House classification(1937)
 Based on their Mental attitude , patients can grouped under two classifications.
DR.MM House in 1950 classified patient’s psychology into four types,
A .Classification 1
Class1 - Philosophical
Class2 - Exacting
Class3 - Hysterical
Class4 - Indifferent
B . Classification 2
1.Cooperative2
1. Apprehensive
1. Anxious
2. Frightened
3. Obsessive or exacting
4. Chronic complainer
5. Self- conscious
2.Un cooperative
A. Classification 1
1.Class 1- Philosophical
 WHO presented themselves prior to the extraction of their teeth.
 Have no experience in wearing denture, and do not anticipate any special
difficulties in that regard.
 This type of patients can be described as easy going, congenial.
 Mentally well adjusted, co operative and confident of the dentist.
2.Class 2-Exacting
 This type of patients are precise, above average in
intelligence , concerned.
 In their dress and appearances , usually dissatisfied by their
previous treatment.
 Do not have confidence in their dentist.
 Its difficult to satisfy them but once they satisfied they
become supporter to dentist’s.
3.Class3-Hysterical
 This type of patient does not want to have any treatment
done.
 They comes out of compulsion from their relative and
friends.
 They have a highly negative attitude to the dentist and the
treatment.
 They have un realistic expectations and want the denture
should be better than their natural teeth .
 They are the most difficult patient to manage.
4. Class4-Indifferent
 This type of patient are unconcerned about their
appearance and feel very little or no necessity for teeth
for mastication.
 Hence, They are uncooperative and will hardly try to
become accustomed to denture.
 They will not maintain the dentures properly and do not
appreciate the effort and skill of the dentist.
House's classification (Psychological)
Prognosis
Class 1: Philosophical
Accepts dentist's judgement and instructions,
Excellent-Best prognosis.
Class 2: Exacting
Methodical and demanding, asks a lot of questions,
Best- good prognosis.
Class 3: Hysterical
Emotionally unfit, never happy, Good-poor prognosis.
Class 4: Indifferent
Doesn't care about dental treatment and gives up easily.
Good prognosis.
B . Classification 2
1.Cooperative2
 These patients represent the optimum group.
 They may/may not recognize the need for dentures but they are open-
minded and are amenable to suggestion.
 Procedure can be explained with very little effort and they become fully
cooperative.
1.Apprehensive
 Even though these patients realise the need for dentures they have
some irrational problem, which cannot be overcome by ordinary
explanation.
 The approach to all of these patients is to talk with them and to make
them speak out their thoughts about denture.
Apprehensive patients are of different types namely,
1. Anxious
 These patients are anxious and upset about the un certainities
of wearing denture.
 They often put themselves into a neurotic state.
 In extreme and rare cases they may be psychotic.
2.Frightened
 They may fair for the cancer,
 They fear that they will not be able to wear the teeth.
 They may fear that the teeth will not look well.
 Extreme cases should be referred to a psychiatrist.
3.Obsessive or exacting
These patients are of exacting nature and are
accustomed to giving directions to others.
This type of patient is handle firmly.
At outside only suppose to tell that would not
be allowed to direct the denture
construction.
4.Chronic complainers
 They are a group of people who are
habituated fault finding and dissatisfied.
 It is best to have an understanding with
such patients before work commences.
5.Self- conscious
 It is wise to give overt reassurance to the
self- conscious patient and
 Permit participation in the reconstruction
as far as feasible in order to
 Established some responsibility in the
result.
2. Uncooperative
 This type of patients present themselves usually upon
being urgent by relative and friends.
 They does not feel for dentures, although the need exists.
 Their general attitude is negative.
 They constitute an extremely difficult group of potential
dentures wears and tax the dentist’s patience to the limit,
 Most cases ,An attempt to make dentures for these
individuals is a waste of time.
 In this case dentist must collect information about the
patient habit , diets , past dental history and the physical
characteristics.
 The expectation of the patient should be taken into
consideration to achives patient satisfaction.
