Pharmacovigilance has been developed by WHO solely with the intention of responding to the special
needs. Over the last decade, it has been increasingly recognized that the scope of pharmacovigilance
needs to be extended more , much beyond the present signals of safety concerns. Globalization,
consumerism, the resulting expansion in free trade and communication across national and
international borders, and increasing use of the Internet have all contributed to a change so that people
can access information on all types of medicinal products. So pharmacovigilance is indeed important
and the changing patterns in drug use require a shift in the approach to pharmacovigilance so that it
can respond to the prevailing patterns of drug use within society in a better way.
In today's advanced age of medicines, science and technology, there has been an increasing public
awareness of safety issues related to medical products. This has led to an increased importance of
pharmacovigilance.
Pharmacovigilance in India & global perspective -
Pharmacovigilance (PV) plays a key role in the healthcare system through assessment,
monitoring and discovery of interactions amongst drugs and their effects in human.
Pharmaceutical and biotechnological medicines are designed to cure, prevent or treat
diseases; however, there are also risks particularly adverse drug reactions (ADRs) can
cause serious harm to patients. Thus, for safety medication ADRs monitoring required for
each medicine throughout its life cycle, during development of drug such as pre-
marketing including early stages of drug design, clinical trials, and post-marketing
surveillance. PV is concerns with the detection, assessment, understanding and
prevention of ADRs. Pharmacogenetics and pharmacogenomics are an indispensable part
of the clinical research. Variation in the human genome is a cause of variable response to
drugs and susceptibility to diseases are determined, which is important for early drug
discovery to PV. Moreover, PV has traditionally involved in mining spontaneous reports
submitted to national surveillance systems. The research focus is shifting toward the use
of data generated from platforms outside the conventional framework such as electronic
medical records, biomedical literature, and patient-reported data in health forums. The
emerging trend in PV is to link premarketing data with human safety information
observed in the post-marketing phase. The PV system team obtains valuable additional
information, building up the scientific data contained in the original report and making it
more informative. This necessitates an utmost requirement for effective regulations of the
drug approval process and conscious pre and post approval vigilance of the undesired
effects, especially in India. Adverse events reported by PV system potentially benefit to
the community due to their proximity to both population and public health practitioners,
in terms of language and knowledge, enables easy contact with reporters by
electronically. Hence, PV helps to the patients get well and to manage optimally or
ideally, avoid illness is a collective responsibility of industry, drug regulators, clinicians
and other healthcare professionals to enhance their contribution to public health. This
review summarized objectives and methodologies used in PV with critical overview of
existing PV in India, challenges to overcome and future prospects with respect to Indian
context.
WHO international drug monitoring programme-
In 1968, during the 16th World Assembly the 16.36 resolution called for “a systematic collection of
information on serious adverse drug reactions during the development and particularly after medicines
have been made available for public use”. This led to the formation of the WHO Programme for
International Drug Monitoring (PIDM).
Basic terminologies used in Pharmacovigilance-
Absolute risk
Risk in a population of exposed persons; the probability of an event affecting members of a particular population
(e.g. 1 in 1,000). Absolute risk can be measured over time (incidence) or at a given time (prevalence).

Adverse event
Any untoward medical occurrence that may present during treatment with a pharmaceutical product but which
does not necessarily have a causal relationship with this treatment.
Synonym: adverse experience
Adverse (drug) reaction (ADR)
A response which is noxious and unintended, and which occurs at doses normally used in humans for the
prophylaxis, diagnosis, or therapy of disease, or for the modification of physiological function. (WHO, 1972).
An adverse drug reaction, contrary to an adverse event, is characterized by the suspicion of a causal relationship
between the drug and the occurrence, i.e. judged as being at least possibly related to treatment by the reporting
or a reviewing health professional.
Allopathy
Non-traditional, western scientific therapy, usually using synthesised ingredients, but may also contain a purified
active ingredient extracted from a plant or other natural source; usually in opposition to the disease.
Compare homeopathy.
Association
Events associated in time but not necessarily linked as cause and effect.
Attributable risk
Difference between the risk in an exposed population (absolute risk) and the risk in an unexposed population
(reference risk). Also referred to as excess risk.
Attributable risk is the result of an absolute comparison between outcome frequency measurements, such as
incidence.
