This case report describes the first documented case of isolated macrodontia (enlarged tooth) of an impacted upper second premolar. A 12-year-old female presented with a persistently unerupted upper right second deciduous molar and underlying permanent second premolar. Radiographic imaging revealed the premolar had an unusually large molar-like crown with two short roots. The macrodontic premolar and deciduous molar were surgically extracted. At over 10mm in width and 15mm in length, the premolar was significantly larger than normal. Orthodontic treatment closed the post-extraction space without complications. malocclusion. The treatment plan was to correct the malocclusion in a way that torques the roots
more onto bone and to change her dental hygiene methods. With an extensive review of the
literature, this case review attempts to make sense of the enigma of gingival recession and
demonstrates an excellent treatment solution to concomitant orthodontic and periodontal
problems.

Partial or complete edentulism has multiple implications in relation to function, esthetics and future rehabilitative treatment. This case report illustrates the management of a patient with extreme consequences of partial edentulism in the maxillary arch and total edentulism in the mandibular arch. The main clinical findings were unopposed remaining teeth, over eruption of the remaining teeth, loss of vertical dimension of occlusion, and significant disfigurement of the occlusal plane. Following the diagnostic procedure, a well-coordinated prosthodontic treatment involving liaison with other dental disciplines was indicated. The management involved an innovative combination of fixed and removable prostheses in conjunction with intentional root canal therapy of the remaining natural teeth. Series of provisional prostheses were applied to facilitate the transition to the final treatment.

Endodontic Management of Unusual Case of Type II Dens Invaginatus – A Case Re...QUESTJOURNAL
ABSTRACT: A thorough knowledge of the normal anatomy of the root canal system as well as the possible aberrancies is imperative for success of endodontic therapy .The challenge lies in diagnosing these conditions properly, complete debridement of the root canal system, and 3-dimensional sealing of the same. Morphological dental anomalies of the maxillary lateral incisors are relatively common . Endodontic treatment for teeth that exhibit the dental anomaly, dens invaginatus, can be difficult due to the bizarre anatomy and relative inaccessibility of the diseased pulp tissue. Dens invaginatus is a developmental malformation of teeth. Affected teeth show a deep infolding of enamel and dentine starting from the foramen caecum or even the tip of the cusps and which may extend deep into the root. . The malformation shows a broad spectrum of morphologic variations and frequently results in early pulp necrosis. Dens invaginatus is a critical condition for endodontic treatment once it frequently presents a complex internal anatomy and might be associated with incomplete root and apical development. Therefore every effort must be made to identify and diagnose the variation. This article presents an Unusual case of Endodontic Management of maxillary left lateral incisior with Type II Dens Invaginatus .

Abstract: Congenitally missing lateral incisors create an esthetic problem with specific orthodontic and prosthetic considerations. Selecting the appropriate treatment option depends on many factors, such us the malocclusion, the anterior relationship, specific space requirements, bone volume, root proximity, the condition of the adjacent teeth, and esthetic prediction mainly when the canine must be reshaped.Resin bonded bridges were considered to be doomed owing to their very high decementation rate, have come alive once again because of newer resin based cements. This article will discuss the variety of treatment managements in case of space opening and treated with two 2-unit cantilevered resin-bonded fixed partial dentures supported by the cuspids. This conservative treatment plan was cost-effective without having any significant biological cost. Keywords: Agenesis, Resin- bonded fixed partial denture, interim prosthesis.

Agenesis, the absence of permanent teeth, is a common occurrence among dental patients. The total incidence of tooth agenesis is about 4.2% among patients that are seeking orthodontic treatment and with the exception of third molars, the maxillary lateral incisors are the most common congenitally missing teeth with about a 2% incidence. The maxillary lateral incisor is the second most common congenitally absent tooth. There are several treatment options for replacing the missing maxillary lateral incisor, including canine substitution, tooth-supported restoration, or single-tooth implant. Dental implants are an appropriate treatment option for replacing missing maxillary lateral incisor teeth in adolescents when their dental and skeletal development is complete. This case report presents the treatment of a patient with congenitally missing maxillary lateral incisor using dental implants. The paper discusses the aspects of pre-prosthetic orthodontic diagnosis and the treatment that needs to be considered with conservative and fixed prosthetic replacement.

