خانم هایی که هیچ گونه سابقه ابتلا به عفونت یا التهاب لگن را ندارند می توانند از IUD برای جلوگیری از بارداری استفاده نمایند. خانم هایی که تا کنون هیچ گونه حاملگی نداشته اند به خاطرعوارض خون ریزی و انقباض های رحمی که ممکن است بر اثر IUD ایجاد شود به میزان بیشتری از سایر خانم ها ممکن است خواهان عدم استفاده از IUD شوند. همچنین در این خانم ها احتمال افتادن IUD به بیرون بیشتر از خانم هایی است که قبلا سابقه حاملگی داشته اند.
گاهی اوقات بعضی از خانم هایی که از IUD استفاده می کنند ممکن است در طی قاعدگی های خود دچار انقباض های شدید رحمی، افزایش خون ریزی و کمر درد و یا پشت درد شوند. مصرف قرص های ایبوبروفن درهنگام شروع قاعدگی می تواند به کاهش این مشکلات کمک نماید. حدود 10 تا 15 در صد خانم ها به خاطر بروز این گونه دردها و خون ریزی ها در طی سال اولی که از IUD استفاده کرده اند اقدام به بیرون آوردن آن می کنند. در طی سال های بعدی این میزان کاهش یافته و به 3 درصد می رسد. خانم هایی که قبلا بچه هایی به دنیا آورده اند در هنگام مصرف IUD کمتر دچار خون ریزی و درد می شوند و بنابراین احتمال کمتری دارد که به این دلایل اقدام به بیرون آوردن IUD نمایند.
1- عفونت یا التهاب لگن
یک عفونت جدی رحم، لوله فالوپ و تخمدان ها می باشد که می تواند باعث نازایی، عقیم شدن و دردهای مزمن شود. خطرایجاد بیماری التهاب لگن در طی 20 روز اول نصب IUD بسیار زیاد می باشد. در طی این مدت از هر 1000خانمی که IUD گذاشته است 10 نفر به التهاب لگن دچار می شود اما از روز 21 به بعد این میزان کاهش یافته و از هر 1000 خانم، فقط یک نفر بر اثر نصب IUD دچار التهاب لگن می گردد.
علایم و نشانه های التهاب لگن عبارت هستند از: درد های شکمی، درد در هنگام مقاربت، خون ریزی یا ترشح از مهبل و تب. در هنگام مواجه شدن با چنین علائمی، سریعا با پزشک خود تماس بگیرید.
2- سوراخ شدن رحم
در موارد نادری ممکن است ایجاد شود. درچنین مواقعی IUD با سوراخ کردن رحم وارد شکم می گردد و ممکن است نیاز به انجام عمل جراحی و بیرون آوردن آن باشد.
3- بیرون افتادن IUD
در طی سال اول نصب IUD حدود 2 تا 10 در صد می باشد که اکثر آن ها نیز در طی ماه اول اتفاق می افتد. در خانم هایی که سابقه به دنیا آوردن بچه هایی را دارند فقط 2 تا 3 در صد احتمال وقوع چنین حالتی وجود دارد.
بعداز سال اول، بیرون افتادن IUD از رحم به خارج آن به میزان زیادی کاهش می یابد و به حدود یک تا سه در صد می رسد. اگر بیرون افتادن IUD بدون این که خانم متوجه آن شود اتفاق بیفتد، ممکن است حاملگی روی دهد.
4- وقوع حاملگی
اگر با وجود IUD در داخل رحم، حاملگی رخ دهد باید سریعا اقدام به بیرون آوردن آن نمود. ادامه حاملگی همراه با IUD خطر سقط خود به خودی را دو برابر و خطر زایمان زودرس را چهار برابر می نماید.
خانم هایی که با وجود IUD حامله می شوند احتمال بیشتری دارد که به حاملگی خارج رحمی دچار شوند(حدود 5 در صد). عدم ایجاد قاعدگی در طی یک ماه، خون ریزی های نامنظم و بروز علائم حاملگی( مثل تهوع و استفراغ، حساس شدن پستان ها، دفع مکرر ادرار و خستگی) را باید به پزشک گزارش نمود. حاملگی خارج رحمی در صورتی که به موقع درمان نگردد می تواند باعث مرگ شود.
نولی پار:زنی که هرگز یک مورد حاملگی را تا بعد از هفته ی ۲۰ ادامه نداده است.
PID: Pelvic Inflammatory Disease (PID)
مصرف بسته بعدی پس از يک فاصله 7 روزه آغاز خواهد شد، ديگر مهم نيست که در اين 7 روز چه زمانی قاعدگی رخ می دهد. مهم اين است بين مصرف دو بسته قرص بايد 7 روز کامل فاصله و استراحت باشد.
