Cloud Interoperability in a Hospital Ecosystem

The hospital has implemented diverse healthcare systems, including EMR, EHR, RPM, Telehealth, and
others. The challenge is to seamlessly exchange cri�cal data among these systems, specifically related to
healthcare providers, laboratory results, and ongoing clinical trials.
Interoperability Components & Data Formats:
AIMDek Interoperability Engine:
We deployed a sophis�cated interoperability engine that adheres to the latest USA-based healthcare regulatory
standards (such as HL7, FHIR, and DICOM).
This engine acts as a centralized hub for data exchange, ensuring seamless communica�on between various
systems.
Remote Pa�ent Monitoring (RPM):
The hospital employs RPM solu�ons to monitor pa�ents remotely, collec�ng
data on vital signs, ac�vity levels, and other relevant health metrics.
The interoperability engine integrates with RPM systems, enabling the
seamless transfer of pa�ent-generated health data to both EMR and EHR
systems. This ensures that healthcare providers can monitor and respond
to changes in pa�ent health in real �me.
Data Format:
Con�nua Health Alliance standards (such as IEEE 11073 and ISO/IEEE 11073) were
used for RPM data, defining communica�on protocols for health devices.
Interoperability File Format:
XML or HL7 CDA (Clinical Document Architecture) as interoperability formats for
remote monitoring data.
Lab Data Exchange:
Secure exchange of lab data between the hospital's laboratory informa�on
system which was legacy card punch system combined with na�ve mobile
applica�on and they were both connected to EMR and EHR systems.
This ensured that healthcare providers have real-�me access to the latest
lab results, enabling them to make informed decisions about pa�ent care.
Data Format:
DICOM was used for Lab data communica�on protocols with different healthcare
systems.
Interoperability File Format:
DICOM based Implicit VR Li�le Endian and Explicit VR Li�le Endian, Metadata Tags,
Pixel Data and Mul�-frame DICOM.

Telehealth Solu�ons:
Telehealth solu�ons were implemented to facilitate virtual consulta�ons
and remote healthcare services using Vidyo.
Our interoperability engine enables the exchange of pa�ent data,
including consulta�on notes and treatment plans, between Telehealth
pla�orms and EMR/EHR systems, ensuring that virtual care is seamlessly
integrated into the pa�ent's overall healthcare record.
Data Format:
Combina�on of Con�nua Health Alliance Standards for device communica�on and
DICOM (Digital Imaging and Communica�ons in Medicine) for medical imaging data.
Clinical Trial Collabora�on:
The hospital ac�vely par�cipates in clinical trials to advance medical
research. The interoperability engine extends its capabili�es to integrate
with external clinical trial databases.
Pa�ent eligibility criteria, enrolment status, and relevant trial data are
exchanged between the hospital's systems and external databases,
fostering collabora�on, and suppor�ng research ini�a�ves.
Data Format:
CDISC were used to define communica�on protocols for clinical trial pla�orms.
EMR and EHR Integra�on:
The interoperability engine enabled bidirec�onal communica�on between
the EMR and EHR of Epic Systems, ensuring that pa�ent records are
consistent across both pla�orms.
Health updates, medica�on changes, and treatment plans ini�ated in the
EMR are instantly reflected in the EHR, providing a unified view of pa�ent
informa�on.
EMR (Electronic Medical Records) Data Format:
Con�nua Health Alliance standards (such as IEEE 11073 and ISO/IEEE 11073) were
used for RPM data, defining communica�on protocols for health devices.
EMR (Electronic Medical Records) Interoperability File Format:
HL7 messages encoded in XML or JSON format, following the FHIR (Fast Healthcare
Interoperability Resources) standard for modern interoperability.
EHR (Electronic Health Records):
HL7 FHIR resources encoded in XML or JSON format. FHIR resources were used for
provider, pa�ent and lab informa�on exchange including Pa�ent, Encounter,
Observa�on, and others.