Abstract: Advances in bonding techniques and materials allow for reliable bracket placement on ectopically positioned teeth. This prospective study evaluates the outcome of forced orthodontic eruption of impacted canine teeth in both palatal and labial positions. Eighty-two impacted maxillary canines in 2200patients were included in the study and were observed for 2006 to 2013 ,in Center for Dentistry research and Aesthetics, Jatt/Israel after exposure. Following exposure by means of a palatal flap or an apically repositioned buccal flap, an orthodontic traction hook, with a Titanium Button with chain by Watted (Dentaurum) attached, was bonded to each impacted tooth using a light cured orthodontic resin cement. A periodontal dressing was placed over the surgical site for a period of time. All teeth were successfully erupted. Complications consisted of: failure of initial bond, at the time of surgery, which required rebonding; premature debonding at the time of pack removal and; debonding of brackets during orthodontic eruption. There was no infection, eruption failure, ankylosis, resorption or periodontal defect (pocket greater than 3 mm) associated with any of the exposed teeth. Forced orthodontic eruption of impacted maxillary canines with a well bonded orthodontic traction hook and ligation chain, used in conjunction with a palatal flap or an apically repositioned labial flap, results in predictable orthodontic eruption with few complications. The main purpose of our study is to present the corrective movement of impacted canines using various surgical-orthodontic techniques Materials and method: Eighty-two impacted maxillary canines in 2200patients were included in the study and were observed for 2006 to 2013 ,in Center for Dentistry research and Aesthetics, Jatt/Israel after exposure. Following exposure by means of a palatal flap or an apically repositioned buccal flap, an orthodontic traction hook, with a Titanium Button with chain by Watted (Dentaurum) attached, was bonded to each impacted tooth using a light cured orthodontic resin cement. For this study we used only the batch of patients who presented upper impacted canine. Depending on the clinical status, we used the following surgical techniques: repositioned flap, gingival translation flap, window flap method and local mesh application. After surgery for 39 patients we considered that canine traction with an orthodontic device was necessary in order to obtain a vertical position of the teeth. The orthodontic systems used were: fixed orthodontics, with a Titanium Button with chain by Watted (Dentaurum). Results and discussion: We used the repositioned flap for 39patients with deep impacted canines in order to uncover the teeth and to bond an auxiliary orthodontic device, the gingival translation flap for 27 patients with superficial impacted canines: 10cases with apical translation and 2 with lateral and apical translation. The window flap was used for 22 patients with palatal impaction. After surgery all patients continued orthodontic treatment in order to correct every dental malposition and to obtain a neutral occlusion with esthetical, functional and stabile results.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).

Abstract:
Introduction:Isolated macrodontia of a second premolar is a very rare dental anomaly that has been
described in the literature only in nine patients, but never in the upper jaw.
Case presentation:This case report presents for the first time the clinical, radiographic, and macroscopic
findings of isolated macrodontia of the impacted maxillary permanent second premolar in a 12-year-old female
of Caucasian ethnicity. The X-ray and CBCT examination revealed the presence of an impacted macrodontic
second premolar and its distinct morphology characterized by large multitubercularmolariform crown and two
short conical roots. Orthodontic treatment was proposed; the second deciduous molar as well as macrodontic
second premolar weresurgically removed and space was orthodontically closed. The treatment was performed
without any unexpected complications.
Conclusion:Premolars affected by macrodontia often exhibit the failure of eruption and cause disturbance of
the developing occlusion. The tooth extraction is the only possible solution and it is recommended to extract
macrodontic premolar before the development is completed. This case report demonstrates that isolated
macrodontia of the second premolar is not limited to the mandible but may also occur in the maxilla.