قرص های تری فازیک هم مانند منوفازیک ها در بسته های 21تایی یا 28تایی می باشند.
و فقط در 6 ماهی که خانم به طور کامل شیر میدهد مؤثر هستند. یعنی اگر کسی شیر خشک میدهد تأثیر آن بسیار کمتر میشود و بهتر است از روش دیگری هم زمان استفاده کند.

از تقسیم (وزن بدن به کیلوگرم ) بر اندازه (قد به متر) به توان ۲، BMI بدن فرد محاسبه می گردد.
قرص صبح بعد از نزدیکی یا قرص های اورژانسی
این قرص ها فقط درموارد اورژانسی کاربرد دارد مثل پاره شدن کاندوم. و نباید به عنوان روش پیشگیری در مقاربت های عادی استفاده شود
در این روش تجویز هورمون استروژن به میزان بالایی تجویز می شود.
1- قرص HD ، 2 عدد در اولين فرصت و 2 عدد 12 ساعت بعد.
2- قرصLD ، 4 عدد در اولين فرصت، و 4 عدد 12 ساعت بعد
3- قرص تری فازيک ، 4 عدد قرص سفيد در اولين فرصت و 4 عدد قرص سفيد 12 ساعت بعد.
امپول B6 بصورت عضلانی در کاهش تهوع موثر است.
قرص های LD دوز پایین نسبت به HD را دارد.
یک برش کوچک با بی حسی موضعی و در شرایط کاملا استریل قرار می گیرند

توجه داشته باشید که یک سیکل قاعدگی، از روز نخست پریود شدن تا روز قبل از پریود شدن بعدی محاسبه میشود. به عبارت دیگر، تعداد روزهای بین یک پریود، تا پریود بعدی را یک سیکل قاعدگی می نامیم.
بعد از اینکه شروع پریود در تقویم علامت زده شد، تعداد روزهایی که سیکل بطول انجامیده است محاسبه شود. برای شمارش تعداد روزها باید از روز نخست پریود، تا روز قبل از شروع پریود بعدی را شمارش کنید.
برای مثال، در تصویر بالا روز ۹ تیر آغاز سیکل میباشد و اگر طول سیکل را ۲۸ روز در نظر بگیریم، آخرین روز سیکل برابر است با ۵ مردادماه و شروع سیکل بعدی (پریود بعدی) ۶ مرداد خواهد بود.
این روش را برای ۸ (هشت) تا ۱۲ (دوازده) سیکل ادامه داده شود. هرچه تعداد سیکلهای محاسبه شده بیشتر باشند، دقت محاسبه ی زمان تخمک گذاری بالاتر خواهد بود.
روز شروع پریود و طول دوره ی هر سیکل را به صورت جدول تهیه کنید.
این جدول دارای دو ستون می باشد. ستون نخست محتوی روز آغازین پریود و ستون دوم تعداد روزهایی است که آن سیکل طول کشیده است. توجه داشته باشید که تعداد روزهای سیکل با تعداد روزهایی که خونریزی مشاهده میکنید متفاوت است. در این جدول باید تعداد روزهایی را که سیکل شما طول کشیده است وارد کنید، نه تعداد روزهایی که خونریزی مشاهده کرده اید.
در جدول فوق، از ستون دوم، بزرگترین عدد را پیدا کنید. سپس عدد ۱۱ را از بزرگترین عدد بدست آمده از جدول فوق کسر کنید. به تعداد روزهای عدد حاصل شده، از روز نخست شروع سیکل فعلی خود بشمارید. این روز، روز آخر از بازه ی زمانی است که میتوانید اقدام به باردار شدن کنید.
کوچکترین عدد مربوط به سیکل ۵ است که برابر با ۲۷ میباشد. بنابراین اگر طبق فرمول گفته شده عدد ۱۸ را از ۲۷ کسر کنیم، حاصل عدد ۹ خواهد بود.
بزرگترین عدد جدول برابر است با ۳۲ و اگر طبق فرمول عدد ۱۱ را از آن کسر کنیم حاصل عدد ۲۱ خواهد بود. پس روز آخر برابر است با ۲۱ برج. بنابراین شانس باردار شدن شما در بازه ی تاریخی بین 9تا ۲۱ آن ماه در حداکثر خود قرار دارد.
تنها 10 % زنان دارای چرخه قاعدگی (دقیقا) 28 روز میباشند.
