This study analyzed clinical data from 101 COVID-19 patients hospitalized in Wuhan, China between February and April 2020. The patients were classified as having moderate (n=47), severe (n=36), or critical (n=18) illness. Older age, pre-existing conditions like diabetes and hypertension, and higher blood levels of inflammatory markers were associated with more severe illness. Multivariate analysis identified older age, diabetes, chronic liver disease, and elevated neutrophil, C-reactive protein, D-dimer, and interleukin levels as independent risk factors for severe COVID-19.

This study examined the relationship between symptom onset to admission time (SOAT) and the severity of COVID-19 through a meta-analysis and retrospective analysis. The meta-analysis found that patients with adverse outcomes had a longer SOAT than those without adverse outcomes. The retrospective analysis initially found no significant difference in SOAT between severe and mild COVID-19 patients. However, after adjusting for confounding factors using propensity score matching, patients in the severe group were found to have a longer SOAT. This suggests that shortening SOAT may help reduce progression to severe illness in COVID-19 patients.

Background: With the widespread use of highly active antiretroviral therapy, the epidemic of HIV has evolved into a chronic disease. HIV is extremely stigmatizing, resulting in highly emotionally charged responses to disclosure. World Health Organization (WHO) recommends that children should be informed of their HIV status at ages of 6 to 12 years and full disclosure at about 8 to 10 years. Disclosure process is much more difficult when the person being disclosed to is an adolescent. However, disclosure of HIV to a child should be an ongoing process that may last several years depending on the cognitive development of the child.
Methods: This study investigated the determinants of HIV status disclosure among HIV infected adolescents. A total of 209 HIV infected adolescents (10-19 years) who have been on treatment for at least six months, and are taking lifelong anti-retroviral therapy from Bondo County Hospital, Got Agulu and Uyawi Sub County Hospital in Bondo Sub County were enrolled. Simple random sampling was employed in selecting the adolescents. Data was collected using a structured questionnaire. Quantitative data was analysed using both descriptive and inferential statistics while statistical tests including Pearson Correlation analysis and multiple linear regression were used to test the hypotheses.
Results: Findings on the overall parental perceptions regarding risks and benefits of disclosure and disclosure of HIV status to adolescents show that 180 (86.12%) of the respondents had a negative attitude compared to 29 (13.88%) who held a positive attitude. 122 (58.37%) of the respondents believed that overall availability and quality of counselling was moderate. 10 (4.78%) of the respondents believed that the overall availability and quality of counselling was high. Quality services and perception of the parents have been found to be good predictors of disclosure of HIV status among the newly diagnosed adolescents in Bondo sub-County, p-value<0.05.
Conclusion and recommendation: This study identified quality of service and perception of the parents as the two factors determining the disclosure of HIV status. There is a correlation between the parental perceptions regarding risks and benefits of disclosure and the quality of counselling to parental disclosure of HIV status to adolescents. Therefore the study recommends deliberate efforts to ensure quality service delivery and age specific disclosure counselling to caregivers to equip them with adequate knowledge on disclosure.

Current State of Evidence: Vitamin D and Immunity During the COVID-19 Pandemic
Vitamin D and immunity were discussed in the context of COVID-19. There is emerging evidence that vitamin D status may be associated with COVID-19 incidence and severity, however available studies have limitations and cannot prove causation. Maintaining adequate vitamin D through diet and sunlight is recommended for the general population, while more research is still needed, including randomized controlled trials, to fully understand any relationship between vitamin D supplementation and COVID-19 prevention or treatment. In the meantime, identifying and treating vitamin D deficiency is suggested, regardless of links to respiratory infection.

This study analyzed 3,615 patients who tested positive for COVID-19 at a large New York City hospital system between March 4-April 4, 2020. The researchers found that among patients under 60 years old, those with a BMI between 30-34 were twice as likely to be admitted to the hospital and 1.8 times as likely to require critical care compared to those with a BMI under 30. Patients under 60 with a BMI over 35 were 2.2 times as likely to be hospitalized and 3.6 times as likely to need critical care, indicating that obesity is an under-recognized risk factor for worse COVID-19 outcomes in younger patients.

