The hospital is developing a nurse-run outpatient heart failure clinic to address high readmission rates for heart failure patients. The clinic aims to improve patient education, monitoring, and care coordination. The student has been asked to develop a plan for one component of the clinic - an orientation course plan, discharge education plan, or care coordination plan. The plan must be evidence-based, address patient needs, and indicate how effectiveness will be measured.

Overview
Write a 3-4 page evidence-based health care delivery plan for one component of a heart failure clinic.
Nursing within an organization is a critical component of health care delivery and is an essential ingredient in patient outcomes (Kelly & Tazbir, 2014). The concern for quality care that flows from evidence-based practice generates a desired outcome. Without these factors, a nurse cannot be an effective leader. It is important to lead not only from this position but from knowledge and expertise.
By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:
•Competency 2: Explain the accountability of the nurse leader for decisions that affect health care delivery and patient outcomes. ◦Describe accountability tools and procedures used to measure effectiveness.
•Competency 3: Apply management strategies and best practices for health care finance, human resources, and materials allocation decisions to improve health care delivery and patient outcomes. ◦Develop an evidence-based plan for health care delivery.
•Competency 4: Apply professional standards of moral, ethical, and legal conduct in professional practice. ◦Apply professional and legal standards in support of a care plan.
•Competency 5: Communicate in manner that is consistent with the expectations of a nursing professional. ◦Write content clearly and logically, with correct use of grammar, punctuation, and mechanics.
◦Correctly format citations and references using current APA style.

Context
In an effort to improve the patients' health literacy concerning heart failure, it is important that the clinic staff and the hospital staff present a consistent, evidence-based message on self-care to these patients and their families in order to decrease acute exacerbation and re-admissions. Review current evidence for clinical practice guides or protocols when developing your patient teaching plans and materials. Consider the following:
•What does the patient know about the disease process as a baseline?
•What does the patient need to do understand as far as the best self-care processes?
•Can the patient identify proper medication compliance?
•Is there a financial issue that affects compliance?
•Who buys and prepares the food in the home?
•Can the patient verbalize when to seek medical assistance?
Questions to Consider
To deepen your understanding, you are encouraged to consider the questions below and discuss them with a fellow learner, a work associate, an interested friend, or a member of your professional community.
•What factors contribute to inadequate quality of care?
•How effective are organizational mandates for quality?
•How do financial concerns impact health and safety goals?

Deliverable:
Develop an evidence-based plan for health care delivery.
Scenario:
The hospital where you work has an issue with increased readmissions within 30 days of discharge. After examining the core measures, it was found that heart failure was the most common core measure disease process experiencing the highest rate of readmissions. The leadership team has given your team the charge of developing a nurse-run outpatient heart failure clinic. The purpose of this clinic is to ensure that discharge education is presented to the patient in an orderly, consistent manner and complies with evidence-based practice protocols. Since these patients may be discharged from a variety of areas in the facility, having the heart failure clinic staff take ownership of the process will improve both consistency and compliance. There are cardiologists that interact with the staff and patients, but the day-to-day operations of the clinic are designed and supported by the nurses as they interact with appropriate members of the other health care team disciplines promoting the best care for the heart failure patients.

The hospital leadership established these objectives for the clinic services:
Evaluate and maximize proper medication therapy.
Conduct regular diet, exercise, and stress management classes for the patients.
Monitor physiological indicators for the patients (lab work, weights, vital signs, ECGs).
Provide a case management system for patients in the program post-discharge.
The overall goals for the heart failure clinic are to:
Enroll greater than 90 percent of the patients with a primary or secondary diagnosis of HF prior to discharge.
Facilitate discharge planning to achieve 100 percent compliance with patient education prior to discharge (discharge planning).
Decrease readmission rates in this population by 5 percent over the next year.
The leadership team has asked you to provide them with an evidence-based plan for one of the components of the clinic. You may use any combination of documents (for example, a spreadsheet or a table) in addition to explanatory information to convey information clearly and succinctly.