Examples: If the exposed persons with a particular outcome are A, the exposed persons without the outcome are
B, the unexposed persons with the outcome are C and the unexposed persons with the outcome are D, then the
attributable risk is calculated as : [A / (A+B)] - [C / (C+D)]. If, during the same time period, the incidence of rash
in a population treated with medicine X is 35/1,500=0.023, and the incidence of rash in a population not treated
with X is 5/2,000=0.0025, the attributable risk is (35/1,500) - (5/2,000) = 0.0205.
Benefit
An estimated gain for an individual or a population. See also Effectiveness/Risk.
Benefit - risk analysis
Examination of the favourable (beneficial) and unfavourable results of undertaking a specific course of action.
(While this phrase is still commonly used, the more logical pairings of benefit-harm and effectiveness-risk are
slowly replacing it).
Biological products
Medical products prepared from biological material of human, animal or microbiologic origin (such as blood
products, vaccines, insulin).
Causality assessment
The evaluation of the likelihood that a medicine was the causative agent of an observed adverse reaction.
Causality assessment is usually made according established algorithms.
Caveat document
The formal advisory warning accompanying data release from the WHO Database: it specifies the conditions and
reservations applying to interpretations and use of the data.
CIOMS
The Council for International Organizations of Medical Sciences. CIOMS is a body set up under World Health
Organization and UNESCO. It has developed a series of guidelines on pharmacovigilance, drawn up by a
committee of volunteers from Industry, regulatory authorities, WHO and others.
The main guidelines concern the international reporting form (CIOMS I); periodic safety update reports (CIOMS
II); core data sheets (CIOMS III); benefit-risk assessments (CIOMS IV); practical issues in pharmacovigilance
(CIOMS V); clinical trial safety data (CIOMS VI); and development safety update reports (CIOMS VII).
Clinical trial
A systematic study on pharmaceutical products in human subjects (including patients and other volunteers) in
order to discover or verify the effects of and/or identify any adverse reaction to investigational products, and/or
to study the absorption, distribution, metabolism and excretion of the products with the objective of ascertaining
their efficacy and safety.
Combinations database
Quarterly report of all drug-ADR associations produced by the BCPNN scan.
Common Epidemiology
In pharmacovigilance, an event with a frequency between 1 in 100 and 1 in 10
Compliance
Faithful adherence by the patient to the prescriber's instructions.
Control group
The comparison group in drug-trials not being given the studied drug.
Critical terms
Some of the terms in WHO-ART are marked as "Critical Terms". These terms either refer to or might be indicative
of serious disease states, and warrant special attention, because of their possible association with the risk of
serious illness which may lead to more decisive action than reports on other terms.

Data-mining
At the UMC, the use of an automated tool, based on Bayesian logic, for the scanning of the WHO database
(Vigibase) in the process of detecting drug-adverse reaction associations: the BCPNN. Knowledge-detection is the
preferred term for the process.
Dechallenge
The withdrawal of a drug from a patient; the point at which the continuity, reduction or disappearance of adverse
effects may be observed.
Effectiveness/risk
The balance between the rate of effectiveness of a medicine versus the risk of harm is a quantitative assessment
of the merit of a medicine used in routine clinical practice. Comparative information between therapies is most
useful. This is more useful than the efficacy and hazard predictions from pre-marketing information that is limited
and based on selected subjects.
Efficacy
The ability of a drug to produce the intended effect as determined by scientific methods.
Epidemiology
The science concerned with the study of the factors determining and influencing the frequency and distribution of
disease, injury and other health-related events and their causes in a defined human population for the purpose of
establishing programs to prevent and control their development and spread (Dorland's Illustrated Medical
Dictionary).

Essential medicines
Essential medicines are those that satisfy the priority health care needs of the population. They are selected with
due regard to public health relevance, evidence on efficacy and safety, and comparative cost-effectiveness.

EVMPD
The EudraVigilance Medicinal Product Dictionary (EVMPD) has been developed by the European Medicines Agency
in collaboration with the EudraVigilance Joint Implementation Group. The main objective of the EVMPD is to
assist the pharmacovigilance activities in the European Economic Area.
EUDRAGENE
Eudragene is a European collaboration that established a collection of DNA samples as a resource for studying
genes which influence serious or adverse drug reactions (ADRs). Identifying genes that influence susceptibility to
adverse reactions will advance understanding of the basis of adverse drug reactions and may also lead to the
development of tests that can predict individual susceptibility to adverse reactions.
