- A 72-year-old male presented with breathlessness and was found to have a giant right coronary artery aneurysm measuring 22x27 cm. - He underwent high-risk percutaneous coronary intervention to address the aneurysm with placement of multiple covered stents. - The procedure was technically challenging due to the large size and irregular shape of the aneurysm, but ultimately was successful in sealing the aneurysm and excluding it from circulation.

Septal Ablations Presentation
Septal ablation involves injecting alcohol into the septal artery to induce a controlled infarction and is used to treat HOCM. The procedure uses TEE monitoring and a temporary pacemaker. Atrial ablation uses a catheter to deliver energy like cryothermy, radiofrequency, or lasers to scar tissue causing arrhythmias like Afib or flutter. Both procedures carry risks like infection or damage but have high success rates around 90% for treating conditions like HOCM, Afib, or SVT.

Vascular access for haemodialysis requires either an autogenous arteriovenous fistula, a PTFE graft, or a tunnelled line. A good fistula has adequate size, blood flow, and low complication rates. Formation of an arteriovenous fistula involves connecting an artery such as the radial or brachial to a vein like the cephalic or basilic. Complications include steal syndrome, aneurysms, pseudoaneurysms, venous hypertension, and infection. Steal syndrome is treated with distal reconstruction and interval ligation. Buttonhole and rope-ladder techniques are used for cannulating fistulas and have benefits over traditional needle cannulation.

Chronic total occlusion pci
Coronary CTO is characterized by heavy atherosclerotic plaque burden within the artery, resulting in complete (or nearly complete) occlusion of the vessel. Although the duration of the occlusion is difficult to determine on clinical grounds, a total occlusion must be present for at least 3 months to be considered a true CTO. Patients with CTO typically have collateralization of the distal vessel on coronary angiography, but these collaterals may not provide sufficient blood flow to the myocardial bed, resulting in ischemia and anginal symptoms. CTO is clinically distinct from acute coronary occlusion, which occurs in the setting of ST-segment–elevation myocardial infarction, or subacute coronary occlusion, discovered with delayed presentation after ST-segment–elevation myocardial infarction. Clinical features and treatment considerations of these entities differ considerably from CTO.
Among patients who have a clinical indication for coronary angiography, the incidence of CTO has been reported to be as high as 15% to 30%. Patients with CTO are referred for angiography because of anginal symptoms or significant ischemia on noninvasive ischemia testing. Patients who are symptomatic will have stable exertional angina resulting from a limitation of collateral vessel flow to meet myocardial oxygen demand with stress. Of patients referred for PCI in clinical trials of CTO PCI, only 10% to 15% of patients are asymptomatic. It is likewise uncommon for patients with CTO to present with an acute coronary syndrome caused by the CTO itself.

Lost Balloon - Manish Ramlall
A 45-year-old man was admitted with NSTEMI and underwent stenting of a lesion in his LAD artery. During post-dilation of the stent, the balloon failed to deflate after inflation and its shaft snapped, leaving the deflated balloon trapped in the patient's artery. Another catheter was inserted through the radial artery to inject contrast and a second wire and balloon were used along with a trapping technique to pull the entire apparatus including the trapped balloon out of the patient's artery. The procedure was successful and the patient was treated for the balloon complication without further issues.

- 2. • 72 years male with recent varicose vein operation. • Admitted with Breathlessness and pneumonia. • Worsening of dyspnea and becoming unwell. • Echo pericardial effusion (consistency and appearance looks like blood) with compression of RV. • Urgent CT scan chest acute pericardial hemorrhage with RV compression. • Surgical evacuation of hematoma RCA aneurysm found but no evident of active bleeding. • Few days later in CITU massive bleeding in drains shock emergency sternotomy in CITUruptured RCA aneurysmsurgical repair of aneurysm but unable to graft the RCA due to adhesions. • CT CA Giant RCA aneurysm (22x27 cm). • MDT for high risk PCI as aneurysm was still liable to re- bleed again.

- 6. 2 orthogonal angiogram views showing that the wire was not in the proper alignment with the distal opening of the aneurysm. - Spend a lot of time trying to find the exit of the aneurysm into the distal vessel
- 7. Complete 360 degree loop of the sion blue wire inside the aneurysm -Successful exit into the distal RCA
- 8. Complete loop of the wire Finecross+ exchange for grand Slam wire Guideliner + 3.0x15 mm balloon To straighten the wire loop
- 9. Balloon assisted guideliner tracking Into distal RCA Xience 3.5x48 mm as a scaffold which covered Both the entrance and the exit sites of the aneurysm Posdilated proximal and distal part with 4.0 NC
- 10. -Positioning of 2 Aneugraft covered stent 4x 27 mm - Post dilated with 4.0 NC at high pressure
- 11. -Results after the covered stents -proximal perforation in the native RCA that To seal by repetitive balloon tamponade Positioning of a third covered stent 4.0x 18 mm that post-dilated with 4.5 NC at high pressure
- 12. Final results angiographically and by IVUS
