The concept of a knowledge worker
The term “knowledge worker” was first coined by Peter Drucker. Ducker defined knowledge workers as high-level workers who apply theoretical and analytical knowledge, acquired through formal training, to develop products and services (CFI, 2019). He emphasized that due to the high level of productivity and creativity of knowledge worker, that they would be the most valuable assets in the 21st century organization. Professionals that can be referred to as knowledge worker includes engineers, pharmacists, architects, financial analysts, public accountants, physicians, scientists, design thinkers, and lawyers. Furthermore, knowledge workers have a high degree of expertise, experience, education and the primary purpose of their jobs involve the distribution, creation and application of knowledge.

Over the course of your undergraduate degree you have been taught about your scope of practice as
a student and the scope of practice of registered nurses under many different frameworks – legal,
ethical, practical and so on. It is now time to consider how the change in your roles will influence
your scope of practice. Historically the scope of practice of nurses was clearly defined. In the
current contemporary model of health practice the professional boundaries between the health
professions is becoming increasing blurred (Fedoruk & Hoffman, 2013 p24). The issue is further
compounded due to the scope of nursing practice expanding with roles such as advanced practice
nurse, as well as the increase in semi-independent practitioners in community settings.
In this time of transition you must be careful that you understand the scope of practice of the role
you aspire to, that of an experienced practicing Registered Nurse and the role you are about to
undertake that of new registered nurse. Benner describes the new graduate as an advanced
beginner one who is competent but relies on the rules they have been taught to guide them in their
practice. Advanced beginners are seen as nurses who have a wide theory base of practice but are
still developing the ability to make content dependant judgements (Benner, 1984 p 21-22). This
means that you should not set the bar too high for yourselves and realise that you are still learning
and developing the clinical judgement skills that can only come from exposure to clinical situations,
known as experiential learning. If we take Benner at her word then it takes at least 2-3 years of
clinical experience following registration as a registered nurse to be deemed competent and even
then she says the competent nurse will lack speed and efficiency (Benner, 1984 p 26-27).

SBAR communication tool is an effective and simple communication method that works across all the healthcare disciplines to create a safer working environment. Müller et al. (2018) reveal that using SBAR enhances efficacy, accuracy, and efficiency of handoff reports which boosts patient safety by improving communication and encouraging patient involvement Ineffective, absent, or inadequate communication leading to patient safety concerns has become a big problem in many care facilities, including where I work. According to Herawati et al. (2018), poor patient handoff communication from caregivers reduces patient safety. Regardless of the technological advancements, interdisciplinary care providers in many care facilities still do not communicate effectively, threatening patient's safety.
The use of written handoff using SBAR was implemented two years ago in the hospital I work. The handoff is easily accessible by all interdisciplinary team directly working with the patient and this is also used as a communication tool when giving report. According to Uhm et al. (2019), implementing a well-developed SBAR program of communication grounded on learning theory improves nurses' communication skills and boosts their confidence during patient handover. All the team member in the hospital identified the implementation of SBAR (Situation-Background-Assessment-Recommendation) as a solution for the persisting communication problem among caregivers, especially during patient handoffs within the hospital. Without current best evidence, practice is rapidly outdated, often to the detriment of the patient. Evidence based practice is the conscientious use of current best practice in making decisions about patient care (Sackett, Richardson, Rosenberg, & Hayes, 2000). It is important for health care professionals to ask questions about their current clinical practice. The concept of a knowledge worker
The term “knowledge worker” was first coined by Peter Drucker. Ducker defined knowledge workers as high-level workers who apply theoretical and analytical knowledge, acquired through formal training, to develop products and services (CFI, 2019). He emphasized that due to the high level of productivity and creativity of knowledge worker, that they would be the most valuable assets in the 21st century organization. Professionals that can be referred to as knowledge worker includes engineers, pharmacists, architects, financial analysts, public accountants, physicians, scientists, design thinkers, and lawyers. Furthermore, knowledge workers have a high degree of expertise, experience, education and the primary purpose of their jobs involve the distribution, creation and application of knowledge.