Excipients
All materials included to make a pharmaceutical formulation (e.g. a tablet) except the active drug substance(s).
Formulary
A listing of medicinal drugs with their uses, methods of administration, available dose forms, side effects, etc,
sometimes including their formulas and methods of preparation.
Generic (multisource product)
The term "generic product" has somewhat different meanings in different jurisdictions. Generic products may be
marketed either under the non-proprietary approved name or under a new brand (proprietary) name. They are
usually intended to be interchangeable with the innovator product, which is usually manufactured without a
license from the innovator company and marketed after the expiry of patent or other exclusivity rights.
Harm
The nature and extent of actual damage that could be caused by a drug. Not to be confused with risk.
Herbal medicine
Includes herbs, herbal materials, herbal preparations and finished herbal products.
Homeopathy
The treatment of disease by means with similar effects to the disease. Compare Allopathy.
HTA

ICH
The International Council for Harmonisation of Technical Requirements for Pharmaceuticals for Human Use.
Incidence
The extent or rate of occurrence, especially the number of new cases of a disease in a population over a period
of time.
Knowledge-detection
Preferred term as the alternative to data mining; searching for combinations and patterns using BCPNN.

One of the prime reference sources for information about drugs throughout the world. Published by the
Pharmaceutical Press, UK.
Medical error
"An unintended act (either of omission or commission) or one that does not achieve its intended outcomes."

MedDRA
MedDRA or Medical Dictionary for Regulatory Activities is a clinically-validated international medical terminology
used by regulatory authorities and the regulated biopharmaceutical industry. The terminology is used through the
entire regulatory process, from pre-marketing to post-marketing, and for data entry, retrieval, evaluation, and
presentation.
Member countries
Countries which comply with the criteria for, and have joined the WHO Programme for International Drug
Monitoring.
National pharmacovigilance centres
Organisations recognised by governments to represent their country in the WHO Programme (usually the drug
regulatory agency). A single, governmentally recognized centre (or integrated system) within a country with the
clinical and scientific expertise to collect, collate, analyse and give advice on all information related to drug
safety.
Neural network
A type of artificial intelligence used in the BCPNN to scan the WHO ADR database (Vigibase).
Over the counter (OTC)
Medicines which are available for purchase without prescription.
Pharmacoepidemiology
Study of the use and effects of drugs in large populations. See also epidemiology.
Pharmacology
Study of the uses, effects and modes of action of drugs.
Pharmacovigilance
The science and activities relating to the detection, assessment, understanding and prevention of adverse effects
or any other drug-related problem.
Phocomelia
Characteristic deformity caused by exposure to thalidomide in the womb, also very rarely occurring
spontaneously. Meaning: limbs like a seal.
Phytotherapy
Western-style, scientific treatment using plant extracts or materials.
Placebo
An inactive substance (often called a sugar pill) given to a group being studied to compare results with the
effects of the active drug.
Polypharmacy
The concomitant use of more than one drug, sometimes prescribed by different practitioners.
Post-authorisation safety study (PASS)
A pharmacoepidemiological study or a clinical trial carried out in accordance with the terms of the Marketing
Authorisation, with the aim of identifying or quantifying a safety hazard relating to an authorised medicinal
product.
Post-marketing
The stage when a drug is generally available on the market.
Predisposing factors
Any aspect of the patient's history (other than the drug) which might explain reported adverse events (genetic
factors, diet, alcohol consumption, disease history, polypharmacy or use of herbal medicines, for example).
Pre-marketing
The stage before a drug is available for prescription or sale to the public.
Prescription event monitoring (PEM)
System created to monitor adverse drug events in a population. Prescribers are requested to report all events,
regardless of whether they are suspected adverse events, for identified patients receiving a specified drug. Also
more accurately named "cohort-event monitoring".
Prescription only medicine (POM)
Medicinal product available to the public only on prescription.
Prophylaxis
Prevention or protection.
PSUR
The Periodic Safety Update Report (PSUR) is a stand-alone document that allows a periodic but comprehensive
assessment of the worldwide safety data of a marketed drug or biological product.
QPPV
Qualified Person responsible for Pharmacovigilance
Rare
In pharmacovigilance an event with a probability between 1 in 10,000 and 1 in 1,000.
Rational drug use
An ideal of therapeutic practice in which drugs are prescribed and used in exact accordance with the best
understanding of their appropriateness for the indication and the particular patient, and of their benefit, harm
effectiveness and risk.
