The UC San Diego Clinical Geriatrics Interprofessional Symposium will be held on October 4-5, 2014 at the UCSD Skaggs School of Pharmacy. The two-day symposium will provide health professionals who care for older adults with strategies for addressing issues like delirium prevention, wound care, geriatric pharmacology, end-of-life care, and care transitions. Speakers will include faculty from UC San Diego's geriatrics program and VA San Diego Healthcare System. The symposium aims to employ principles of geriatric medicine and improve care for older patients.

PROGRAM OVERVIEW
The UC San Diego Geriatrics Division and Fellowship Program are proud to host their first inaugural Clinical Geriatrics Interprofessional
Symposium. This symposium will focus on core issues oŌen perƟnent in providing quality, paƟent‐centered clinical care for the elderly.
This two‐day course will provide parƟcipants with the latest strategies for addressing topics, such as delirium prevenƟon, wound care,
geriatric pharmacology concerns, end‐of‐life decisions and palliaƟve care, along with ensuring good transiƟons of care. Building on the
oŌen interdisciplinary nature of geriatrics care, lectures will include panel presentaƟons by mulƟple levels of care providers with ample
opportunity for parƟcipant engagement and discussion. Speakers and facilitators will largely include faculty and staff from UC San Diego’s
naƟonally recognized geriatrics program and the VA San Diego Healthcare System.
TARGET AUDIENCE
The target audience for this acƟvity includes health care professionals who care for older adults, including physicians, registered nurses,
nurse pracƟƟoners, pharmacists, physical and occupaƟonal therapists, and social workers. The content covered during this symposium
will be applicable within mulƟple seƫngs, parƟcularly within acute, post‐acute, long‐term care, palliaƟve and home care environments.
PROGRAM OBJECTIVES
At the conclusion of this conference, parƟcipants should be able to:
 Employ the principles of geriatric medicine;
 Discuss current management techniques and treatments available for different geriatric condiƟons;
 IdenƟfy paƟent‐centered and system‐wide strategies designed to reduce iatrogenic events;
 Describe the various levels of care and when a transiƟon is most appropriate; and
 Recognize psychosocial issues that can impact care and decision‐making.
