This study aimed to determine the best pre-operative risk stratification tool for predicting major adverse cardiac events after vascular surgery. The study compared several risk predictors: the revised cardiac risk index (RCRI), pre-operative biomarkers (BNP, troponin, CRP), and ambulatory Holter monitoring. On multivariate analysis, only pre-operative troponin elevation and BNP above the optimal cutoff were independently associated with cardiac events. Both BNP and troponin risk stratification significantly improved risk classification. However, troponin decreased correct classification of patients with complications. Pre-operative BNP evaluation was found to be the single best clinically useful predictor of postoperative cardiac outcomes.

This study investigated the safety, efficacy and cost effective analysis of a pharmaco-invasive strategy compared with primary (PCI) strategy for ST-Segment Elevation Myocardial Infarction (STEMI) in Gaza.
Methods: We ran domized 145 patients presenting within 2 hours of symptom onset of acute ST elevation myocardial infarction to primary PCI or for pharmaco-invasive PCI 2-24 hours after streptokinase, except in the event of failed reperfusion, in which case, emergency angiography was recommended. The primary endpoint a composite of death, shock and congestive heart failure at 30 days. Secondary end points: total bleeding and failed streptokinase required emergent PCI. Tertiary end points: cost effective analysis.

Methods: This study was done in Department of surgery at Patna medical college, Patna, Bihar, India from April
2014 to October 2015 on 100 patients. Physiological variables were collected prior to induction of anesthesia and
operative variable collected during operation chi-square test was used for expected and actual mortality differences.
Results: In present study 100 patients of peritonitis due to different cause of intestinal perforation were studied.
Comparison of observed and POSSUM predicted mortality and morbidity rates were done. Observed to expect
mortality and morbidity ratio was 1.005 and 1.001 respectively and there was no statistically significant difference
between the predicted and observed values.
Conclusions: This study confirms and validates the findings of previous work that POSSUM is an accurate and
reliable tool for estimating in-hospital mortality.

To evaluate the diagnostic value of Copeptin as a novel biomarker in early diagnosis of Acute Myocardial Infarction. 56 patients with acute Myocardial Infarction (STEMI) and 25 healthy controls who were admitted to the Cardiology and Clinical Pathology Departments, national heart institute (NHI) from October 2015 to April 2016. The kit used a double-antibody sandwich enzyme-linked immune-sorbent assay (ELISA) to assay the level of Human Copeptin in samples. As regard copeptin, the median range of copeptin level was 242.5pg/ml in patient group and 75pg/ml in control group. The comparative study between the two groups shows a significant difference (p < 0.05) Conclusion: Copeptin is a reliable diagnostic tool in patients with AMI (STEMI) with sensitivity 85.7%, specificity 86.7%, PPV 96% and NPV 61.9%.

Abstract
Background: The surgical safety checklist is widely used to improve the quality of perioperative care. However, clinicians
continue to debate the clinical effectiveness of this tool.
Methods: Prospective analysis of data from the International Surgical Outcomes Study (ISOS), an international obser-
vational study of elective in-patient surgery, accompanied by a systematic review and meta-analysis of published
literature. The exposure was surgical safety checklist use. The primary outcome was in-hospital mortality and the
secondary outcome was postoperative complications. In the ISOS cohort, a multivariable multi-level generalized linear
model was used to test associations. To further contextualise these findings, we included the results from the ISOS
cohort in a meta-analysis. Results are reported as odds ratios (OR) with 95% confidence intervals.
Results: We included 44 814 patients from 497 hospitals in 27 countries in the ISOS analysis. There were 40 245 (89.8%)
patients exposed to the checklist, whilst 7508 (16.8%) sustained �1 postoperative complications and 207 (0.5%) died
before hospital discharge. Checklist exposure was associated with reduced mortality [odds ratio (OR) 0.49 (0.32e0.77);
P<0.01], but no difference in complication rates [OR 1.02 (0.88e1.19); P¼0.75]. In a systematic review, we screened 3732
records and identified 11 eligible studies of 453 292 patients including the ISOS cohort. Checklist exposure was associated
with both reduced postoperative mortality [OR 0.75 (0.62e0.92); P<0.01; I2¼87%] and reduced complication rates [OR 0.73
(0.61e0.88); P<0.01; I2¼89%).

Summary
Although brain natriuretic peptide has been shown to be superior to the revised cardiac risk index for risk stratification
of vascular surgical patients, it remains unknown whether it is superior to alternative dynamic risk predictors, such as
other pre-operative biomarkers (C-reactive protein and troponins) or myocardial ischaemia monitoring. The aim of this
prospective observational study was to determine the relative clinical utility of these risk predictors for the prediction of
postoperative cardiac events in elective vascular surgical patients. Only pre-operative troponin elevation (OR 56.8, 95%
CI 6.5–496.0, p < 0.001) and brain natriuretic peptide above the optimal discriminatory point (OR 6.0, 95% CI 2.7–12.9,
p < 0.001) were independently associated with cardiac events. Both brain natriuretic peptide and troponin risk
stratification significantly improved overall net reclassification (74.6% (95% CI 51.6%–97.5%) and 38.5% (95% CI 22.4–
54.6%, respectively)); however, troponin stratification decreased the correct classification of patients with cardiac