Rechallenge
The point at which a drug is again given to a patient after its previous withdrawal - see dechallenge (the point at
which the administration of drug has been stopped due to an event or adverse drug reaction).
Record linkage
Method of assembling information contained in two or more records, eg in different sets of medical charts, and in
vital records such as birth and death certificates. This makes it possible to relate significant health events that are
remote from one another in time and place.
Reference risk
Risk in a population of unexposed persons. Also called baseline risk. Reference risk can be measured over time
(incidence) or at a given time (prevalence). The unexposed population refers to a reference population, as closely
comparable to the exposed population as possible, apart from the exposure.
Regulatory authority
The legal authority in any country with the responsibility of regulating all matters relating to drugs.
Relative risk
Ratio of the risk in an exposed population (absolute risk) and the risk in an unexposed population (reference
risk). Relative risk is the result of a relative comparison between outcome frequency measurements, e.g.
incidences.
Risk
The probability of harm being caused; the probability (chance, odds) of an occurrence.
SAEC
The International Serious Adverse Events Consortium (SAEC) is a non-profit consortium formed in October 2007
between industry, academia, the Wellcome Trust, and the US Food and Drug Administration to identify genetic
variants associated with serious adverse events.
Serious Adverse Event or Reaction
A serious adverse event or reaction is any untoward medical occurrence that at any dose:
 results in death
 requires inpatient hospitalisation or prolongation of existing hospitalisation
 results in persistent or significant disability/incapacity
 is life-threatening
Side effect
Any unintended effect of a pharmaceutical product occurring at normal dosage which is related to the
pharmacological properties of the drug.
Signal
Reported information on a possible causal relationship between an adverse event and a drug, the relationship
being unknown or incompletely documented previously. Usually more than a single report is required to generate
a signal, depending upon the seriousness of the event and the quality of the information. The publication of a
signal usually implies the need for some kind of review or action.
Spontaneous reporting
System whereby case reports of adverse drug events are voluntarily submitted from health professionals and
pharmaceutical manufacturers to the national regulatory authority.
Thalidomide
Drug prescribed in the 1950s as a mild sleeping pill and remedy for morning-sickness for pregnant women. Led to
serious birth defects. Returning to favour as a treatment for leprosy.
Traditional medicines
Traditional medicine is the sum total of the knowledge, skills, and practices based on the theories, beliefs, and
experiences indigenous to different cultures, whether explicable or not, used in the maintenance of health as well
as in the prevention, diagnosis, improvement or treatment of physical and mental illness. See also allopathic

Uncommon
In pharmacovigilance an event with a frequency between 1 in 1.000 and 1 in 100.
Unexpected adverse reaction
An adverse reaction, the nature or severity of which is not consistent with domestic labelling or market
authorization, or expected from characteristics of the drug.
Vigibase
The name for the WHO International ADR Database.
Vigimed
E-mail conferencing facility, exclusive to member countries of the WHO Programme for International Drug
Monitoring.
Establishing Pharmacovigilance programme: The nation wide programme, sponsored and coordinated by the
country’s central drug regulatory agency – Central Drugs Standard Control Organization (CDSCO) – to establish and
manage a data base of Adverse Drug Reactions (ADR) for making informed regulatory decisions regarding marketing
authorization of drugs in India for ensuring safety of drugs.
The National Pharmacovigilance Centre at CDSCO coordinates the programme. The National Centre will operate under the
supervision of the National Pharmacovigilance Advisory Committee (NPAC) to recommend procedures and guidelines for
regulatory interventions.
A new pharmacovigilance centre can start operating very quickly. The development of a
pharmacovigilance system, however, from the first and uncertain stage to becoming an established and
effective organisation, is a process that needs time, vision, dedication, expertise and continuity. The
most promising location for a new pharmacovigilance centre may depend on the organisation and
development of the healthcare system in the country and other local issues.
A governmental department (health authority, drug regulatory agency) can be a good host for a
pharmacovigilance centre. However, any department in a hospital or academic environment, working in
clinical pharmacology, clinical pharmacy, clinical toxicology or epidemiology, may be a suitable starting
point for pharmacovigilance. The reporting of adverse drug reactions may start locally, perhaps in one
hospital, then extend to other hospitals and family practices in the region, and progress step by step
into a national activity. In some countries professional bodies such as the national medical association
may be a good home for the centre.
