This prospective randomized trial compared peritoneal irrigation versus suction alone during laparoscopic appendectomy for perforated appendicitis in children. 220 patients under 18 with perforated appendicitis were randomly assigned to receive either peritoneal irrigation with 500mL saline or suction only. The primary outcome measured was postoperative abscess rate. There was no significant difference in abscess rates between groups, with rates of 19.1% for suction only and 18.3% for irrigation. Other outcomes like hospital stay, charges, and operative time also did not significantly differ. The study concludes that irrigation provides no advantage over suction alone for perforated appendicitis during laparoscopic appendectomy in children.
