DENTAL
IMPLANTS
oA Dental implant is a prosthetic device or an
alloplastic material that is placed into the jaw
to support a crown or removable denture.”
INTRODUCTION :
• They are small sterile solid masses consisting of a
highly purified drug, made by compression or
molding or exclusion.
DENTAL IMPLANT TREATMENT PLAN
DEPENDS ON :-
• HOW MANY TEETH ARE MISSING?
• WHAT IS THE DEGREE OF BONE LOSS?
ARE THE REMAINING TEETH IN GOOD POSITION AND DO THEY HAVE A
LONG-TERM PROGNOSIS?
• WHAT DOES THE PATIENT EXPECT AS THE END RESULT?
• WHAT TREATMENT WILL SUPPORT THE BEST COSMETIC OUTCOME?
• WHAT IS THE PATIENT’S BUDGET?
WHY DENTAL
IMPLANTS?
• Dental implants help maintain
bone volume.
• Helps prevent the unnecessary
grinding of tooth.
• Gives natural emergence
profile.
• Better retention and
unmatched stability.
• Prosthesis is of the same size
as the natural predecessor.
ADVANTAGES
•They are more natural and comfortable
•Have a higher success rate
•Improve chewing function
•Lead to a lower risk of cavities developing in
nearby teeth
•Lead to better maintenance of bone at the site of
the lost tooth
•Cause decreased sensitivity in nearby teeth
•Do not need to be taken out and cleaned every
night
DISADVANTAGES :-
1. EXPENSIVE
2. PATIENT REQUIRES SURGERY
3. TIME CONSUMING PROCEDURE
4. TECHNICALLY COMPLEX
TYPES OF DENTAL IMPLANTS
Endosteal implants Subperiosteal implant
 Mucosal
MUCOSAL IMPLANT
-THEY ARE INSERTED INTO
THE MUCOSA.
-PAINFUL
-REQUIREMENT OF
CONTINUAL WEAR
• ENDOSTEAL IMPLANTS
• These types of dental implanting way includes implanting the tooth
directly into the jawbone with the help of a small surgery.
• The post or metallic cylinder is placed into the gum and then left for
some months for it to heal completely and the jaw bones to
surrounding the titanium cylinder. And when the tissue surrounding
the gum has healed, the second part of the Implantation surgery is
performed which includes bridging.
• This is done to connect the post to the implant which is original.
Then comes the third and the final part of the process which is
placing an artificial tooth or teeth and attaching it to the metallic
cylinder directly or sometimes grouped on a Bridge.
• Subperiosteal Implants
These types of dental implanting way include fixing of a metal frame into
the jawbone and just below the gum tissue.
This again might take up considerable time and when the healing process
of the gums completes, the metallic frame fixes to the jawbone properly.
Then the third step includes the observation of the fact that the metallic
posts, which are attached to the frame are protruding through the gums or
not. If they are protruding then it is the ideal time to mount the artificial
teeth to the posts. This is what the process includes.
TRANS-OSSEOUS IMPLANT
• Transosteal or Transosseous implants are also
known as staple bone implants and
transmandibular implant.
• Penetrates both cortical plate and passes
through the thickness of the alveolar bone
• Usually restricted to anterior area of mandible.
• It is not used much because they necessitate
an extraoral surgical approach.
HOW DO IMPLANTS WORK?
• A dental implant is composed of three
different parts. The first part is the
implant itself, which looks like a tiny
screw and is surgically attached to the
jawbone. The implant is what is now going
to act as a natural tooth root. Once the
implant has healed,
The
Implant
• The second part is attached
to the implant, which is
called the abutment. Once
the abutment has healed, the
final and last part of the
implant is the prosthesis.
• An abutment is a metal connector
that your dental professional will
into your dental implant after you've
healed from your surgery. The abutment
screws into the implant and securely
holds your crown (your fake tooth) in
place.
• The prosthesis is the false tooth, also
known as a dental crown, which is then
attached to the abutment. The end result
is a new false tooth that looks and acts
just like a natural tooth.
