This document discusses initiatives taken by Indraprastha Apollo Hospitals in India to create a culture of safety. A survey identified areas of concern among staff such as long work hours and fears about mistakes being held against them. The hospital implemented initiatives like encouraging incident reporting, multidisciplinary huddles, leadership rounds, yoga sessions, and suggestion boxes. A follow-up survey showed improvements in staff behaviors and perceptions of safety. Patient satisfaction scores increased while the average length of stay and medication errors decreased, demonstrating the positive impact of prioritizing a culture of safety.
