Pulmonology is a medical specialty that deals with diseases involving the respiratory tract. Pulmonologists are physicians who specialize in diagnosing and treating respiratory diseases using procedures like bronchoscopy. The field includes general pulmonology and interventional pulmonology, which uses minimally invasive techniques to diagnose and treat lung conditions. Pulmonologists may perform surgical procedures and interpret pulmonary function tests and imaging to evaluate lung function and diagnose issues like lung cancer.
People with obstructive lung disease have difficulty exhaling all the air from their lungs due to damage or narrowing of the airways. This causes shortness of breath, especially during exertion as there is less time to exhale before inhaling again. Restrictive lung disease prevents lungs from fully expanding due to stiffness in the lungs or chest wall. Both types of lung disease cause shortness of breath but are treated differently. Obstructive lung disease is treated with bronchodilators and anti-inflammatory medications while restrictive lung disease has fewer treatment options but may involve immunosuppressants, oxygen therapy, or lung transplantation in severe cases.

Emphysema is a condition that damages the air sacs in the lungs called alveoli, causing them to lose their elasticity and become enlarged. This makes breathing difficult. The main causes are smoking cigarettes and exposure to air pollution or dust. Symptoms include shortness of breath, coughing, and fatigue. Diagnosis involves medical history, physical exam, chest X-rays, and lung function tests. Treatment focuses on quitting smoking, using inhalers, oxygen therapy, exercise, and lifestyle changes. Managing the condition can help prevent further lung damage and associated complications.
Acute respiratory viral infections (ARVI) are serious infections that prevent normal breathing and can spread throughout the respiratory system. According to the WHO, ARVI kill an estimated 2.6 million children annually worldwide. Symptoms include congestion, runny nose, cough, sore throat, and fatigue. Advanced symptoms include difficulty breathing, dizziness, and loss of consciousness. Children, the elderly, and those with weak immune systems are most at risk. Prevention methods focus on frequent hand washing and avoiding touching the face. Treatment involves antipyretic and antiviral medications to alleviate symptoms.
ASSESSMENT, EVALUATION AND TREATMENT OF RESPIRATORY CONDITIONSPeace Samuel
Physiotherapy plays an important role in managing respiratory conditions. The document discusses respiratory disease definitions, anatomy, assessment techniques including inspection, palpation, percussion and auscultation of the lungs, common respiratory diseases like obstructive and restrictive conditions, and how physiotherapy can aid diseases through pulmonary rehabilitation and breathing exercises.
Chronic obstructive pulmonary disease (COPD) is a common lung disease characterized by airflow limitation that is usually progressive. It is the third leading cause of death in the United States. The two main conditions that make up COPD are chronic bronchitis and emphysema. Cigarette smoking is the leading risk factor. Symptoms include dyspnea, chronic cough, and sputum production. Diagnosis is confirmed by spirometry showing airflow limitation. Management focuses on smoking cessation, bronchodilators, glucocorticoids, pulmonary rehabilitation, oxygen therapy, and managing exacerbations and comorbidities.
The respiratory system introduces oxygen to the body and removes carbon dioxide through a series of anatomical structures including the nasal cavity, pharynx, trachea, bronchi, bronchioles, and alveoli. In the alveoli, oxygen and carbon dioxide are passively exchanged between the blood and air by diffusion. Diseases like asthma and chronic obstructive pulmonary disease make breathing difficult by constricting the airways or destroying lung tissue. Lung cancer, commonly caused by smoking, is treated through surgery, radiation, and chemotherapy depending on the cancer type and stage.

Lung disease is any condition that affects the lungs and prevents them from working properly. There are three main types of lung disease: airway diseases which affect the tubes that carry air to the lungs, lung tissue diseases which affect lung tissue structure, and lung circulation diseases which affect blood vessels in the lungs. Common lung diseases include asthma, lung cancer, and chronic obstructive pulmonary disease (COPD). Asthma causes inflammation and narrowing of the airways and is diagnosed based on a person's medical history, physical exam findings, and results of breathing tests.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

Oxygen in the air is essential for our life. Lung, like heart, is vital in delivering oxygenated
blood to entire body. Lungs illness is often overlooked and diagnosed late unlike heart
ailments. Awareness about lung illness is lacking among doctors as well as common
man. Lung, like skin, is directly exposed to harmful environmental air pollutants and
breathing; cough problem is increasing day by day especially in the fast growing urban area of
PCMC. Lung/Respiratory illness can range from simple allergic cough, Asthma, Smoking/dust
related COPD to Sleep Disorder breathing(like Snoring), Tuberculosis, Lung Cancer and deaths
in ICU from Respiratory failure(ARDS, Hospital acquired Pneumonia).
In India, unlike western world, a dedicated comprehensive Pulmonology combined with
Critical Intensivist super specialty training is lacking in Medical Colleges and university.
5. CLINIC CONSULTATION
Pulmonology is the subspecialty of internal medicine concerned with diseases of the lungs
and bronchial tubes, which often involves evaluation of the upper respiratory tract (nose,
pharynx and throat) as well as the heart. Pulmonologists must first complete seven or more
years of medical school and postgraduate training and become board certified in Internal
Medicine. Then, for an additional two to three years, they study conditions specific to the
respiratory system.
What pulmonologists do :
Pulmonologists are specially trained in diseases and conditions of the chest, such as
pneumonia, asthma, tuberculosis, emphysema, or complicated chest infections.
