This document discusses imaging modalities used in prostate cancer, including transrectal ultrasound (TRUS), multiparametric MRI, CT, and others. TRUS is commonly used to assess prostate size and guide biopsies, but has limitations. Multiparametric MRI, which combines T2-weighted imaging with diffusion-weighted imaging and dynamic contrast enhancement, provides the best imaging for detecting and staging prostate cancer, with a sensitivity of 81% and specificity of 91%. The document recommends a multiparametric MRI protocol using a pelvic phased array coil and optionally an endorectal coil on a 1.5T or 3T magnet for prostate imaging.
This case presentation describes a 57-year-old female who underwent a live donor kidney transplant. Post-operatively, she developed delayed graft function and a urinary tract infection. She later had prolonged abdominal drainage and multiple recurrent infections requiring several hospital admissions. Surgical exploration and marsupialization were ultimately required to treat a large lymphocele that formed post-transplant.
This document provides information about urogynecological fistulas from the Department of Urology at Govt Royapettah Hospital and Kilpauk Medical College in Chennai. It discusses the different types of genitourinary fistulas including vesicovaginal fistula, their causes such as obstetric injuries or gynecological surgeries. It provides details on congenital uterine anomalies, pelvic inflammatory disease, endometriosis, adenomyosis, leiomyomas (fibroids), and endometrial polyps. Transvaginal ultrasound, CT, MRI, and hysterosalpingography are described as methods for diagnosing these conditions. Symptoms can include abnormal bleeding, pelvic pain, and infertility. Early diagnosis is important but can be challenging without invasive methods.
Varicocele is a dilation and twisting of the veins within the pampiniform plexus that drain the testicles. It is commonly seen in 15-30 year old males and is a cause of infertility in around 40% of infertile males. The left side is more commonly affected due to anatomical factors. Treatment involves surgical ligation of the veins through various approaches like inguinal or retroperitoneal. Other options include embolization or laparoscopy. Surgery is recommended for infertile males with a varicocele to potentially improve semen parameters and fertility chances.
Ultrasound elastography of breast and prostate lesionsRajesh Venunath
Ultrasound elastography is a technique that maps relative tissue stiffness. It has applications in evaluating breast and prostate lesions. Breast and prostate cancers are generally stiffer than normal tissues, appearing darker on elastograms. Ultrasound elastography can help differentiate benign from malignant lesions and improve biopsy and therapy guidance for conditions like breast fibroadenomas, cysts, invasive ductal carcinoma, and prostate cancer. It has sensitivities over 68% and specificities over 72% for detecting cancers and abnormalities.

This case report describes a caesarean scar ectopic pregnancy in a 28-year old woman with two previous caesarean deliveries. Ultrasound found the gestational sac located in the lower uterine segment within the region of the previous caesarean scar. 3D imaging confirmed these findings. The differential diagnosis considered cervical ectopic pregnancy and missed abortion but color Doppler showed peripheral flow around the sac, confirming a caesarean scar ectopic pregnancy. Early detection of caesarean scar ectopic pregnancies by ultrasound is important to reduce risks of complications like hemorrhage which can require emergency hysterectomy.
MR imaging is useful for staging prostate cancer once diagnosis is established through biopsy. It allows for identification of extracapsular extension, seminal vesicle invasion, and lymph node involvement. The departmental cases demonstrated various MRI findings of prostate cancer, including low T2 signal in the peripheral zone, restricted diffusion, and increased choline on MR spectroscopy. MRI is more sensitive and specific than other imaging modalities for local staging of prostate cancer when combined with MR spectroscopy.
1) The document discusses guidelines for staging prostate cancer based on Gleason scoring from biopsy and prostatectomy specimens.
2) It also covers updates to screening recommendations, including individualized decision making for men aged 55-59, and improving screening specificity through better use of PSA, imaging, and biomarkers to decrease overdetection.
3) Recent trials demonstrate improved overall survival when adding darolutamide or abiraterone to standard androgen deprivation therapy and docetaxel chemotherapy for metastatic castration-sensitive prostate cancer.
This document describes the renogram procedure. It provides details on:
- The radiopharmaceuticals used, including 99mTc-DTPA, 99mTc-MAG3, and 99mTc-DMSA
- How the procedure is performed, including patient preparation, image acquisition, and time-activity curve analysis