Letter to the Editor: Population Health Policy Advocacy
Write a 3–5 page letter to the editor of an academic or professional journal. Your submission should be succinct yet substantive.
Note: Each assessment in this course builds on the work you completed in the previous assessment. Therefore, you must complete the assessments in this course in the order in which they are presented.
Advocating for new policies is an important aspect of the master’s-prepared nurse. For new policies to be compelling they need to be supported by evidence. Supporting data can be used to illustrate why new policies and interventions are needed to help address a specific health issue. Compelling data can help sway the stakeholders and gain support for your policy.
Another aspect of advocacy is disseminating new policies and interventions outside of the immediate care environment. This can be done by reaching out to professional organizations as well as academic and professional journals. A letter to the editor is one strategy for disseminating information to a wider audience, and to potentially enlist support throughout the wider professional community.
By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:
Competency 1: Design evidence-based advanced nursing care for achieving high-quality population outcomes.
Evaluate the current state of the quality of care and outcomes for a specific issue in a target population.
Justify why a developed policy will be vital in improving the quality of care and outcomes for a specific issue in a target population.
Competency 2: Evaluate the efficiency and effectiveness of interprofessional interventions in achieving desired population health outcomes.
Analyze the ways in which interprofessional aspects of a developed policy will support efficient and effective achievement of desired outcomes for the target population.
Competency 3: Analyze population health outcomes in terms of their implications for health policy advocacy.
Analyze how the current state of the quality of care and outcomes for a specific issue in a target population necessitates health policy development and advocacy.
Advocate for policy development in other care settings with regard to a specific issue in a target population.
Competency 4: Communicate effectively with diverse audiences, in an appropriate form and style, consistent with organizational, professional, and scholarly standards.
Communicate in a professional and persuasive manner, writing content clearly and logically with correct use of grammar, punctuation, and spelling.
Integrate relevant sources to support assertions, correctly formatting citations and references using APA style.

SWOT Assignment – CSUSB
Purpose:
In this assignment you will apply what you have learned from Chapters 2 and 3 by conducting a SWOT analysis of CSUSB.
Instructions
1. Use the layout template provided in the assignment details. Save it as a Word document and submit it to Blackboard.
2. Begin by populating the Strengths section of your chart. Identify what you perceive to be the strengths of CSUSB as compared to other universities. For example, answering the following questions should provide you with a start, but this list is not meant to be exhaustive:
a. What advantages does CSUSB have that others don’t have?
b. What does CSUSB do better than anyone else?
c. What resources can CSUSB access?
d. What do other people see as the strengths of CSUSB?
e. What accomplishments should CSUSB be most proud of?
f. What are the values of CSUSB and are they a strength or a weakness?
g. What is the reputation/brand of CSUSB and is it a strength or a weakness?
3. Continue to fill in the other three sections in your chart by:
a. Identifying the weaknesses of CSUSB compared to other universities.
b. Identifying opportunities that exist or will exist in the future (think environmental scan from Chapter 3) that CSUSB might be well positioned to take advantage of.
c. Identifying threats that exist or will exist in the future (again, think environmental scan from Chapter 3) that CSUSB will need to take steps to address in order to avoid.
4. Follow the layout example below. Use well written, bulleted sentences and make sure that you provide clear support for each of your bullet points. For example, you cannot simply state that the school has a good/bad reputation without providing a sentence or two to support your position.

Recommending an Evidence-Based Practice Change
My Facilityl is focused on providing quality healthcare to all patients regardless of their differences.
The facility is has a culture of embracing change as long as it helps in improving the patients’ health outcomes.
However, since our hospital is a community-based health service facility, there are some of things that need to be changed.
The healthcare facility offers cancer services including screening and management services. Screening is offered to the community occasionally when the facility organizes cancer awareness where they get more cancer professionals from other hospitals to help provide screening services to the people.
Recommending an Evidence-Based Practice Change
The problem facing the healthcare facility currently is the lack of cancer screening awareness among the community members and enough oncologists.
Cancer screening services require advanced technology and machines to ensure detection and diagnosis of cancer.
Cancer is one of the top diseases causing high mortality rates around the world presently.

Part 4B: Critical Appraisal of Research
Given my examination, the best practice that rises out of the exploration I checked on is Evidence-Based Practice (EBP), whereby in a clinical setting, it is considered as a fundamental component for guaranteeing that patients are given quality care just as treatment services. EBP is viewed as reasonable just as meticulous use of clinical practices that depend on current evidence. Also, medical care experts, with the help of EBP, can settle on successful decisions in connection to medicinal services operations. EBP depends on various pieces of evidence that incorporate qualitative just as quantitative research, controlled preliminaries, case reports, expert opinion, and scientific standards.
In this specific case, the clinical practices dependent on EBP help with giving better care just as treatment benefits as per patient values alongside clinical aptitude (Forrest, 2008). EBP depends on evidence gathered from qualitative research. Consequently, the quantitative analysis assumes a significant role in collecting data about current practices to be effected for the improvement of clinical skills and in gathering the patient's values. The research examines that are ineffectively structured, and inadequate reporting is contended to influence quantitative analysis crosswise over various spheres that incorporate medicinal services, future research, decision making, and health policy. In such manner, distinguishing reporting rules including diagnosis test studies (STARD), observational studies (STROBE), meta-analyses of observational studies (MOOSE), consolidated criteria for reporting qualitative research (COREQ) and randomized controlled trials (CONSORT) were used in these peer-reviewed articles.
Recognizing that clinicians have time constraints but then need to give the ideal care to their patients, the evidence-based methodology offers clinicians an advantageous technique for discovering current research to help in making clinical decisions, answer patient questions, and investigate alternative therapies, strategies, or materials. With a comprehension of how to viably use EBDM, professionals can rapidly and helpfully remain current with scientific discoveries on points that are essential to them and their patients.