I. Introduction
Macrodontia (alsocalledmegalodontism, megadontiaormacrodontism) is a rareshapeanomalydescribed
as gigantism in teeth. Itisoftenassociatedwithsystemicdisordersorsyndromes such as type I diabetes [1],
otodental syndrome, facialhemihyperplasia, KBG syndrome,Ekman-Westborg-Julin syndrome [2] and 47 XYY
syndrome. Isolatedmacrodontiaoccursonly very rarely [3, 4, 5, 6]. The prevalence of permanent teeth
withmacrodontiaisis0.03 to 1.9% [7, 8, 9], more frequently in men [8, 9]. Macrodontiacanbedividedinto
“truegeneralized“, “relativelygeneralized” and “isolated” form. In the truegeneralizedform, all teeth in the
dentition are largerthannormal teeth and thisconditionisusuallyassociatedwithpituitarygigantism [10].
Relativelygeneralizedform of macrodontiais thestate of normallysizedorslightlylarger teeth in
smalljaws [2]. The isolatedform of macrodontiaaffectsindividual teeth and is very rare. Itisdescribed as a
simpleenlargement of dental mass [11, 12] and itmayoccasionallybeassociatedwithmorphologicalabnormalities
[5, 13]. This type of macrodontiaisfound more frequently in incisors and canines [14] and has
onlyrarelybeendescribed in molars. Itisextremelyrare in premolars. The exactpathogeneticmechanism of
isolatedmacrodontiastillremainsunknown [15]. Untilnow, scientificliteraturedescribesonlyninecases of
isolatedmacrodontia of second premolar. This case study presents amacrodonticmaxillary permanent right
second premolar (tooth #15)in the firsttime in the scientificliterature.
II. Case presentation
The 12-year-old female of Caucasian ethnicitywas sent to the Division of PaediatricDentistry because
of the persistence of maxillary deciduous right second molar (tooth #55) and the retention of the right second
premolar (tooth #15). Personal as well as family medical history was not significant. The patient’s permanent
dentition was almost completely erupted, only tooth #55 persisted and tooth #15 was unerupted. All erupted
teeth were of normal size. A panoramic radiograph (Fig. 1) revealed abnormal size and shape of the tooth #15
characterized by gigantic multitubercularmolariform crown. The CBCT was made to determine the exact
position of the tooth, scans accurately displayed multitubercular enlarged crown of tooth 15, its significant
palatal dislocation and two short conical roots (Fig. 1). The orthodontic treatment with fixed appliance in both
jaws was proposed and extractions of teeth 55, 15, 24, 35 were planned. According to the recommendation of
orthodontist the patient was referred for surgical removal of teeth #55 and #15 and subsequent orthodontic
treatment, including space closure. CBCT helped to facilitate its surgical removal with minimal damage to
adjacent teeth and anatomical structures.The tooth #15 was extracted surgically in one piece in local anaesthesia,
without any unexpected complications (Fig. 1). The crown of the extracted tooth, measured with a digital

calliper, revealed mesiodistal dimension corresponding to 10.64 mm, vestibulooral dimension to be 13.11 mm
and 15.42 mm in occlusoapical direction.The wound was sutured with resorbable material and standard
postoperative care was undertaken. The articulation in Angle Class Iwas planned. Thelevellingof both dental
arches was performed by using nickel titanium wires with increasing diameter (.012–.018).A space closure,
aligning and a movement of teeth was performed by using more rigid .017x.025 steel wires. Planned anchorage
was mild on the right side and middle on the left side. Orthodontic treatment was carried out from January 2012
to July 2014.

III. Discussion
Isolatedmacrodontia of a second premolarisanextremelyrareanomaly. To ourknowledge, untilnow,
onlymacrodontia of the mandibular not maxillary second premolarwasdescribed, eitherunilaterally [5, 6, 10]
orbilaterally [3, 12, 15]. In all but onecases, thisanomalywasobserved in a patientbetween the age of 8 and 14
years, whichcorresponded to the time of eruption of second premolars. Premolarsaffected by macrodontiaoften
exhibit the failure of eruption and cause disturbance of the developingocclusion [15, 3]. Multituberculism (Fig.
1) has beenpreviouslydescribed in conjunctionwithmacrodontia and othershaped anomalies affecting the
wholedentition [15]. In accordancewiththis case report and previouspublications, multituberculism has
alsobeenpresent in isolatedmacrodontia of premolars [3, 5, 6, 15]. The enlargement of the pulpalcavityispresent
in this report as well as in most cases in literaturedescribing teeth withmacrodontia [15]. Due to the very similar
morphology of macrodontic second premolar to molar, Dugmoreevensuggestedusing the term
"macrodonticmolariformpremolars" [3]. The experiencefromprevioussimilarcasesshowsthattoothextractionis the
onlypossiblesolution and itisrecommended to extractmacrodonticpremolarbefore the development of dental
structuresiscompleted [3, 5, 6].

IV. Conclusion
This case report demonstrates that isolated macrodontia of the second premolar is not limited to the
mandible [3], but may also occur in the maxilla and the extraction of affected tooth seems to be the only
solution.
