The hospital billing system is a complex tool utilized to provide accurate accounts of patient services. The fundamentals of which address the people using the system, components, processes, data/information and technology. When properly used the billing system is an effective tool for the procurement of financial obligations for services rendered, but when accessed in the wrong way it has the ability to hurt the hospital, patients, employees or all of the above.
People:
Any person in contact with a patient is potentially one who uses the billing system and subsequently any person in contact with patients, whether direct or indirect, are responsible for billing. Doctors, nurses, pharmacists and secretaries are responsible for ensuring that charges are properly submitted for billing, while indirect patient services such as EVS and food service costs are built into the cost of hospitalization. Everyone involved in the usage of the billing system, as well as the patients, are affected by the system. Missed charges related to patient care can cause a financial deficit and negatively impact the staff in ways such as missed raises and poor working conditions. Extra and wrong charges can cost the patient extra money as well.
Organizational Components
: All departments use the billing system in a hospital. Central Electronic Health Records can be accessed across the hospital ensuring that charges are properly entered and ultimately submitted to the insurance companies for reimbursement. For example when a patient leaves a unit for a procedure or test, technicians can access the EHR to enter charges, which all members of the health team can access. This spreads the accountability across all members of the health care team.
Processes:
Data entry is the primary process in which all users of the billing system are engaged. Without the entry of data into the hospitals EHR, charges cannot be determined and therefore revenue for the hospital can be lost.
Data/Information:
Many data elements are important in the billing system but the top 5 are: Patient identification, patient demographic information(billing address, contact info), insurance information, dates of service/services rendered and service providers identification. Without these key elements there is a potential for missed revenue and lack of payment.

Electronic Health Record (EHR) is continuing to evolve in today’s medical facilities. The American Recovery and Reinvestment Act mandated health organizations to transition to Electronic Medical Records (EMR) by January 1, 2014 in order to maintain Medicaid and Medicare reimbursement (USF Health, 2019). For this week’s discussion I will reflect on the module resources on digital information tools and technologies. I will discuss the healthcare technologies used in the health organization I work for. Lastly, I will reflect on any possible health technologies and the impact it has on nursing practice and healthcare delivery.
The health organization I work for switched to EPIC software for EMR two years ago. EPIC is user friendly for nurses as well as patients. EPIC corresponds to MyChart giving patients access to on their medical records (MyChart, 2019). I work in outpatient Endoscopy clinic where part of patient After Visit Summary (AVS) we educate patients on the use of MyChart and provide them with a code so they can access MyChart from home. Patients can access their medical records, and view lab results, make appointments, communicate with their provider, and pay their medical bills with MyChart (2019). ProVation is another form of technology used in the facility I work for. ProVation is used by the physicians where they document the Endoscopy procedure outcomes and results noted by the physician (ProVation, 2019). The physicians can document their findings as well as list out orders or management solutions for the patients (ProVation, 2019).
EHR has made it possible for nurses to provide efficient patient care as it has given us the ability to share patient information with other providers and health care organization departments, such as pharmacy, laboratory, etc (HealthIT, 2018). Patient care and experiences are improving because patients are being included in their care and they can make decisions in the plan of care. Having quick access to medical records and information results in increased patient satisfactions. Also, by combining patient portals inpatient healthcare facilities has resulted in a decrease of medical errors and adverse events (Dyes et al, 2017).
EHR poses a list of challenges with the main one being security safety. Web-based technology does put us at increased risk of breach of information by hackers. It can also be challenging for individuals to navigate web-based health technology, resulting in decreased patient satisfaction. Our older population is reluctant when it comes to using technology, they prefer paper written information. Documentation errors are associated with improper utilization, due to insufficient training (David, 2017).

