing care, especially in nursing homes.
3) Individual, group, and organizational interventions can help reduce nurse burnout, but group strategies seem most effective. Further research is needed to evaluate the limitations of various intervention strategies.
Improving work environments and empowering nurse leaders are recommended to reduce burnout and improve the quality of nursing care.
Decision-Making in Nursing LeadershipNursing leadership requires.docxvickeryr87
Decision-Making in Nursing Leadership
Nursing leadership requires critical thinking and high cognitive ability as one has to make decisions regarding patient care, nurses' welfare, and team development. Nurse leaders make decisions regarding nursing practice, evidence-based practice, and points of advocacy in my organization.
While nurse leaders focus on ensuring nurses perform per the organization's expectations, they also advocate for nurses' and patients' welfare. For instance, nurse leaders had to advocate for nurses' safety, especially as they handle COVID-19 patients, ensuring they get the necessary personal protective equipment (Majers & Warshawsky, 2020). Ensuring nurses have adequate personal protective equipment further guarantees patients' safety and improves the quality of care as there is no transmission of the virus from one patient to another.
Nurse leadership focuses on issues affecting nurses, patients, and the healthcare organization. As a result, effective leadership among nurse leaders secures their decision-making on healthcare setting issues (Sfantou et al., 2017). Effective leaders identify problems in their space, gather views on how these problems can be solved, and communicate the recommendations with nurses and organizational leaders. As a nurse leader, I can be invited to the table and be an active participant in decision-making by demonstrating effective leadership and being proactive in finding amicable solutions to arising issues.
In summary, my organization makes decisions in a centralized manner where leaders decide on the way forward and communicate the recommendations to team members. While this organizational structure appears inclusive for all leaders, only effective leaders take the lead in making critical decisions regarding practice in their areas of practice.

In nursing, proper decision-making is essential because it aids in the collection, storage, and processing of data and the provision of knowledge and information. In carrying out and managing their activities and when working with patients, nurses depend on decisions made across all managerial levels (Namnabati et al., 2017). As one becomes more knowledgeable of the profession's decision-making process, new responsibilities and tasks emerge in the nursing sector. As a result, nurses will have to choose the right decision that best suits their needs (Ellis, 2017).Because of the scope and variety of nature programs in health care, nursing leaders must first consider the underlyin.
The document discusses the roles and responsibilities of a DNP-prepared nurse educator. It explores how a DNP nurse educator can teach both in academic and clinical settings while also bridging the gap between nursing education and practice. It then presents a PICOT question regarding the implementation of a diabetes self-management education program and examines strategies to address barriers to effective diabetes management.
The document discusses the roles and responsibilities of a DNP-prepared nurse educator. It compares the activities of an academic DNP nurse educator, who teaches graduate nursing students, to a clinical DNP nurse educator, who provides education in a healthcare setting. The document also proposes a case study involving implementing a Diabetes Self-Management Education and Support program to impact patients' fasting blood sugar and self-management skills over 8-10 weeks. Barriers to diabetes self-management are discussed, as well as strategies to address those barriers through various forms of patient education.

The literature review's purpose is to highlight what has already been researched in terms of active intervention that can be applied in addressing burnout and improving the quality of care provided.
In identifying the literature that will be used in the study, the search involved an inclusion and exclusion criteria. In the inclusion criteria, all the studies had to be not older than five years, written in English, peer-reviewed, and addressing burnout in healthcare facilities. The databases that were used are BMC, PubMed, Google Scholar, and Science Direct. The search utilized keywords that include “burnout,” “healthcare quality,” “patient satisfaction,” “patient safety,” “Compassion Fatigue,” and “COVID-19.” The literature review addressed the conceptual framework of the study, intervention strategies, compassion fatigue (CF) and burnout, COVID-19 and burnout, burnout and patient safety, and burnout and patient satisfaction.
Theoretical Framework
The theoretical framework that will be applied in the research is the Quality Health Outcomes Model (QHOM). QHOM touches on several aspects that show the association between the context or system, client characteristics, healthcare interventions, and patient outcomes. The model posits that the quality of the provider's services is essential in ensuring excellent outcomes and patient satisfaction.
DesHarnais (2011) provides a conceptual framework and definitions of quality. The author states that Donabedian provided definitions of quality that reflect the goals and values of the current medical care system and those that encompass the broader society it serves. DesHarnais (2011) writes that Donabedian used three aspects of care, namely, structure, process, and outcomes. Rebar (2019) defines
quality, as well as its implications in the provision of care. The author provides the National Academies of Sciences, Engineering, and Medicine definition of healthcare quality. Healthcare quality is defined as the degree to which healthcare services for populations and individuals increase the likelihood of desired health outcomes consistent with current professional knowledge. Rebar writes that quality entails timeliness, patient-centeredness, equity, efficiency, and effectiveness. Patient satisfaction that is connected to the perceived quality of care drives market competition and reimbursement.Burnout Survey
There are several burnout surveys available to use for assessing the level of burnout with staff. According to Halbesleben and Demerouti (2005), Maslach Burnout Inventory (MBI) is most commonly used, however, researchers have criticisms. The MBI uses three subscales of measurement: emotional exhaust.