- Factors that can affect the procedure such as hydration, medications, and kidney positioning
This seminar discusses carcinoma penis. Dr. A.I. Mazumder is the moderator and Dr. Biswajit Deka is the presenter. Carcinoma penis is uncommon but psychologically devastating when diagnosed. It is important to differentiate between benign, premalignant, and malignant conditions. Metastasis often occurs if diagnosis or treatment is delayed and can be lethal. The cancer typically spreads slowly at first and may not cause symptoms, allowing it to progress before being found.
Scrotal ultrasound is used to evaluate pathology in the scrotum such as palpable masses, pain, asymmetry, or trauma. The testicles are examined in both transverse and sagittal planes using ultrasound to distinguish various pathologies. Common findings include extratesticular masses like hydroceles, varicoceles, and spermatoceles as well as intratesticular conditions such as testicular torsion and epididymitis. Doppler ultrasound can detect reduced or absent blood flow that indicates potential testicular ischemia from conditions like torsion.
Morbidly adherent placenta is an abnormal placental attachment that can cause life-threatening bleeding. It is most commonly seen in women with a history of cesarean delivery and placenta previa. Ultrasound is the gold standard for diagnosis and can identify thinning of the myometrium and abnormal vasculature. When morbidly adherent placenta is suspected, delivery at a tertiary care center with a multidisciplinary team is recommended to manage the significant risks of hemorrhage. Leaving the placenta in situ during cesarean hysterectomy is typically the safest approach to minimize blood loss.
This document summarizes treatment options for ovarian cancer after diagnosis. It discusses that most cases are advanced at presentation and surgery is used to stage and debulk the cancer. Chemotherapy with a taxane and platinum agent is recommended for early and advanced stage disease. For advanced cancer, intraperitoneal chemotherapy may provide a survival benefit compared to intravenous treatment alone. Recurrent cancer can be treated with additional chemotherapy or surgery depending on factors like time since last treatment and extent of recurrence. Future research aims to improve treatment tolerability and identify new targeted therapies.

This document discusses colpocleisis, a surgical procedure for advanced pelvic organ prolapse. It provides a brief history of the procedure and describes it as involving denuding and suturing the anterior and posterior vaginal walls. The document outlines the procedure's prerequisites, advantages like being simple and having low morbidity, and outcomes like high anatomic success and patient satisfaction rates. Potential concerns discussed include regret, body image, and worsening urinary incontinence. Alternatives like pessaries or anatomical repairs are mentioned as being more complex or palliative rather than curative. Unanswered questions about the procedure are also noted.
Rigid and flexible cystoscopes and ureteroscopes are used to visualize the urinary tract. Rigid cystoscopes use a rod lens system for imaging while flexible ones use fiber optic bundles. Key parts include the sheath, bridge, telescope and obturator. Rigid cystoscope sheaths come in various sizes and have markings to measure the prostatic urethra. Flexible scopes have advantages like better patient comfort and ability to navigate difficult anatomy. Digital flexible ureteroscopes now have a camera chip at the tip for improved imaging. Accessories include graspers, baskets and lasers that are compatible with working channels.
This document provides tips for surgeons starting and improving their skills in laparoscopic colorectal surgery. It discusses acceptable conversion and complication rates, as well as operating parameters. It recommends starting with basic laparoscopy training before applying skills to colorectal procedures. Choosing appropriate early cases and allowing extra time are also suggested. Tips include accurate pre-op planning, adhering to oncological principles, and being versatile with port and incision placement. Operating with experienced colleagues, reviewing cases, and auditing results can help surgeons continue improving.
Presentation1.pptx, radiological imaging of female infertility.Abdellah Nazeer
This document discusses radiological imaging for evaluating female infertility. It begins by defining infertility and describing its prevalence. Imaging plays a key role in the diagnostic evaluation of infertility in women. Hysterosalpingography is often the initial study to evaluate fallopian tube patency. Additional imaging with ultrasound, MRI, or hysterography may be needed to further characterize any abnormalities. The document then discusses various causes of infertility related to the uterus, ovaries, fallopian tubes, and hormones. It describes imaging findings for specific conditions like endometriosis, polycystic ovaries, and Müllerian duct anomalies. The appropriate use of imaging modalities is important for directing patient care and management of infertility.

Minimally invasive and endoscopic management of benign prostaticDr. Manjul Maurya
The document discusses various minimally invasive procedures for treating benign prostatic hyperplasia (BPH), including transurethral resection of the prostate (TURP), bipolar TURP, and prostatic urethral lift. TURP uses an electrified loop to remove prostatic tissue, while bipolar TURP incorporates both the active and return portions on the same electrode to avoid risks of traditional TURP like TUR syndrome. Prostatic urethral lift mechanically opens the urethra using permanent implants rather than ablating tissue. The document provides details on techniques, risks, and benefits of these various procedures for treating BPH.
Urethral strictures are more commonly seen in the anterior urethra. They are commonly seen secondary to gonococcal urethritis or trauma. The normal urethral lumen is 4mm or less in diameter and has small thin walls. A stricture appears as a segment of narrowed lumen with irregularity and thickening of the wall due to fibrosis and scarring.
Presentation11, radiological imaging of ovarian torsion.Abdellah Nazeer
Ovarian torsion refers to the twisting of an ovary on its vascular pedicle, which can cut off its blood supply. It is a gynecological emergency that requires urgent surgery. Radiological imaging plays an important role in the diagnosis. Ultrasound is usually the initial imaging method, showing signs such as an enlarged ovary without blood flow. CT and MRI can further evaluate for complications like hemorrhage or infarction. Prompt diagnosis and treatment are needed to prevent ovarian necrosis from the loss of blood supply.