Over the course of your undergraduate degree you have been taught about your scope of practice as
a student and the scope of practice of registered nurses under many different frameworks – legal,
ethical, practical and so on. It is now time to consider how the change in your roles will influence
your scope of practice. Historically the scope of practice of nurses was clearly defined. In the
current contemporary model of health practice the professional boundaries between the health
professions is becoming increasing blurred (Fedoruk & Hoffman, 2013 p24). The issue is further
compounded due to the scope of nursing practice expanding with roles such as advanced practice
nurse, as well as the increase in semi-independent practitioners in community settings.
In this time of transition you must be careful that you understand the scope of practice of the role
you aspire to, that of an experienced practicing Registered Nurse and the role you are about to
undertake that of new registered nurse. Benner describes the new graduate as an advanced
beginner one who is competent but relies on the rules they have been taught to guide them in their
practice. Advanced beginners are seen as nurses who have a wide theory base of practice but are
still developing the ability to make content dependant judgements (Benner, 1984 p 21-22). This
means that you should not set the bar too high for yourselves and realise that you are still learning
and developing the clinical judgement skills that can only come from exposure to clinical situations,
known as experiential learning. If we take Benner at her word then it takes at least 2-3 years of
clinical experience following registration as a registered nurse to be deemed competent and even
then she says the competent nurse will lack speed and efficiency (Benner, 1984 p 26-27).

SBAR communication tool is an effective and simple communication method that works across all the healthcare disciplines to create a safer working environment. Müller et al. (2018) reveal that using SBAR enhances efficacy, accuracy, and efficiency of handoff reports which boosts patient safety by improving communication and encouraging patient involvement Ineffective, absent, or inadequate communication leading to patient safety concerns has become a big problem in many care facilities, including where I work. According to Herawati et al. (2018), poor patient handoff communication from caregivers reduces patient safety. Regardless of the technological advancements, interdisciplinary care providers in many care facilities still do not communicate effectively, threatening patient's safety.
The use of written handoff using SBAR was implemented two years ago in the hospital I work. The handoff is easily accessible by all interdisciplinary team directly working with the patient and this is also used as a communication tool when giving report. According to Uhm et al. (2019), implementing a well-developed SBAR program of communication grounded on learning theory improves nurses' communication skills and boosts their confidence during patient handover. All the team member in the hospital identified the implementation of SBAR (Situation-Background-Assessment-Recommendation) as a solution for the persisting communication problem among caregivers, especially during patient handoffs within the hospital. On a daily basis, healthcare providers are faced with an array of clinical decisions to be made in an efficient and timely manner. Translating evidence into best practices is one way to achieve this. Without current best evidence, practice is rapidly outdated, often to the detriment of the patient. Evidence based practice is the conscientious use of current best practice in making decisions about patient care (Sackett, Richardson, Rosenberg, & Hayes, 2000). It is important for health care professionals to ask questions about their current clinical practice. In this week’s threaded discussion you will ask that burning question that you ask in your daily care of your patients.
You’ll need to focus on asking the right questions, narrowing the questions to one that is nurse driven and the need for change is evident. The question that you formulate will be the question for your Capstone Project.
The process of reviewing scholarly articles for a change in practice is an important part of the development of any type of research project that can lead to a change in practice. As you are appraising the systematic review and other scholarly articles for your change project, think about areas of the article such as sample size, the population, type of study, discussion and limitations. Critiquing a research article will allow you to evaluate the scientific merit of the study and decide how the results may be useful in practice.

In the intensive care unit, the nurse initial assessment requires turning down sedation to perform a more accurate neurological assessment. In my Department I was not taught the confusion assessment method during my orientation. My preceptor would assess neurological status by asking the patient to perform certain tasks such as squeeze their hands, wiggle their toes, or questions that they would nod yes or no to assess for whether the response was correct. There was no
- 2. consistency across the board as to nursing staff doing proper CAM-ICU assessments. I did later discover after being on the unit for about six months that CAM-ICU assessment was to be a part of our initial assessment. However, I did begin working the unit at the beginning of the pandemic, so maybe that’s why I didn’t learn proper technique initially. Concerns that arose with the use of CAM-ICU assessment was why there was no consistent use of the method among the nurses in the unit. It would only be used when the patient exhibited bizarre behavior. I also pondered if nurses knew how to properly use the method.There were many articles on CAM-ICU, varying from quality improvement, evidence based practices and nursing research. Use of the CAM-ICU during daily sedation stops in mechanically ventilated patients as assessed and experienced by intensive care nurses – related more to quality improvement because its aim related to the nurses experiences with the method.Else, B.