The hospital billing system is a complex tool utilized to provide accurate accounts of patient services. The fundamentals of which address the people using the system, components, processes, data/information and technology. When properly used the billing system is an effective tool for the procurement of financial obligations for services rendered, but when accessed in the wrong way it has the ability to hurt the hospital, patients, employees or all of the above.
People:
Any person in contact with a patient is potentially one who uses the billing system and subsequently any person in contact with patients, whether direct or indirect, are responsible for billing. Doctors, nurses, pharmacists and secretaries are responsible for ensuring that charges are properly submitted for billing, while indirect patient services such as EVS and food service costs are built into the cost of hospitalization. Everyone involved in the usage of the billing system, as well as the patients, are affected by the system. Missed charges related to patient care can cause a financial deficit and negatively impact the staff in ways such as missed raises and poor working conditions. Extra and wrong charges can cost the patient extra money as well.
Organizational Components
: All departments use the billing system in a hospital. Central Electronic Health Records can be accessed across the hospital ensuring that charges are properly entered and ultimately submitted to the insurance companies for reimbursement. For example when a patient leaves a unit for a procedure or test, technicians can access the EHR to enter charges, which all members of the health team can access. This spreads the accountability across all members of the health care team.
Processes:
Data entry is the primary process in which all users of the billing system are engaged. Without the entry of data into the hospitals EHR, charges cannot be determined and therefore revenue for the hospital can be lost.
Data/Information:
Many data elements are important in the billing system but the top 5 are: Patient identification, patient demographic information(billing address, contact info), insurance information, dates of service/services rendered and service providers identification. Without these key elements there is a potential for missed revenue and lack of payment.

Electronic Health Record (EHR) is continuing to evolve in today’s medical facilities. The American Recovery and Reinvestment Act mandated health organizations to transition to Electronic Medical Records (EMR) by January 1, 2014 in order to maintain Medicaid and Medicare reimbursement (USF Health, 2019). For this week’s discussion I will reflect on the module resources on digital information tools and technologies. I will discuss the healthcare technologies used in the health organization I work for. Lastly, I will reflect on any possible health technologies and the impact it has on nursing practice and healthcare delivery.
The health organization I work for switched to EPIC software for EMR two years ago. EPIC is user friendly for nurses as well as patients. EPIC corresponds to MyChart giving patients access to on their medical records (MyChart, 2019). I work in outpatient Endoscopy clinic where part of patient After Visit Summary (AVS) we educate patients on the use of MyChart and provide them with a code so they can access MyChart from home. Patients can access their medical records, and view lab results, make appointments, communicate with their provider, and pay their medical bills with MyChart (2019). ProVation is another form of technology used in the facility I work for. ProVation is used by the physicians where they document the Endoscopy procedure outcomes and results noted by the physician (ProVation, 2019). The physicians can document their findings as well as list out orders or management solutions for the patients (ProVation, 2019).
EHR has made it possible for nurses to provide efficient patient care as it has given us the ability to share patient information with other providers and health care organization departments, such as pharmacy, laboratory, etc (HealthIT, 2018). Patient care and experiences are improving because patients are being included in their care and they can make decisions in the plan of care. Having quick access to medical records and information results in increased patient satisfactions. Also, by combining patient portals inpatient healthcare facilities has resulted in a decrease of medical errors and adverse events (Dyes et al, 2017).
EHR poses a list of challenges with the main one being security safety. Web-based technology does put us at increased risk of breach of information by hackers. It can also be challenging for individuals to navigate web-based health technology, resulting in decreased patient satisfaction. Our older population is reluctant when it comes to using technology, they prefer paper written information. Documentation errors are associated with improper utilization, due to insufficient training (David, 2017).

Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.
O estabelecimento da amamentação e o manejo das principais intercorrências é contemplada.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

Health card consists of bar code to scan and patient’s demographic data and patient’s medication are displayed in screen when the bar code is scanned. Looking through its benefits, through health card we can avail cashless hospitalization. Health card serves as an identity for individual for their treatment. Through this technology we can avoid medication errors, abbreviations errors and cost related to furniture, racks, cupboard etc., in pharmacy sector.
