This document summarizes a project by St Oswald's Hospice to improve transitions and support for young adults aged 16-25 with life-limiting conditions. The project aims to enable GPs to confidently support these young adults and improve their willingness to engage with primary care. A multi-disciplinary team provides outpatient assessments focusing on symptom management, goal-setting, future care planning, and signposting to other services. They work closely with GPs, other professionals, and families. The team hopes this dedicated service will empower young adults and families to focus on their goals rather than worrying about future crises.