Managed care refers to the practice of managing some aspects of the medical services i.e. the cost and the quality of the medical services. Managed care can be managed by healthcare providers when providing healthcare services. Utilization management refers to the processes or the activities that are involved in the management of care. for example Uutilization management is relied on in terms of managing the cost in the health plan. It also refers to the routine functions which are utilized for the management of the cost of the medical services. Utilization management is entailing aspects such as prospective, concurrent, and the retrospective review. A prospective review is involving the process of tackling the utilization before its occurrence; con-current review is addressing the utilization as it happens; whereas the retrospective review occurs after the fact. Case management, on the other hand, is relied on in delivering substantial savings. In case of management, trained nurses are relying on the nurses to help in the coordinating the aspects of care like rehabilitation, home care, and health education thus helping in the improvement of the outcomes and the reduction of the expenses (Peter, 2019). Executive Summary
Medication error being a systematic problem is not a new case. It is something that nurses and other healthcare providers have experienced. The error could be a result of recurring issues of just human error. It could also lead to more severe injuries where the patient could find themselves with a new condition such as itching rashes or skin problems that could either be temporary or permanent. These medication errors could even lead to death in some cases, significantly hurting the patient's family, especially when they know that the death of their loved one could have been prevented. It could make the healthcare provider responsible for the error start doubting themselves and start feeling guilty and ashamed for the action. It could even lead to depression throughout their life. If the deceased's family decides to file a lawsuit against the nurse or the one who was responsible, it could result in them losing their license. Medication errors can also impact the hospital where the healthcare provider is working, and patients even lose trust in the kind of treatment offered in that hospital. The occurrence of medication errors could cause the organization not to achieve its goals and objectives of providing quality care to patients for better results. Since human is to error, a medication error of not greater than 5% is allowed, but currently, it is at 39.5% (Barker et al. 2020)
As a nursing leader, I would recommend using various strategies to help minimize the occurrence of such medication errors in the future. The management should make it their responsibility to establish a safety culture and constantly report the current system and how it is performing. Healthcare providers should understand that humans are to error, and no one is to be blamed or receive a harsh form of punishment. The healthcare provider responsible for the medication error should acknowledge their mistake and report to the nursing leaders or the supervisor in charge to ensure patient safety before things get worse.

NURS6247 Phoenix Nurse Burnout Issue in Healthcare.docxstirlingvwriters
Nurse burnout is a significant issue that negatively impacts both nurses and patients. It occurs when nurses experience physical, emotional, and psychological stress or strain from their work. This can lead to feelings of detachment, lack of motivation, and diminished emotions. Left unaddressed, nurse burnout can become a permanent problem. It is linked to poorer patient outcomes, higher medical errors, and decreased patient and nurse satisfaction. The document discusses the need for healthcare organizations to recognize nurse burnout using tools like the Maslach Burnout Inventory and implement interventions to address its causes and prevent its effects on nurses and quality of care.
Jennifer Hopes has chosen to focus her senior project on challenges with recruiting and retaining healthcare professionals. She discusses the increasing demand for primary care physicians due to factors like healthcare reforms and an aging population. However, rural areas struggle with recruitment due to low populations and incentives. Her proposed training program will address retention issues like lack of appreciation, poor feedback, and unsupportive cultures. It will teach leadership effective communication and talent management strategies. The program aims to benefit the organization by improving quality of care, patient satisfaction, costs, and access to services by sustaining an engaged healthcare workforce.

