Objective
:
For this assignment, you will create the assessment to implement the new HER in a Health care setting. The assessment phase is foundational to all other EHR implementation steps, and involves determining if the practice is ready to make the change from paper records to electronic (EHRs), or to upgrade their current system to a new certified version. You will be encourage to choose a Community Health Center or a Doctor’s Office. The Assessment is designed because our world has been radically transformed by digital technology – smart phones, tablets, and web-enabled devices have transformed our daily lives and the way we communicate. Medicine is an information-rich enterprise. A greater and more seamless flow of information within a digital health care infrastructure, created by electronic health records (EHRs), encompasses and leverages digital progress and can transform the way care is delivered and compensated. With EHRs, information is available whenever and wherever it is needed.
The Health Information Technology for Economic and Clinical Health (HITECH) Act, a component of the American Recovery and Reinvestment Act of 2009, represents the Nation’s first substantial commitment of Federal resources to support the widespread adoption of EHRs. As of August 2012, 54 percent of the Medicare- and Medicaid-eligible professionals had registered for the
meaningful use incentive program

ASSIGNMENT GUIDELINES (2 points /10%):
The
assessment
should look at the current state of the practice:
Are administrative processes organized, efficient, and well documented?
Are clinical workflows efficient, clearly mapped out, and understood by all staff?
Are data collection and reporting processes well established and documented?
Are staff members computer literate and comfortable with information technology?
Does the practice have access to
high-speed internet connectivity
?
Does the practice have access to the financial capital required to purchase new or additional hardware?
Are there clinical priorities or needs that should be addressed?
Does the practice have specialty specific requirements?
Through the Regional Extension Centers (RECs), we’ve learned that these questions and assessment tools provide a good understanding of the current state of the practice and can help identify key goals for improvement. Often, these goals relate to patient quality, patient satisfaction, practice productivity and efficiency, improved quality of work environment, and most important to the overall goal – improved health care.

INTRODUCTION
Health information technology (HIT) has been revolutionized by the development of the electronic health record (EHR) and its expansion. The generalization of its use includes the transformation of paper patient charts and protected health information (PHI) into electronic forms and from there the options to revolutionize HIT began. The development of the EHR has brought a multitude of advantages and challenges with its implementation in the HIT world. Although the functions of EHR have allowed for both public and private sectors to utilize its possibilities, the private sector will be focused on in this situation. The ability for an electronic system to safely document, record, save, integrate and share with other instantly has allowed HIT to expand rapidly to all corners of the world.
EHR functionalities
In health information technologies, electronic health records have been identified by the Institute of Medicine as having eight core components which include the following [1]: recording patient health information and data, result management, physician order management, decision support, electronic communication and connectivity, patient support, administrative process and reporting, reporting requirements, and population health. Table 1 allows a visual representation of the core components along with other variable options available through the EHR [2]:
As seen, the electronic health record is much more than digitized patient records. As the EHR has the ability to provide functions ranging from patient care through supportive administration functions and even the function of informational infrastructure guidance in order to better aid the staff of the practice in clinical and clerical office services.
Integration of Practice Management System
Practice management systems (PMS) have multiple levels of integration with electronic health record systems yet there are difficulties at each level. Levels of integration can range from full system communication sharing all data entered in to each system or select data pending on the organizations need for level of integration from one system to another [3]. To distinguish the two systems, practice management systems provide more clerical functions of a physician’s office such as patient billing profiles, insurance verification, appointment scheduling and sending appointment reminders [3]. From the SWOT analysis, the overall strategy that Barbara should recommend is a system that is capable of meeting the needs of the healthcare facility effectively and efficiently. The strategy focuses on having systems that are fast to allow for easy processing of information and offer quality support to the patients. It should also have a high level of functionality to allow for the normalizing, analyzing, access and the storage of the entire patient's data and saving it for easy retrieving in the future. The system should also be user-friendly so that the professionals and the staff using it can be in a position to easily maneuver in the process of care delivery. The other component of the system that the company should consider is that it should have a wide range of features to enhance maximum utilization and the ease of data access by the patients and physicians. Finally, the medical professionals should also be trained on how to use the system upon implementation.

