This document outlines a data collection plan to evaluate a program across 5 levels: 1) reactions/satisfaction and planned actions, 2) learning, 3) application/implementation, 4) business impact, and 5) ROI. For each level, it identifies the broad program objectives, measures, data collection methods/instruments, data sources, timing, and responsibilities. It also includes spaces to record baseline data, comments, and an ROI analysis plan identifying data items to measure, methods to isolate program effects, how to convert data to monetary values, cost categories, intangible benefits, communication targets, and other issues during application.