TYPES OF DENTAL IMPLANTS DEPENDING UPON
THE MATERIAL USED
1. METALLIC IMPLANTS
• TITANIUM
• TITANIUM ALIMINIUM VANDIUM
• COBALT CHROMIUMM
MOLYBDENUM
• STAINLESS STEEL
• ZIRCONIUM
• TANTALUM
• GOLD
• PLATINUM
DEPENDING ON THEIR ACTION WITH BONE
1. BIO-ACTIVE
• HYDROXYAPETITE
• TRI CALCIUM PHOSPHATE
• CALCIUM PHOSPHATE
2. BIO INERT
GETTING APPROVAL
Since getting dental implants placed
in one’s mouth requires a surgical
procedure, it is necessary for them
to first be approved to undergo the
surgical process. There is a list of
requirements that every dental
patient must meet before being
approved to undergo the process.
This includes having a sufficient
amount of jawbone so that the
implant can be surgically inserted.
If they do not, they will need to be
approved for a bone graft.
Bone requires stimulation to retain its form and functions.
Loss of teeth leads to loss of width up to
4mm of height.
Hence, it becomes important that the dental practitioner considers
not only the loss of tooth but also the loss of bone.
• They must also be in overall good health. This
includes oral health as well as body health.
• Patients must also be fully committed to taking
proper care of their new dental implants as
well as the rest of the teeth, which means not
smoking.
• Dental patients who may not receive the
approval they are looking for include those who
have been diagnosed with diabetes, cancer, are
taking certain medications that suppress the
immune system and/or drink too much alcohol.
POTENTIAL COMPLICATIONS OF IMPLANT SURGERY
People who undergo this procedure may experience complications during or
afterward. The issues may include:
•nerve damage, resulting in altered sensation in the surgical area
•an opening of the incision following surgery
•movement of the implant
•exposure of the implant above the gumline
•infection of the implant
People who experience movement or exposure of the implant may need to
undergo additional procedures to improve the health of the bone and gums or
remove or replace the implant.
The following are some signs and symptoms that an implant
placement has been unsuccessful:
•the implant is excessively mobile
•pus or other secretions come from the site
•pain when tapping the implant
•rapid, progressive bone loss
IMPLANT SIZE
GREATER THE AREA OF THE IMPLANT-BASE SYSTEM, LESSER WILL
BE THE CONCENTRATED FORCE TRANS,ITTED TO THE CREST OF
BONE AT THE IMPLANT INTERFACE. SIMILARLY, THE GREATER WILL
BE THE SURFACE AREA OF THE IMPLANT-BASE SYSTEM, BETTER
THE PROGNOSIS OF IMPLANT.
• FOR EACH 0.25 MM INSCREASE IN DIAMETER, THE
SURFACE AREA OF A CYLINDER INCREASES BY MORE
THAN 10%.
• FOR EACH 3.00 MM INCREASE IN LENGTH, THE SURFACE
AREA OF A CYLINDER INCFREASES BY MORE THAN 10%.
IMPLANT LOCATION
Ideally, occlusal forces should be directed along the long axis of the
implants. Therefore, the angle of the osseous ridge crest is the key
determinant of implant angulation.
The distance between an implant and any adjacent “landmark”
(natural tooth or other implant), which should not be less than 2.00
mm
IMPLANT ANGULATION WITH RESPECT
TO THE OSSEOUS RIDGE CREST
PROCEDURES
Each person is likely to have a different experience of
dental implant surgery. Factors that may influence this
include:
•the number of teeth requiring replacement
•the location of the implants within the jaw
•the quality and quantity of bone at the implant site
•the person’s underlying oral and systemic health
Depending on these factors, additional procedures may
be necessary.
SINUS AUGMENTATION
PLACING AN IMPLANT IN THE UPPER JAWBONE IS USUALLY DIFFICULT
BECAUSE OF THE LOCATION OF THE SINUSES.
THE SURGEON MAY NEED TO PERFORM A SINUS AUGMENTATION — A
PROCEDURE TO LIFT THE FLOOR OF THE SINUSES TO ALLOW MORE BONE
TO DEVELOP SO THAT THE IMPLANTATION CAN BE SUCCESSFUL.
RIDGE MODIFICATION
SOME PEOPLE HAVE A JAWBONE ABNORMALITY THAT PREVENTS ENOUGH
BONE FOR AN IMPLANT FROM DEVELOPING. IN SUCH CASES, A SURGEON
MAY NEED TO PERFORM A RIDGE MODIFICATION.
THIS INVOLVES LIFTING THE GUM TO EXPOSE THE AREA OF DEFORMED
BONE. THE SURGEON WILL THEN USE A BONE OR BONE SUBSTITUTE TO
REPAIR AND BUILD UP THE AREA. THIS IMPROVES THE QUALITY OF THE JAWBONE
IN PREPARATION FOR DENTAL IMPLANT SURGERY.

