On examination, he had swelling and tenderness over his left lateral malleolus. Radiographs showed fractures of the left calcaneus and lateral malleolus. He was admitted for open reduction internal fixation surgery of the fractures. Calcaneus fractures are commonly caused by high-energy injuries like falls from heights. They often require operative treatment for displaced or intra-articular fractures to restore anatomy and function.

Musculoskeletal Masqeuraders - Rolling the 'Clinical Dice'
I recently delivered this presentation on 'MSK Masqueraders' at the National Exhibition Center (NEC) in Birmingham for the 2015 Therapy Expo conference.
Basically this was a brief insight and overview of MSK Masqueraders and the impact on clinical practice. The context of each slide was expanded during the conference session and hopefully the presentation below gives you a flavour of the topics I covered. Be mindful that this presentation is a snippet of what I would usually cover so, is not a complete overview of the topic of Masqueraders, which is a challenging area of clinical practice.
Having knowledge of conditions that can masquerade as MSK pathology is a key aspect of the physiotherapist’s clinical development. More and more roles and opportunities are arising where we are responsible for first line assessment and care, which means we must have an ability to screen effectively, systematically and understand when the patient is presenting with symptoms that don't quite fit with an MSK presentation.
Index of suspicion, pattern recognition and understanding when and how to streamline you assessment to ascertain clarity on the next steps for a patient that you are concerned about can be challenging but is vital.

Achilles tendinopathy treatment with triple therapy
1) Achilles tendinopathy is a common cause of heel pain, especially in amateur athletes between ages 28-55.
2) A pilot study used triple therapy involving two laser sources at different wavelengths and powers to treat 5 patients with chronic Achilles tendinopathy.
3) After 9 treatment sessions over 3 weeks, patients experienced significant reductions in pain based on VAS and algometer scores, and improvements on ultrasound and thermography, with no adverse reactions. The study concludes triple therapy may be an appropriate treatment for Achilles tendinopathy.

Meniscus injury / tear

Symptoms include knee pain, swelling, stiffness, tenderness, pain with squatting, popping or clicking in the knee, and limited motion. Meniscus tears are classified as longitudinal, horizontal, radial, or flap tears. Exams like McMurray's test and Apley's test are used to diagnose tears. Treatment involves medications, surgery if the meniscus cannot be repaired, physiotherapy including exercises and bracing, and rehabilitation protocols after arthroscopic surgery or meniscal repair surgery. Isokinetic training after arthroscopy can help improve knee function and muscle strength recovery.
Hand examination

Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
