Medication administration errors(MAE) can threaten patient outcomes and are a dimension of
patient safety directly linked to nursing care. Children are particularly vulnerable to medication errors because
of their unique physiology and developmental needs.
Aims: The present study aims to examine types, stages and causes of medication errors. Barriers of medication
administration errors reporting and its facilitator at pediatric University hospitals from nurses point of view.
Methods: A descriptive study was conducted in Pediatric intensive care units, medical, surgical and urology
ward of children's university hospital at Mansoura University, intensive care units, kidney dialysis at
Abouelrash pediatric hospital and general wards of Elmonaira at Cairo University Hospitals. 80 nurses were
included in the study after fulfilling the criteria of selection. A structured interview questionnaire that consists
of four sections was used.
Results: The highest types of medication errors as reported by studied nurses occurred when the medication is
delivered by the wrong route, the highest stage of medication errors done by nurses was missing of medication
then patient monitoring and administration and the highest cause of medication errors was due to heavy
workload. The results of this study indicated that the strongest perceived barriers to medication administration
errors reporting were fear from consequences of reporting, then managerial factor and then the process of
reporting from the nurse's viewpoint. The nurses agree that identifying benefits of reporting followed agree that
feeling safe about working environment, and agree that good professional relationship with physicians was the
most facilitating factors of reporting medication errors.
Conclusions: It was concluded that medication errors result from interrelated factors, the strongest perceived
barriers to medication administration errors reporting were fear from consequences of reporting, and good
relationship with nurse managers and physicians were the most facilitators of reporting medication errors.
Recommendation: The study recommended that the assessment of medication errors should be done
periodically and in- service training program about medication administrations should be applied

Despite the morbidity and cases of widespread drug poisoning, clinical guidelines are largely written by taking into account only one treatment at a time. The cumulative impact of multiple treatments is rarely considered. Drug treatment for people with several diseases produces a complex regimen called “polypharmacy” with a potential combination of harmful and even lethal drugs that can be prevented. This polypharmacy causes in many cases the death of some people due to drug interactions. The vast majority of these deaths can be prevented by detecting interactions before taking these medications. But the problem is that such information exists in a state that is difficult to access for the general public, much less for people with little knowledge in the field. Although the pharmacist is unmistakable and most viable source to avoid such a problem, he cannot know what the patient does not mention because he is not aware of what may affect his treatment. To remedy this, we aim in this paper to develop an ergonomic Android application that will inform the patient about the potential risks of such drug interactions. The application is optimized to handle various databases and operate automation of QR code.

Because risk is influenced by the extent of HD exposure as well as the potency and toxicity of HDs, USP standards have been developed to minimize exposure and promote HCW safety. Although there is an increased awareness of the risks of HD exposure, these safety standards are not universally implemented and compliance is often inadequate. Occupational health/safety professionals are responsible for executing a comprehensive HD program that includes hazard communication, administrative, and engineering controls.

Identifying a Researchable Problem
Nurses are at the forefront of evidence based practice questions. Everyday healthcare treatments are governed by policies and procedures created as a result of evidence based practice questions. Over the years, many nurses begin to perform their duties not giving much thought to the rationales behind what they are doing. However, under the circumstances, improvements to healthcare are based off the experiences and curiosity of nurses delivering direct patient care. In this assignment, a researchable nursing topic will be identified and an answerable evidence based practice question will be generated to resolve the issue presented.
Area of Interest
Post-operative surgical site infections are ranked among the highest of all healthcare associated infections. In 2011, the Centers for Disease Control and Prevention (CDC) reported an estimated 721,800 infections in acute care hospitals across the United States (CDC, 2011). An estimated 157,500 of the infections were reported as surgical site infections (CDC, 2011).
Due to an increase in post-operative surgical site infections, hospitals, and outpatient surgical clinics have adopted and implemented changes to their skin preparation techniques by using a Chlorhexidine-Gluconate Ethanol or Hibiclens solution versus Povidone-Iodine or Betadine as a pre-operative scrub. The purpose of the pre-operative scrubs is to prevent bacterial contamination of the surgical site (Dizer, Hatipogula, Kaymakcioglu, Tufan, Yava, Iyigum, & Senses, 2009). Both Hibiclens and Betadine have been used for decades. Although Chlorhexidine is highly recommended as the skin preparation solution due to the high risk for an allergic reaction to Povidone-iodine, many healthcare facilities around the world continue its use. As a result of these changes, many healthcare professionals have researched and questioned the advantage of Hibiclens over Betadine.
Feasibility
Below are five questions formulated to determine their feasibility related to the use of Chlorhexidine verses Povidone-iodine in preventing postoperative surgical site infections.
1. How effective is the use of Chlorhexidine verses Povidone-iodine as a preoperative skin solution?
2. Will Chlorhexidine as a preoperative skin preparation solution reduce surgical site infections?
3. Will the use of Chlorhexidine result in overall savings for the hospital?