Genetic diversity is important for long-term population viability and supports biodiversity. A lack of genetic diversity threatens survival, as seen in populations like the Florida panther which faces health issues due to inbreeding. Population management considers conservation values and aims to balance costs and benefits through efforts like reintroducing invasive species. Conservation methods include ex situ options like zoos and botanical gardens which support genetics but can also reduce diversity, as well as in situ protections under the Endangered Species Act which helps preservation but faces challenges in enforcement.

Virtualization and cloud services continue to gain momentum as more.docxsdfghj21
Virtualization and cloud services are gaining popularity as organizations seek to reduce costs and improve efficiency. As cited by Portnoy (2012), companies typically adopt virtualization first before purchasing new physical servers. Under virtualization policies, new projects are satisfied using virtual resources instead of physical servers unless a need cannot be met virtually. Organizations begin to virtualize infrastructure services and realize fiscal benefits, leading to migrating more workloads from expiring servers to the growing virtual environment.

Advanced control scheme of doubly fed induction generator for wind turbine us...IJECEIAES
This paper describes a speed control device for generating electrical energy on an electricity network based on the doubly fed induction generator (DFIG) used for wind power conversion systems. At first, a double-fed induction generator model was constructed. A control law is formulated to govern the flow of energy between the stator of a DFIG and the energy network using three types of controllers: proportional integral (PI), sliding mode controller (SMC) and second order sliding mode controller (SOSMC). Their different results in terms of power reference tracking, reaction to unexpected speed fluctuations, sensitivity to perturbations, and resilience against machine parameter alterations are compared. MATLAB/Simulink was used to conduct the simulations for the preceding study. Multiple simulations have shown very satisfying results, and the investigations demonstrate the efficacy and power-enhancing capabilities of the suggested control system.

Embedded machine learning-based road conditions and driving behavior monitoringIJECEIAES
Car accident rates have increased in recent years, resulting in losses in human lives, properties, and other financial costs. An embedded machine learning-based system is developed to address this critical issue. The system can monitor road conditions, detect driving patterns, and identify aggressive driving behaviors. The system is based on neural networks trained on a comprehensive dataset of driving events, driving styles, and road conditions. The system effectively detects potential risks and helps mitigate the frequency and impact of accidents. The primary goal is to ensure the safety of drivers and vehicles. Collecting data involved gathering information on three key road events: normal street and normal drive, speed bumps, circular yellow speed bumps, and three aggressive driving actions: sudden start, sudden stop, and sudden entry. The gathered data is processed and analyzed using a machine learning system designed for limited power and memory devices. The developed system resulted in 91.9% accuracy, 93.6% precision, and 92% recall. The achieved inference time on an Arduino Nano 33 BLE Sense with a 32-bit CPU running at 64 MHz is 34 ms and requires 2.6 kB peak RAM and 139.9 kB program flash memory, making it suitable for resource-constrained embedded systems.

Low power architecture of logic gates using adiabatic techniquesnooriasukmaningtyas
The growing significance of portable systems to limit power consumption in ultra-large-scale-integration chips of very high density, has recently led to rapid and inventive progresses in low-power design. The most effective technique is adiabatic logic circuit design in energy-efficient hardware. This paper presents two adiabatic approaches for the design of low power circuits, modified positive feedback adiabatic logic (modified PFAL) and the other is direct current diode based positive feedback adiabatic logic (DC-DB PFAL). Logic gates are the preliminary components in any digital circuit design. By improving the performance of basic gates, one can improvise the whole system performance. In this paper proposed circuit design of the low power architecture of OR/NOR, AND/NAND, and XOR/XNOR gates are presented using the said approaches and their results are analyzed for powerdissipation, delay, power-delay-product and rise time and compared with the other adiabatic techniques along with the conventional complementary metal oxide semiconductor (CMOS) designs reported in the literature. It has been found that the designs with DC-DB PFAL technique outperform with the percentage improvement of 65% for NOR gate and 7% for NAND gate and 34% for XNOR gate over the modified PFAL techniques at 10 MHz respectively.