17. Alt Üriner Sistem Obstriksiyonu
(LUTO) - I
• LUTO, sıklıkla postüretral valv
(PUV) ve üretral atreziye
bağlıdır
• Renal anomalilerin 1/3’ünü
oluşturur
• PUV, 5000-8000 canlı erkek
doğumda 1 görülür
• 1250 fetal USG de 1PUV tespit
edilir
• Tedavi edilmezse perinatal
fetal mortalite % 95’e yakındır
• Mortalitenin önemli nedeni
akciğer hipoplazisi ve ciddi
oligohidroamniosa bağlı gelişir

Perkütanöz vesicoamniotic şant, obstrüksiyonu
tedavi etmek için kullanılan en iyi metod
Bir double pigtailed catheter (JJ stent, double J
stent or pig-tail stent) (either Rodeck/Rocket or
Harrison shunts) usg ve lokal aneztezi altında
yerleştirilir
Şantın distal ucu fetal mesaneye ve proksimal ucu
amniyotik kaviteye yerleştirilir
Amaç idrarın amniotik kaviteye drene olmasını
sağlamaktır
Ciddi oligohidramniyoz varsa şant yerleştirmeden
önce şantın yerleştireceği bir alan oluşturmak için
Amniyoinfusion gerekebilir
LUTO için vesicocentesis, vesico-amniotic shunting ve
open fetal bladder surgery sonuçlarını araştıran
çalışmalarda vesico-amniotic drainage
sonuçları ile en iyi neticeler elde edildiği
gösterilmiştir
19. Alt Üriner Sistem Obstriksiyonu
(LUTO) - III
• Çoğu vaka, olgu sunumu düzeyinde (kontrol
grupları yok)
• Bu amaçla PLUTO isminde bir RCT planlandı
orta-ciddi LUTO vakalarında veziko-amniotik
şantın sonuçları bekleniyor
– Araştırmanın primer sonuçları kreatinin seviyesi ve
perinatal mortalite iken, sekonder sonuçları
postnatal reflü derecesi, mesane duvarı kalınlığı ve
renal dilatasyon olarak belirlenmiş

• 1/3000 – 5000 canlı doğumda (hidden mortality)
• Tahmini 1/2200
• %40 vakada diğer kromozomal ya da anatomik
patolojiler eşlik eder
– Kromozomal analiz
– Fetal detaylı usg
– Fetal ekokardiyografi
• En çok solda (%85-95), %10 sağda, %2 bilateral
28. Konjenital Diyafragmatik Herni (CGH) - II
•
Erken tanı alan CDH tedavi edilmeden bırakılırsa fetal ölüm
oranı yüksektir ve postnatal mortalite yaklaşık % 30
civarındadır
– pulmoner hipoplazi
– persitant pulmoner hipertansiyon
•
Prognozu belirlemede
– Akc ve kafa oranı (LHR)
•
•
•
Gebelik haftasından bağımsız olarak Akc hipoplazi derecesini gösterir
O/E LHR oranı %15 altında olan vakalarda ciddi perinatal
problemler olmaz
%15-25 arası olgularda ciddi pulmoner hipoplazi ile karşı karşıya
kalınabilir
• %25 üzeri olgularda hayatta kalma oranı %60’a kadar
gerilemektedir.
– diğer Akc büyüklüğün karşılaştırılması

İntrauterin tedavide amaç; trakeanin kapatılması yoluyla pulmoner parankimal
hiperplazi yaratarak ve hücre büyümesini tetikleyen akciğer sıvısının akciğerde kalıp
damarlanmayı arttırmasını sağlayarak akciğer gelişimini kontrol etmektir.
Fetoscopic Endoluminal Tracheal Occlusion (FETO)
European FETO konsorsiyumu ciddi O/E oranı olan ve yaşam oranı % 15 olan LHR
grubuna önermektedir
210 FETO uygulanan fetus ortalama 27 hf doğurtuldu

•
37. haftanın altında doğum oranı % 47.1
% 97.1 canlı doğum ve %48 cerrahi sonrası canlı taburcu oranı
İlk çalışma sonuç belirleyicileri olarak O/E LHR oranı membranların prematür rüptürü ve gestasyonel
yaş olarak belirlenmiştir
Post FETO Akc alanının genişlemeside iyi prognoz faktörü olarak bulunmuştur
Geç haftalarda FETO yapılması prematür doğum risikini azaltır ve uygun akc gelişimine izin verir.
Şimdi moderate pulmoner hipoplazisi olan fetuslarda FETO çalışması avrupada
sürdürülmektedir.