One of the ways in which the selection of eClinical works EHR will contribute to strategy is that it is the ability to maintain highly organized data; it’s fast and also has amazing features. The EHR system adapted for use in the organization should be in a position to increase effectiveness, efficiency, achieve quality in the delivery of care and also enhance the patient’s outcomes (Sinha et al., 2013). Due to its organization, the system will make it easy for the health care professionals to retrieve the patients’ information while at the same time ensuring security to prevent access of the patient information by unauthorized persons. The e-Clinical works will also contribute to the strategy because it offers low and affordable prices and has low maintenance costs and this aids in the reduction of the costs that the healthcare facilities incur in the maintenance. Compliance in healthcare is the method of observing guidelines, rules, and regulations. Healthcare compliance can cover a wide range of activities, which comply with internal and external regulations. These rules and regulations are put in place to protect patient safety, patient information, and billing practices. Examining quality management, this is a process to improve treatments, outcomes, satisfaction, and the delivery of care. As quality management is put into place, this can help organizations and the healthcare industry identify other policies needed to be implemented to continue towards the goal of improving the overall delivery of care. Quality management tools can be utilized to support compliance efforts throughout an organization and industry.
One type of quality management tool that can be used as a process control technique to support compliance is a control chart. This type of tool is a graph used to study processes and how they change over time. These types of charts can be used with ongoing processes and help by correcting problems as they arise (ASQ, n.d.). Utilizing this tool can help monitor and help set regulations in areas such as hospital-acquired infections, pre and post-operative complications, and mortality rates. This type of tool involves process thinking, learning through data, study, use of experimentation, measurement systems, and methods of data collection (Suman & Prajapati, 2018). When utilized correctly, it can help the organization to determine how successful processes are before and after the implementation of a new EHR system. It can also help create regulations and determine if additional compliance-related regulations are necessary for the future.
The next type of quality management tool that can be beneficial is fishbone diagrams. This type of diagram is a structured approach that helps with identifying problems and sorting different ideas into categories. It can help as a visual tool to display cause and effect and is often also referred to as a cause and effect diagram. At the head of the diagram is the problem with contributing factors branching off separated into the various categories. This tool can be used to help promote an atmosphere of brainstorming and identifying issues that could have potentially been looked over otherwise (API, n.d.). Applications for the organization as it pertains to the implementation of a new EHR system is that it can help identify issues related to the process as well as help understand other factors that might be aiding to the problem. Compliance in healthcare is the method of observing guidelines, rules, and regulations. Healthcare compliance can cover a wide range of activities, which comply with internal and external regulations. These rules and regulations are put in place to protect patient safety, patient information, and billing practices. Examining quality management, this is a process to improve treatments, outcomes, satisfaction, and the delivery of care. As quality management is put into place, this can help organizations and the healthcare industry identify other policies needed to be implemented to continue towards the goal of improving the overall delivery of care. Quality management tools can be utilized to support compliance efforts throughout an organization and industry.
One type of quality management tool that can be used as a process control technique to support compliance is a control chart. This type of tool is a graph used to study processes and how they change over time. These types of charts can be used with ongoing processes and help by correcting problems as they arise (ASQ, n.d.). Utilizing this tool can help monitor and help set regulations in areas such as hospital-acquired infections, pre and post-operative complications, and mortality rates. This type of tool involves process thinking, learning through data, study, use of experimentation, measurement systems, and methods of data collection (Suman & Prajapati, 2018). When utilized correctly, it can help the organization to determine how successful processes are before and after the implementation of a new EHR system. It can also help create regulations and determine if additional compliance-related regulations are necessary for the future.
The next type of quality management tool that can be beneficial is fishbone diagrams. This type of diagram is a structured approach that helps with identifying problems and sorting different ideas into categories. It can help as a visual tool to display cause and effect and is often also referred to as a cause and effect diagram. At the head of the diagram is the problem with contributing factors branching off separated into the various categories. This tool can be used to help promote an atmosphere of brainstorming and identifying issues that could have potentially been looked over otherwise (API, n.d.). The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.

The start/stop/sustain exercise helps clarify what the new work
environment will be like after the change and help the team
prioritize tasks in the overall EHR implementation plan.
Steps in the Planning Phase
Here are some tactical steps that typically occur during the
EHR implementation planning phase
. You may collaborate and use tools provided by your Regional
Extension Center (REC), IT vendor, and/or EHR vendor (if you
already have an existing EHR product) to complete these
activities.
Analyze and map out the practice’s current workflow and
processes of how the practice currently gets work done (the
7. current state).
Map out how EHRs will enable desired workflows and
processes,
creating new workflow patterns
to improve inefficiency or duplicative processes (the future
state).
Create a contingency plan – or back-up plan – to combat
issues that may arise throughout the implementation process.
Create a project plan for transitioning from paper to EHRs,
and appoint someone to manage the project plan.
Establish a
chart abstraction
plan, a means to convert or transform, information from
paper charts to electronic charts. Identify specific data
elements that will need to be entered into the new EHR and
if there are items that will be scanned.
Understand what data elements may be migrated from your
old system to your new one, such as patient demographics or
provider schedule information. Sometimes, being selective
with which data or how much data you want to migrate can
influence the ease of transition.
Identify concerns and obstacles regarding privacy and
security and create a plan to address them. It is essential to
emphasize the importance of privacy and security when
transitioning to EHRs.

", which is the use of certified EHR technology to achieve
health and efficiency goals. This section provides an overview