PreviewRunning head: PICOT QUESTION 1 PICOT Question Name Institution PICOT
QUESTION 2 PICOT Question The healthcare system involves the implementation of
strategies that are effective in enhancing the delivery of quality health services. The
problem of nursing burnout, however, affects the delivery of effective and efficient health
services. For example, nurses in critical care units are faced with the challenge of burnout,
which tends to affect productivity and patient satisfaction in the essential departments
(Baranda, 2017). The PICOT question will help in evaluating the nursing and evidence-
based solutions that would be effective in avoiding nursing burnout. PICOT Statement How
do critical care nurses (P) prevent burnout (I) through self-care (C) in enhancing patient
satisfaction (O) during clinical practices (T)? Evidence-based Solution Nursing burnout in
critical care units can be prevented through the implementation of evidence-based practices
that are considered useful in enhancing clinical operations. The evidence indicates that
nursing burnout is caused by emotional exhaustion and depersonalization. An evidence-
based approach was used to assess the correlation between nurses’ burnout and the
intensity of critical care unit operations (Greives, 2016). For example, the evidence
approach indicated that nurses’ burnout is caused mainly by the nurse-to-patient ratios,
poor evidencebased clinical practices among the nurses, and incompetence of nursing
leadership (Naves, 2016). Therefore, the solutions incorporated the elements of nursing
leadership, increased rates of nurse-to-patient ratios, and implementation of evidence-
based clinical practices. Nursing Intervention PICOT QUESTION 3 Moreover, nurses’
burnout in critical care units should be prioritized based on the magnitude and essential
2. operations of the department. Nurses should be keen on preventing the burnout that may
affect the quality of service in the critical care units. For example, nursing interventions for
nurses’ burnout can involve the prevention of physical and mental health of the nurses
(Greives, 2016). Nurses should welcome the elements of self-care, which is vital in
protecting one’s mental and physical health. Also, nursing interventions can involve
employee assistance programs such as on-site and off-site retreats, break-out sessions,
stress-reduction programs, mentoring programs, uses of teamwork to avoid monotonous,
rewards and recognition, offering staff , management education and training programs,
and encouraging staff to raise their concerns in the work environment (Baranda, 2017).
Nursing interventions are created within the health organization whereby the management
and the employees collaborate to promote the work environment that would reduce
burnout. Patient Care Furthermore, nurses’ burnout has portrayed adverse effects on the
clinical operations and the outcomes of healthcare service delivery. The results of nurses’
burnout have affected the quality of healthcare services in many ways. Success of
Healthcare System Literature ReviewFor example, nurses with the problem of fatigue are
faced with the challenge of physical exhaustion, sleep deprivation, and depression, which
will affect the nurses in engaging with the patients (Rios-Risquez & Garcia-Izquierdo, 2016).
Patients in the critical care units require extensive care and attention from the nurses, and
any form of neglect can attract high rates of mortality. Nurses’ burnout will also affect the
patient and family satisfaction, which would cause withdrawals from a healthcare facility or
adverse reports raised by the patients’ family members. Nurses’ burnout also jeopardizes
patient safety. Health Care Agency PICOT QUESTION 4 Additionally, nursing practices and
the issues of burnout can be prevented by the agencies that represent the healthcare system
in policy-making procedures. Agencies such as government health care departments and
legislative representing the health system would conduct research and recommend
methods that would be deemed competent in burnout prevention. For example, the
healthcare system can ensure that the patient-nurse ratio is updated to prevent the fatigue
and to overwork of the nursing practitioners (Shenoi et al., 2018). Also, healthcare agencies
and decision-makers should avoid the shortage of human resources, enhance the
availability of healthcare practice resources, and improve management procedures that are
effective in strengthening care provisions and prevention of employee burnout. Nursing
Practice Nurses have the mandate of preventing the problems associated with employee
burnout. Nursing practitioners should enhance and improve their work schedules and
collaborate with other professionals in strategies that would prevent burnout. For instance,
nurses should increase the approaches of engaging with healthy and physical activities,
taking notes of the stressors and evaluating procedures of avoiding them, a delegation of
tasks where possible, setting boundaries in the work environment, avoidance of frequencies
and daily unhealthy activities, setting parameters, practice gratitude and seek from clinical
leaders (Ghavidel et al., 2019). The procedures will be useful in demonstrating the
importance of evidence-based approaches to preventing burnout and fatigue. PICOT
