Diabetes is a rapidly and serious health problem in Pakistan. This chronic condition is associated with serious long-term complications, including higher risk of heart disease and stroke. Aggressive treatment of hypertension and hyperlipideamia can result in a substantial reduction in cardiovascular events in patients with diabetes 1. Consequently pharmacist-led diabetes cardiovascular risk (DCVR) clinics have been established in both primary and secondary care sites in NHS Lothian during the past five years. An audit of the pharmaceutical care delivery at the clinics was conducted in order to evaluate practice and to standardize the pharmacists’ documentation of outcomes. Pharmaceutical care issues (PCI) and patient details were collected both prospectively and retrospectively from three DCVR clinics. The PCI`s were categorized according to a triangularised system consisting of multiple categories. These were ‘checks’, ‘changes’ (‘change in drug therapy process’ and ‘change in drug therapy’), ‘drug therapy problems’ and ‘quality assurance descriptors’ (‘timer perspective’ and ‘degree of change’). A verified medication assessment tool (MAT) for patients with chronic cardiovascular disease was applied to the patients from one of the clinics. The tool was used to quantify PCI`s and pharmacist actions that were centered on implementing or enforcing clinical guideline standards. A database was developed to be used as an assessment tool and to standardize the documentation of achievement of outcomes. Feedback on the audit of the pharmaceutical care delivery and the database was received from the DCVR clinic pharmacist at a focus group meeting.

1. Global warming is one of the most pressing issues facing our planet today. It has been a major
concern for many years, but in the past 10 years, its effects have become more evident. The
consequences of global warming are far-reaching and can be seen all over the world in various
forms. In this essay, we will explore some of these effects that have been observed over the last
decade.
The first effect to consider is rising temperatures across much of the globe due to increased
levels of carbon dioxide trapping heat from sunlight within our atmosphere. This has caused an
increase in the average temperature of around 1 degree Celsius since preindustrial times, which
may not seem like much but is enough to cause noticeable changes such as melting glaciers and
sea ice at higher latitudes or longer droughts further south, causing loss of vegetation cover and
leading to the desertification process. Additionally, extreme weather events like hurricanes and
floods are becoming increasingly common due to warmer air storing more energy, making
storms stronger than ever before.
Another effect of global warming is ocean acidification, which occurs when increased CO2
concentrations in the oceans cause a decrease in pH levels and the bleaching of coral reefs,
making them vulnerable to disease outbreaks and eventually killing them. Furthermore, ocean
currents affected by thermal expansion also disrupt marine life’s natural habitat, thus decreasing
biodiversity even further. Finally, human health concerns arise when polluted air becomes
trapped near ground level as a result of smog produced by the combustion of fossil fuels,
increasing respiratory diseases among the population, particularly children, who suffer the most
from asthma attacks during the summer months when the ozone layer weakens further, allowing
UV rays to penetrate through the atmosphere directly on the earth's surface, damaging skin cells
along the way.
To summarize, global warming has had a dramatic impact on both the environment and human
societies over the last decade, ranging from rising temperatures to ocean acidification, which has
threatened entire ecosystems, including us humans who live here! We must act now before it gets
any worse by reducing emissions and switching to renewable energy production if we want
future generations to live healthy lives without worrying about climate change every single day!