This study aimed to estimate the seroprevalence of COVID-19 among individuals in Daegu, South Korea who had never been diagnosed with COVID-19. The study tested 198 patients and guardians for COVID-19 antibodies. It found an estimated seroprevalence of 7.6%, with 15 positive cases. Most positive cases did not experience COVID-19 symptoms. Based on this rate, the estimated number of undiagnosed COVID-19 cases in Daegu was about 12 times the number of confirmed cases. Seroprevalence was higher among smokers and diabetics. This suggests the number of undiagnosed cases was substantial even with South Korea's aggressive testing and contact tracing strategy.

The study found that 21% of patients experienced diarrhea, with those patients more likely to have severe symptoms and require intensive care. Background: With the widespread use of highly active antiretroviral therapy, the epidemic of HIV has evolved into a chronic disease. HIV is extremely stigmatizing, resulting in highly emotionally charged responses to disclosure. World Health Organization (WHO) recommends that children should be informed of their HIV status at ages of 6 to 12 years and full disclosure at about 8 to 10 years. Disclosure process is much more difficult when the person being disclosed to is an adolescent. However, disclosure of HIV to a child should be an ongoing process that may last several years depending on the cognitive development of the child.
Methods: This study investigated the determinants of HIV status disclosure among HIV infected adolescents. A total of 209 HIV infected adolescents (10-19 years) who have been on treatment for at least six months, and are taking lifelong anti-retroviral therapy from Bondo County Hospital, Got Agulu and Uyawi Sub County Hospital in Bondo Sub County were enrolled. Simple random sampling was employed in selecting the adolescents. Data was collected using a structured questionnaire. Quantitative data was analysed using both descriptive and inferential statistics while statistical tests including Pearson Correlation analysis and multiple linear regression were used to test the hypotheses.
Results: Findings on the overall parental perceptions regarding risks and benefits of disclosure and disclosure of HIV status to adolescents show that 180 (86.12%) of the respondents had a negative attitude compared to 29 (13.88%) who held a positive attitude. 122 (58.37%) of the respondents believed that overall availability and quality of counselling was moderate. 10 (4.78%) of the respondents believed that the overall availability and quality of counselling was high. Quality services and perception of the parents have been found to be good predictors of disclosure of HIV status among the newly diagnosed adolescents in Bondo sub-County, p-value<0.05.
Conclusion and recommendation: This study identified quality of service and perception of the parents as the two factors determining the disclosure of HIV status. There is a correlation between the parental perceptions regarding risks and benefits of disclosure and the quality of counselling to parental disclosure of HIV status to adolescents. Therefore the study recommends deliberate efforts to ensure quality service delivery and age specific disclosure counselling to caregivers to equip them with adequate knowledge on disclosure.

Current State of Evidence: Vitamin D and Immunity During the COVID-19 Pandemic
Vitamin D and immunity were discussed in the context of COVID-19. There is emerging evidence that vitamin D status may be associated with COVID-19 incidence and severity, however available studies have limitations and cannot prove causation. Maintaining adequate vitamin D through diet and sunlight is recommended for the general population, while more research is still needed, including randomized controlled trials, to fully understand any relationship between vitamin D supplementation and COVID-19 prevention or treatment. In the meantime, identifying and treating vitamin D deficiency is suggested, regardless of links to respiratory infection.

This study analyzed 3,615 patients who tested positive for COVID-19 at a large New York City hospital system between March 4-April 4, 2020. The researchers found that among patients under 60 years old, those with a BMI between 30-34 were twice as likely to be admitted to the hospital and 1.8 times as likely to require critical care compared to those with a BMI under 30. Patients under 60 with a BMI over 35 were 2.2 times as likely to be hospitalized and 3.6 times as likely to need critical care, indicating that obesity is an under-recognized risk factor for worse COVID-19 outcomes in younger patients.