When the centre is a country-wide organisation from the start, it should be remembered that much
effort, especially in effective communications, will be needed before a substantial proportion of
practitioners are contributing.
When a centre is part of a larger organisation (for example, a poison control unit, a clinical
pharmacology department, or a hospital pharmacy) providing administrative continuity, it can get going
as long as there is one professional (e.g. a physician or pharmacist) available who is primarily
responsible for pharmacovigilance.
Whatever the location of the centre, pharmacovigilance is closely linked to drug regulation.
Governmental support is needed for national co-ordination. Pharmacovigilance is nobody’s individual
privilege. Good collaboration, co-ordination, communications and public relations are needed for
coherent development and for the prevention of unnecessary competition or duplication.
Pharmacovigilance in the industry: Pharmaceutical companies are international,
hence the number of staff working in this field within the industry, particularly in other
European countries and the USA, is far greater. A major pharmaceutical company such
as Astra has over 100 permanent, experienced staff in pharmacovigilance within its
research and development organisation in Sweden and the UK and a similar number in
local operating companies worldwide. This development has been driven by an increased
recognition of the role of pharmacovigilance, the investigation and marketing of a wider
range of diverse medicinal products and more stringent and detailed regulatory
requirements. The number of individual reports of possible adverse drug reactions
(ADRs) can be considerable, for key marketed products often more than 1000 case
reports a year are received worldwide from health care professionals and other sources.
The aims of pharmacovigilance within the industry are essentially the same as those of
regulatory agencies; that is to protect patients from unnecessary harm by identifying
previously unrecognised drug hazards, elucidating pre-disposing factors, refuting false
safety signals and quantifying risk in relation to benefit. Although the perspectives of
companies and the regulatory agencies may be different they now work more and more
closely together and share information. However, central pharmacovigilance units in
major pharmaceutical companies in many instances are far better resourced and have
much greater ‘in-house’ expertise on the safety of their particular products.
Roles & Responsibilities of a Drug Safety Associate:
1. Needs to ensure that most of the safety reports that are received from the
investigational sites or from the post- marketing sector must be identified &
reported in accordance to the ICH-GCP standards & also by the standard
operational procedures.
2. Reports that are collected must be organized based on the seriousness or
effectiveness of the drug initially.
3. An overall consistency in assessing the adverse reaction reports must be
accurate & legible.
4. Must be judgmental in taking appropriate decisions by applying their
knowledge of FDA & ICH-GCP standard regulations.
5. Must be able to communicate with clinical physicians in drug safety and
assess case report forms.
6. Must be able to distinguish & analyze the ratio of risks & benefits of a drug
that is marketed.
Licence Partners, Contract Research Organisations (CROs) and Market
Authorisation Holders (MAH):
Identification of Medicinal Products (IDMP) is a worldwide effort to harmonize medicinal product
specifications across regulators and industry as part of continuing efforts towards harmonisation in the
pharmaceutical industry. It covers the whole life-cycle of a product from the laboratory onwards.
When implemented, IDMP will provide the basis for the unique identification of medicinal products
and will facilitate the exchange of information between competent authorities, clinical trial sponsors
and marketing authorisation holders (MAH) world-wide. IDMP is based on a set of five ISO standards
published in 2012. These standards establish definitions and concepts describing data elements and
their structural relationships that support the full description of any medicinal product worldwide in a
structured and unique manner. The EU is the first region in the world to start the implementation of
IDMP. Implementation is following a phased approach. A major phase has already started, as EMA
initiated the process of standardising organisation data i.e. organisation name and location address for
relevant stakeholders such as MAHs, sponsors, regulatory authorities, manufacturers and CROs as well
as referential data i.e. standard lists of terms such as catalogues of dosage forms, units of
measurement, and routes of administration. IDMP implementation will require significant effort
particularly from the competent authorities, but also from MAHs, clinical trial sponsors,
manufacturers, CROs and other stakeholders.