Medical utopias are often about good health, absence of suffering, and even delaying of the aging process. The last two decades have seen a tremendous increase in emerging medical technologies to achieve these utopias. The completion of the sequencing of the human genome sets the stage for the next step of genetic and molecular advances. The increase in computing power, storage capacity, connectivity, and the Internet has opened avenues of new diagnostic and therapeutic modalities. The perfecting of sustaining cell growth in vitro and cell nucleus transfer has opened the way to cloning, stem cell harvesting, and a new field of regenerative medicine. However, these emerging technologies bring with them a large number of bioethical concerns that need to be addressed. These concerns involving tissue engineering, bioelectronics, new genetics, cloning, gene therapy, germ-line genome modifications are only the tip of the iceberg. In this paper I will reflect on three areas of concern. Firstly, the emergence of the digital patient will be considered. This digital patient will be deeply formed and informed by health information technology (IT), the social media, and issues involving privacy, confidentiality and data security. Secondly, the direct to customers (DTC) genetic screening tests will be discussed. The ethical issue of buccal swabs taken at home and be tested for genetic diseases and future prediction of other illnesses which is marketed directly to the consumers will be examined. Finally, the development of new pharmaco-therapeutics will be explored. There have been changes in the way new drugs are tested and these changes do raise some ethical concerns. The examination of these ethical issues will be done in the framework of respect for autonomy, beneficence, non-maleficence, and justice.

Scope of problem
Chemotherapeutic agents pose a potential health risk to personnel who prepare, handle, administer,
and dispose of these drugs; chemotherapeutic agents pose any one of the following characteristics:
genotoxicity, carcinogenicity, teratogenicity, or fertility impairment (Itano, Toka, 2005).
Chemotherapy, because of its relatively narrow therapeutic index, is often associated with a greater
risk of adverse events (AEs) than other medications, and when used in combination, may result in an
even greater incidence of AEs (Goodin et al., 2011). Potential effects of exposure to hazardous drugs:
1- short term- occur within hours or days after exposure: contact dermatitis, alopecia, local skin or
mucus membrane irritation, blurred vision, allergic response, dizziness, gastrointestinal (GI) tract
problems, headache. 2- Long term- occur within months or years after exposure: liver damage,
chromosomal abnormalities, increased risk of cancer, reproductive risks (Itano et al., 2005). Green et
al., (2009) reported that the risk for exposurerelated cancers increased in health care workers who
handle chemotherapy and in female health care workers who become pregnant; there are also the
potential hazards of spontaneous abortions, stillbirths, and teratogenicity effects on unborn fetuses.
Patients receiving chemotherapy and their family members can also be exposed to the hazards of
chemotherapy drugs when they handle contaminated equipment or body fluids (CNSA, 2003). Several
studies carried out at hospital units have shown detectable levels of cytotoxic agents in the air, on
surfaces, on gloves, and on different parts of the body. The presence of these drugs in the urine of
hospital personnel has been widely studied. This has led several organizations to develop guidelines or
recommendations with the aim to improve safety during the handling of antineoplastic drugs and

hospitals these drugs are being administered by nurses without proper training or being chemotherapy
certified which may increase exposure to chemotherapeutic agents for healthcare providers (Polovich,
2004). Many institutions introduced and implemented policies and procedures designed to minimize
occupational exposure and consequent risks associated with handling cytotoxic drugs including
economic impact. Health care institutions would have to value the significant expense to comply with
this policy and the real cost must be weighed against the potential high cost of the treatment of the
health care workers.
