Dr. Jay Calvert published a study in Plastic and Reconstructive Surgery showing that revision rhinoplasty is possible for patients who previously underwent rhinoplasty with rib cartilage grafts. The study examined patients who had at least two prior rhinoplasty operations including one using rib cartilage grafts. While such patients were previously considered beyond surgical help, Dr. Calvert and his colleagues achieved favorable outcomes in many cases through careful planning and high-level techniques like regrafting with rib cartilage, ear cartilage or diced cartilage grafts. The revision surgery success rate was 76%, higher than the national average despite the inherent difficulty of these revision cases.

1) Doctors from Partners in Health arrived in Haiti on January 16th, 2010 to provide medical relief after the devastating earthquake. They were sent to St. Nicholas Hospital, 80 miles from Port-au-Prince, which was overwhelmed with over 200 untreated earthquake victims lying on the floor without necessary medical supplies or equipment.
2) Over the next two weeks, the doctors performed 216 earthquake-related surgeries and procedures with only basic resources, and established a triage system to prioritize the many severe injuries. They also trained local staff. Unfortunately 12 patients died from their injuries.

Canaloplasty The Keys to Optimizing Patients' Outcomes
The introduction of canaloplasty into the glaucoma surgical armamentarium was motivated by the desire of clinicians to enhance the quality of patients’ glaucoma care. Patients’ long-term adherence to topical glaucoma medical therapy is well known to be relatively poor. Laser therapy offers a safe alternative to medical therapy but often still requires the addition of topical medication. Traditionally, glaucoma filtration surgery has been reserved for more advanced, uncontrolled glaucoma for obvious reasons. Despite its definite role in glaucoma care, patients undergoing standard trabeculectomy are at significant risk for the development of postoperative infection, cataract, hypotony, bleb dysesthesia, astigmatism, and decreased visual acuity. These potential complications have driven surgeons to pursue surgical alternatives. Canaloplasty is a well-established procedure that has, for the past 3 years, demonstrated impressive efficacy and safety in peer-reviewed prospective studies. Despite growing evidence of its value and increasing performance of the procedure by ophthalmologists all over the world, misconceptions regarding its long-term efficacy as well as challenges in its adoption, surgical

symphysis remodeling after genioplasty.
Method: Fifty-four patients who underwent genioplasty at the end of their orthodontic treatment
were divided into three age groups: younger than 15 years at the time of surgery (group 1), 15 to
19 years (group 2), and 20 years or older (group 3). Twenty-three patients who did not accept
genioplasty and had a follow-up radiograph 2 years after the end of their orthodontic treatment
were used as a control group. Patients were evaluated at three time points: immediate preoperative
(T1), immediate postoperative (T2,) and 2 years postsurgery (T3).
Results: The mean genial advancement at surgery was similar for the three age groups, but the
extent of remodeling around the repositioned chin was greater in group 1, less in group 2, and still
less in group 3. Symphysis thickness increased significantly during the 2-year postsurgery interval
for the three groups, and this increase was significantly greater in group 1 than in group 3.
Remodeling above and behind the repositioned chin also was greater in the younger patients. This
was related to greater vertical growth of the dentoalveolar process in the younger patients. There
was no evidence of a deleterious effect on mandibular growth.
Conclusion: The outcomes of forward-upward genioplasty include increased symphysis
thickness, bone apposition above B point, and remodeling at the inferior border. When indications
for this type of genioplasty are recognized, early surgical correction (before age 15) produces a

Dr Jay Calvert Publishes On Rib Cartilage Graft Dr Jay Calvert, MD, Professional Corporation May 12, 2014 9:00 AM BEVERLY HILLS, Calif., May 12, 2014 /PRNewswire/ – Dr Jay Calvert's publication in the May issue of Plastic and Reconstructive Surgery entitled Reconstructive Rhinoplasty: Operative Revision of Patients with Previous Autologous Costal Cartilage Grafts1 shows how a sample of revision rhinoplasty patients who have had at least two previous rhinoplasty operations including one with rib cartilage has the possibility of nasal repair when skilled rhinoplasty surgeons use the most current techniques. Dr. Jay Calvert, along with his co-author, Dr. Rollin Daniel are known for performing some of the most difficult secondary rhinoplasty operations in the world of plastic surgery. They and fellow author Dr. Anita Patel, discuss the high level of difficulty and the high revision rate associated with trying to repair patients' noses when they have already undergone at least one operation with a rib cartilage graft. Previously, patients who had been treated with rib cartilage grafts for secondary rhinoplasty were considered to be at the end of the road for surgical treatment. However, following rib graft surgery, not all of these procedures turned out to be satisfactory to the patients and they sought out further surgery to try to improve both functionality and appearance of their noses. "When I was approached by a group of patients with problems after rib grafts, I had to set their expectations and my own expectations appropriately. There was no way to know what could be done in these difficult situations and we all had to work together to achieve the best results possible." - Dr. Jay Calvert These operations were planned carefully and performed at the highest level possible. The patients were followed closely to make sure that all that could be done was done to achieve a favorable outcome. Most patients were treated with some form of re-grafting, either with more rib cartilage, ear cartilage or with Diced Cartilage and Deep Temporalis Fascia, a technique first reported by Dr. Rollin Daniel and Dr. Jay Calvert in the same journal in 20042. The results were interesting in that the rate of revision surgery after the initial revision surgery by Dr. Jay Calvert was 24%. The national average for revision among all rhinoplasty patients (primary or secondary) is 15%. However, given the difficult nature of these patients' problems, that was not unpredictable. All of the patients had had at least two operations and some had had more than five operations. The good news was that after those 24% of patients had their second operations with Dr. Jay Calvert, their results were improved such that they did not require any further surgery.