As a member of the nurse team, you have been asked to develop onecomponent of the clinic.
The hospital leadership established these objective.

Integrate relevant sources to support assertions, correctly formatting citations and references using APA style.

WALDEN UNIVERSITY

Write a 3 page evidence-based health care delivery plan for one component of a heart failure clinic.
· Competency 2: Explain the accountability of the nurse leader for decisions that affect health care delivery and patient outcomes.
. Describe accountability tools and procedures used to measure effectiveness.
· Competency 3: Apply management strategies and best practices for health care finance, human resources, and materials allocation decisions to improve health care delivery and patient outcomes.
. Develop an evidence-based plan for health care delivery.
· Competency 4: Apply professional standards of moral, ethical, and legal conduct in professional practice.
. Apply professional and legal standards in support of a care plan.
· Competency 5: Communicate in manner that is consistent with the expectations of a nursing professional.
. Write content clearly and logically, with correct use of grammar, punctuation, mechanics, and current APA style.

Embracing GenAI - A Strategic ImperativePeter Windle
Artificial Intelligence (AI) technologies such as Generative AI, Image Generators and Large Language Models have had a dramatic impact on teaching, learning and assessment over the past 18 months. The most immediate threat AI posed was to Academic Integrity with Higher Education Institutes (HEIs) focusing their efforts on combating the use of GenAI in assessment. Guidelines were developed for staff and students, policies put in place too. Innovative educators have forged paths in the use of Generative AI for teaching, learning and assessments leading to pockets of transformation springing up across HEIs, often with little or no top-down guidance, support or direction.
This Gasta posits a strategic approach to integrating AI into HEIs to prepare staff, students and the curriculum for an evolving world and workplace. We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. The rapid evolution of AI technologies necessitates a proactive and strategic approach if we are to remain relevant.

June 3, 2024 Anti-Semitism Letter Sent to MIT President Kornbluth and MIT Cor...Levi Shapiro
Letter from the Congress of the United States regarding Anti-Semitism sent June 3rd to MIT President Sally Kornbluth, MIT Corp Chair, Mark Gorenberg
Dear Dr. Kornbluth and Mr. Gorenberg,
The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. The Committee on Oversight and Accountability is the principal oversight committee of the US House of Representatives and has broad authority to investigate “any matter” at “any time” under House Rule X.
• The Committee on Ways and Means has been investigating several universities since November 15, 2023, when the Committee held a hearing entitled From Ivory Towers to Dark Corners: Investigating the Nexus Between Antisemitism, Tax-Exempt Universities, and Terror Financing. The Committee followed the hearing with letters to those institutions on January 10, 202
The Roman Empire A Historical Colossus.pdfkaushalkr1407
The Roman Empire, a vast and enduring power, stands as one of history's most remarkable civilizations, leaving an indelible imprint on the world. It emerged from the Roman Republic, transitioning into an imperial powerhouse under the leadership of Augustus Caesar in 27 BCE. This transformation marked the beginning of an era defined by unprecedented territorial expansion, architectural marvels, and profound cultural influence.
The empire's roots lie in the city of Rome, founded, according to legend, by Romulus in 753 BCE. Over centuries, Rome evolved from a small settlement to a formidable republic, characterized by a complex political system with elected officials and checks on power. However, internal strife, class conflicts, and military ambitions paved the way for the end of the Republic. Julius Caesar’s dictatorship and subsequent assassination in 44 BCE created a power vacuum, leading to a civil war. Octavian, later Augustus, emerged victorious, heralding the Roman Empire’s birth.
Under Augustus, the empire experienced the Pax Romana, a 200-year period of relative peace and stability. Augustus reformed the military, established efficient administrative systems, and initiated grand construction projects. The empire's borders expanded, encompassing territories from Britain to Egypt and from Spain to the Euphrates. Roman legions, renowned for their discipline and engineering prowess, secured and maintained these vast territories, building roads, fortifications, and cities that facilitated control and integration.
The Roman Empire’s society was hierarchical, with a rigid class system. At the top were the patricians, wealthy elites who held significant political power. Below them were the plebeians, free citizens with limited political influence, and the vast numbers of slaves who formed the backbone of the economy. The family unit was central, governed by the paterfamilias, the male head who held absolute authority.
Culturally, the Romans were eclectic, absorbing and adapting elements from the civilizations they encountered, particularly the Greeks. Roman art, literature, and philosophy reflected this synthesis, creating a rich cultural tapestry. Latin, the Roman language, became the lingua franca of the Western world, influencing numerous modern languages.
Roman architecture and engineering achievements were monumental. They perfected the arch, vault, and dome, constructing enduring structures like the Colosseum, Pantheon, and aqueducts. These engineering marvels not only showcased Roman ingenuity but also served practical purposes, from public entertainment to water supply.