Adverse drug reaction reporting:
Reporting to regulatory authorities: Best practices for international reporting are that all “affiliates” of
a sponsor report serious, unexpected, and possibly related events to the sponsor in a timely fashion,
ideally within 2 calendar days; this allows the sponsor, in turn, to complete notification to the
responsible regulatory authority within a total of 15 calendar day

Pharmacovigilance has been developed by WHO solely with the intention of responding to the special needs. Over the last decade, it has been increasingly recognized that the scope of pharmacovigilance needs to be extended more , much beyond the present signals of safety concerns. Globalization, consumerism, the resulting expansion in free trade and communication across national and international borders, and increasing use of the Internet have all contributed to a change so that people can access information on all types of medicinal products. So pharmacovigilance is indeed important and the changing patterns in drug use require a shift in the approach to pharmacovigilance so that it can respond to the prevailing patterns of drug use within society in a better way. In today's advanced age of medicines, science and technology, there has been an increasing public awareness of safety issues related to medical products. This has led to an increased importance of pharmacovigilance. Pharmacovigilance in India & global perspective - Pharmacovigilance (PV) plays a key role in the healthcare system through assessment, monitoring and discovery of interactions amongst drugs and their effects in human. Pharmaceutical and biotechnological medicines are designed to cure, prevent or treat diseases; however, there are also risks particularly adverse drug reactions (ADRs) can cause serious harm to patients. Thus, for safety medication ADRs monitoring required for each medicine throughout its life cycle, during development of drug such as pre- marketing including early stages of drug design, clinical trials, and post-marketing surveillance. PV is concerns with the detection, assessment, understanding and prevention of ADRs. Pharmacogenetics and pharmacogenomics are an indispensable part of the clinical research. Variation in the human genome is a cause of variable response to drugs and susceptibility to diseases are determined, which is important for early drug discovery to PV. Moreover, PV has traditionally involved in mining spontaneous reports submitted to national surveillance systems. The research focus is shifting toward the use of data generated from platforms outside the conventional framework such as electronic medical records, biomedical literature, and patient-reported data in health forums. The emerging trend in PV is to link premarketing data with human safety information observed in the post-marketing phase. The PV system team obtains valuable additional information, building up the scientific data contained in the original report and making it more informative. This necessitates an utmost requirement for effective regulations of the drug approval process and conscious pre and post approval vigilance of the undesired effects, especially in India. Adverse events reported by PV system potentially benefit to
  • 10. the community due to their proximity to both population and public health practitioners, in terms of language and knowledge, enables easy contact with reporters by electronically. Hence, PV helps to the patients get well and to manage optimally or ideally, avoid illness is a collective responsibility of industry, drug regulators, clinicians and other healthcare professionals to enhance their contribution to public health. This review summarized objectives and methodologies used in PV with critical overview of existing PV in India, challenges to overcome and future prospects with respect to Indian context. WHO international drug monitoring programme- In 1968, during the 16th World Assembly the 16.36 resolution called for “a systematic collection of information on serious adverse drug reactions during the development and particularly after medicines have been made available for public use”. This led to the formation of the WHO Programme for International Drug Monitoring (PIDM). Basic terminologies used in Pharmacovigilance- Absolute risk Risk in a population of exposed persons; the probability of an event affecting members of a particular population (e.g. 1 in 1,000). Absolute risk can be measured over time (incidence) or at a given time (prevalence). See also attributable risk and relative risk Adverse event Any untoward medical occurrence that may present during treatment with a pharmaceutical product but which does not necessarily have a causal relationship with this treatment. Synonym: adverse experience Adverse (drug) reaction (ADR) A response which is noxious and unintended, and which occurs at doses normally used in humans for the prophylaxis, diagnosis, or therapy of disease, or for the modification of physiological function. (WHO, 1972). An adverse drug reaction, contrary to an adverse event, is characterized by the suspicion of a causal relationship between the drug and the occurrence, i.e. judged as being at least possibly related to treatment by the reporting or a reviewing health professional. Allopathy Non-traditional, western scientific therapy, usually using synthesised ingredients, but may also contain a purified active ingredient extracted from a plant or other natural source; usually in opposition to the disease. Compare homeopathy. Association Events associated in time but not necessarily linked as cause and effect. Attributable risk Difference between the risk in an exposed population (absolute risk) and the risk in an unexposed population (reference risk). Also referred to as excess risk. Attributable risk is the result of an absolute comparison between outcome frequency measurements, such as incidence. Examples: If the exposed persons with a particular outcome are A, the exposed persons without the outcome are B, the unexposed persons with the outcome are C and the unexposed persons with the outcome are D, then the attributable risk is calculated as : [A / (A+B)] - [C / (C+D)]. If, during the same time period, the incidence of rash