The abutment screws into the implant and securely holds your crown (your fake tooth) in place.
  • 18. • The prosthesis is the false tooth, also known as a dental crown, which is then attached to the abutment. The end result is a new false tooth that looks and acts just like a natural tooth.

  • 27. GETTING APPROVAL Since getting dental implants placed in one’s mouth requires a surgical procedure, it is necessary for them to first be approved to undergo the surgical process. There is a list of requirements that every dental patient must meet before being approved to undergo the process. This includes having a sufficient amount of jawbone so that the implant can be surgically inserted. If they do not, they will need to be approved for a bone graft.

  • 29. Bone requires stimulation to retain its form and functions. Loss of teeth leads to loss of width up to 4mm of height. Hence, it becomes important that the dental practitioner considers not only the loss of tooth but also the loss of bone.
  • 30. • They must also be in overall good health. This includes oral health as well as body health. • Patients must also be fully committed to taking proper care of their new dental implants as well as the rest of the teeth, which means not smoking. • Dental patients who may not receive the approval they are looking for include those who have been diagnosed with diabetes, cancer, are taking certain medications that suppress the immune system and/or drink too much alcohol.
  • 31. POTENTIAL COMPLICATIONS OF IMPLANT SURGERY People who undergo this procedure may experience complications during or afterward. The issues may include: •nerve damage, resulting in altered sensation in the surgical area •an opening of the incision following surgery •movement of the implant •exposure of the implant above the gumline •infection of the implant People who experience movement or exposure of the implant may need to undergo additional procedures to improve the health of the bone and gums or remove or replace the implant.
  • 32. The following are some signs and symptoms that an implant placement has been unsuccessful: •the implant is excessively mobile •pus or other secretions come from the site •pain when tapping the implant •rapid, progressive bone loss
  • 33. IMPLANT SIZE GREATER THE AREA OF THE IMPLANT-BASE SYSTEM, LESSER WILL BE THE CONCENTRATED FORCE TRANS,ITTED TO THE CREST OF BONE AT THE IMPLANT INTERFACE. SIMILARLY, THE GREATER WILL BE THE SURFACE AREA OF THE IMPLANT-BASE SYSTEM, BETTER THE PROGNOSIS OF IMPLANT. • FOR EACH 0.25 MM INSCREASE IN DIAMETER, THE SURFACE AREA OF A CYLINDER INCREASES BY MORE THAN 10%. • FOR EACH 3.00 MM INCREASE IN LENGTH, THE SURFACE AREA OF A CYLINDER INCFREASES BY MORE THAN 10%.
  • 34. IMPLANT LOCATION Ideally, occlusal forces should be directed along the long axis of the implants. Therefore, the angle of the osseous ridge crest is the key determinant of implant angulation. The distance between an implant and any adjacent “landmark” (natural tooth or other implant), which should not be less than 2.00 mm

  • 40. PROCEDURES Each person is likely to have a different experience of dental implant surgery. Factors that may influence this include: •the number of teeth requiring replacement •the location of the implants within the jaw •the quality and quantity of bone at the implant site •the person’s underlying oral and systemic health Depending on these factors, additional procedures may be necessary.
  • 41. SINUS AUGMENTATION PLACING AN IMPLANT IN THE UPPER JAWBONE IS USUALLY DIFFICULT BECAUSE OF THE LOCATION OF THE SINUSES. THE SURGEON MAY NEED TO PERFORM A SINUS AUGMENTATION — A PROCEDURE TO LIFT THE FLOOR OF THE SINUSES TO ALLOW MORE BONE TO DEVELOP SO THAT THE IMPLANTATION CAN BE SUCCESSFUL. RIDGE MODIFICATION SOME PEOPLE HAVE A JAWBONE ABNORMALITY THAT PREVENTS ENOUGH BONE FOR AN IMPLANT FROM DEVELOPING. IN SUCH CASES, A SURGEON MAY NEED TO PERFORM A RIDGE MODIFICATION. THIS INVOLVES LIFTING THE GUM TO EXPOSE THE AREA OF DEFORMED BONE. THE SURGEON WILL THEN USE A BONE OR BONE SUBSTITUTE TO REPAIR AND BUILD UP THE AREA. THIS IMPROVES THE QUALITY OF THE JAWBONE IN PREPARATION FOR DENTAL IMPLANT SURGERY.
