Focusing on Patient-Centered Care
During several periods of growth, Family Physicians Group
brought its organization to the next level with the help of
one proven technology partner.
Orlando-based Family Physicians Group (FPG) is Central Florida’s largest
primary care physicians group. With over 60 physicians at 26 offices from
the Gulf coast to the Space coast, FPG has grown to become a leader
within an emerging healthcare model known as the Accountable Care
Organization (ACO). ACO’s voluntarily assume responsibility for both the
quality and cost of health care, for a defined population of patients. Unlike
the traditional fee-for-service reimbursement model, which pays providers
for each case service delivered, an ACO model directly ties payment to
outcomes and the providers’ abilities to deliver care in an efficient manner.
The Quest for Better Coordinated Care
In 2012, FPG entered into an agreement with The Centers for Medicare and
Medicaid Services (CMS) to become Central Florida’s first ACO. FPG’s
newly appointed CIO, Dr. Khalid Moidu, quickly realized that the key to
being able to make this new business model work was an information
technology strategy that centered on process, information sharing and
standardization. For FPG synchronizing 26 individual locations with
separate systems, technology, and workflows was increasingly challenging.
Dr. Moidu believed that with the right technology supporting a standard
delivery model, FPG would be successful in meeting the care quality goals

demanded by management. As the clinical information leader, his first step
was to review and rebuild processes to support a standard workflow across
the 26 clinical sites. In an effort to improve support to the care providers,
FPG’s infrastructure was standardized to include enhanced EMR
performance, a high-performance Wide Area Network (WAN) and improved
communications. Key to these deployments was a training program to
enhance the proficiency of the clinical team on navigating the EMR,
understanding the revised processes and meaningful use of the data.
Enabling Growth and True Community Connectivity
To support this transition, Dr. Moidu knew that he could no longer rely on a
distributed vendor model with multiple contracts and dispersed support.
After a rigorous selection process, Dr. Moidu chose to partner with Bright
House Networks Enterprise Solutions as they had both the breadth and
depth to support the ambitious technology initiatives needed. Key to his
decision was Enterprise Solutions ability to not only provide an end-to-end
managed WAN solution providing a high-speed data fiber network across all
offices; but also for their understanding and ability to deliver the HIPAA
security requirements mandated; a hosted voice solution that provides the
ability to standardize communications across all locations; and the ability to
support a new telehealth monitoring initiative. Partnering with Enterprise
Solutions allows Dr. Moidu to closely coordinate initiatives and as he puts
it, provides “one throat to choke”, allowing FPG to operate at the speed
required.
Proof of Success – Synchronized, Integrated Coordination of
Quality Care
The results have been impressive. FPG’s new fiber network provided by
Enterprise Solutions is allowing the organization to standardize processes
across the clinics enabling data sharing in ways previously unimaginable.
Processes that used to be manual are now becoming automated.
Documents are now more easily shared, patient tracking across clinics
through the EMR is more efficient and the ability to provide analytics across
the organization has led to better decision making. The hosted voice
solution now allows calls to be routed seamlessly across the organization,
dramatically improving customer care coordination. With the help of
Enterprise Solutions, FPG now delivers more efficient, better quality patient
care, and with its technology-enabled shared infrastructure in place, FPG
has successfully grown into a data-driven care team focused on
collaborating and coordinating care to improve outcomes for their patient
population.

Orlando-based Family Physicians Group (FPG) is Central Florida’s largest primary care physicians group.