33. AKC Anomalileri - II
• Bazı yazarlar CCAM volümü ve baş oranını (HC)
prognostik faktör olarak önermiştir.
–
–
–
–
CCAM volümü (CVR) (=kitle en x boy x derinlik x 0,52)
CVR ≤1,6 ise hidrops olasılığı %3’den az
CVR >1,6 ise hidrops olasılığı %75’den fazla
bu öneri tartışmalıdır ve üniversal kabul görmemiştir
• Thoracoamniotik şant,1987
• Şant yerleştirilmesinin ana endikasyonu semptomatik
makrokistik CCAM veya plevral effüzyondur
• Şant Sonuçları ile % 70 survival oranları bildirilmiştir
34. AKC Anomalileri - III
• Solid AKC lezyonları mekanik bası, mediastinal
kayma, kaval obstrüksiyona neden olabilirler, bu
durumda besleyici damarın antenatal laser
kaogülasyon ile tedavisi yapılabilir
• Lazer kaogülasyon lezyonun tam rezolüsyonuna
neden olabilir
• Bunula beraber büyük multikistik veya özellikle
solid lezyonlar hidropsla beraber olduğunda
rezeksiyon sonuçları daha başarılıdır
• Postnatal survival oranları 50%’ye kadar
çıkmaktadır.

36. İkizden İkize Transfüzyon Sendromu
(TTTS) - I
• TTTS evreleme:
– Quintero evrelemesi
•

39. Nöral Tüp Defektleri - I
• Spina bifida veya myelomeningosel
(MMC) SSS’nin konjenital defektidir.
• Amerika’da 2900 canlı doğumda bir
görülmektedir.
• Alt ektremitede paralizi, barsak ve
mesane fonksiyonlarında bozulma,
öğrenme kusurları gibi klinik problemlere
neden olur.
• MMC, SSS’nin defektif gelişmesinden olur
ve hasarın kendisinin yanısıra amniyon
sıvısı teması, bası veya doğrudan travma
gibi sekonder etkilere bağlı olarak klinik
sonuçlar değişir.
• Erken fetal cerrahi sekonder hasarları
azaltılabilir ya da ortadan kaldırabilir.
40. Nöral Tüp Defektleri - II
•
Adzick's ve ark. yaptığı 58
olguluk bir çalışmada, cerrahi
sonrası 3. haftada MRI
incelemesinde, tüm hastalarda
hindbrain’de cerrahi öncesi
izlenen herniasyonun tamamen
kaybolduğu gözlenmiştir.
• Gebeliğin 19-25 hf’larında yapılacak
bir cerrahinin sekonder hasarı
azaltacağı gösterilmiştir.
• Bu gebelik haftasından önce fetal
dokular oldukça frajıldır ve cerrahi
onarım oldukça zor olur.
• Erken girişim hidrosefali gelişim riskini
durdurması veya azaltması açısından
oldukça önemlidir.
• Postnatal dönemde
ventriculoperitoneal (VP) şant takılma
oranı, tedavi edilen olgularda % 46
iken tedavi edilmeyenlerde % 84’ü
geçmektedir. Yine antenatal tedavi
edilenlerde alt ekstremite
fonksiyonları daha iyidir, % 66’lık ciddi
bir kısım destek almadan yürüyebilir.

•
SCT posterior koksikste lokalize Henson nodundaki
ekojenitede (kistik dejenerasyon, internal
hemoraji,kalsifikasyon, artmış vaskülarite) görülür.
Tanıda Usg yeterlidir. MRI ile diğer organ ilişkileri ve
intrapelvik komponent varlığı ayrımı daha net yapılır.
SCT, American Academy of Pediatrics Surgical Section
sınıflaması ile 4 grupta incelenir.
– Tip 1 SCT tamamen external komponentli
yoktur.

• Fetal cerrahi tedavi esas olarak yüksek riskli gruba;
– 27. hf’dan önceki gebelere, Tip 1 vakalara
– Yüksek fetal kardiak output fizyolojisini değiştiren durum
varlığında ve/veya
– Erken fetal hidrops olgusu varlığında uygulanır.
• Tedavide amaç optimal debulking cerrahisidir.
• Cerrahi süresi ort. 15 dk’dır.
• Düşük risk saptanan olgular seri izlemle takip edilerek 36. haftadan
sonra elektif CS ile doğurtulur.
• Yine tip 2,3,4 yüksek riskli olgularda yaklaşım; ciddi maternal ya da
fetal fizyolojiyi etkileyen vasküler problemler, polihidroamnios, fetal