Benefit - risk analysis Examination of the favourable (beneficial) and unfavourable results of undertaking a specific course of action. (While this phrase is still commonly used, the more logical pairings of benefit-harm and effectiveness-risk are slowly replacing it). Biological products Medical products prepared from biological material of human, animal or microbiologic origin (such as blood products, vaccines, insulin). Causality assessment The evaluation of the likelihood that a medicine was the causative agent of an observed adverse reaction. Causality assessment is usually made according established algorithms. Caveat document The formal advisory warning accompanying data release from the WHO Database: it specifies the conditions and reservations applying to interpretations and use of the data. CIOMS The Council for International Organizations of Medical Sciences. CIOMS is a body set up under World Health Organization and UNESCO. It has developed a series of guidelines on pharmacovigilance, drawn up by a committee of volunteers from Industry, regulatory authorities, WHO and others. The main guidelines concern the international reporting form (CIOMS I); periodic safety update reports (CIOMS II); core data sheets (CIOMS III); benefit-risk assessments (CIOMS IV); practical issues in pharmacovigilance (CIOMS V); clinical trial safety data (CIOMS VI); and development safety update reports (CIOMS VII). Clinical trial A systematic study on pharmaceutical products in human subjects (including patients and other volunteers) in order to discover or verify the effects of and/or identify any adverse reaction to investigational products, and/or to study the absorption, distribution, metabolism and excretion of the products with the objective of ascertaining their efficacy and safety. Common Epidemiology In pharmacovigilance, an event with a frequency between 1 in 100 and 1 in 10 Compliance Faithful adherence by the patient to the prescriber's instructions. Efficacy The ability of a drug to produce the intended effect as determined by scientific methods. Epidemiology The science concerned with the study of the factors determining and influencing the frequency and distribution of disease, injury and other health-related events and their causes in a defined human population for the purpose of establishing programs to prevent and control their development and spread (Dorland's Illustrated Medical Dictionary). Pre-marketing The stage before a drug is available for prescription or sale to the public. Prescription event monitoring (PEM) System created to monitor adverse drug events in a population. Prescribers are requested to report all events, regardless of whether they are suspected adverse events, for identified patients receiving a specified drug. Also more accurately named "cohort-event monitoring". Prescription only medicine (POM) Medicinal product available to the public only on prescription. Prophylaxis Prevention or protection. PSUR The Periodic Safety Update Report (PSUR) is a stand-alone document that allows a periodic but comprehensive assessment of the worldwide safety data of a marketed drug or biological product. Risk The probability of harm being caused; the probability (chance, odds) of an occurrence. SAEC The International Serious Adverse Events Consortium (SAEC) is a non-profit consortium formed in October 2007 between industry, academia, the Wellcome Trust, and the US Food and Drug Administration to identify genetic variants associated with serious adverse events. Serious Adverse Event or Reaction A serious adverse event or reaction is any untoward medical occurrence that at any dose:  results in death  requires inpatient hospitalisation or prolongation of existing hospitalisation  results in persistent or significant disability/incapacity  is life-threatening Side effect Any unintended effect of a pharmaceutical product occurring at normal dosage which is related to the pharmacological properties of the drug. Signal Reported information on a possible causal relationship between an adverse event and a drug, the relationship being unknown or incompletely documented previously. Usually more than a single report is required to generate a signal, depending upon the seriousness of the event and the quality of the information. The publication of a signal usually implies the need for some kind of review or action. Spontaneous reporting System whereby case reports of adverse drug events are voluntarily submitted from health professionals and pharmaceutical manufacturers to the national regulatory authority. Thalidomide Drug prescribed in the 1950s as a mild sleeping pill and remedy for morning-sickness for pregnant women. Led to serious birth defects. Returning to favour as a treatment for leprosy.

Establishing Pharmacovigilance programme: The nation wide programme, sponsored and coordinated by the country’s central drug regulatory agency – Central Drugs Standard Control Organization (CDSCO) – to establish and manage a data base of Adverse Drug Reactions (ADR) for making informed regulatory decisions regarding marketing authorization of drugs in India for ensuring safety of drugs. The National Pharmacovigilance Centre at CDSCO coordinates the programme. The National Centre will operate under the supervision of the National Pharmacovigilance Advisory Committee (NPAC) to recommend procedures and guidelines for regulatory interventions. A new pharmacovigilance centre can start operating very quickly. The development of a pharmacovigilance system, however, from the first and uncertain stage to becoming an established and effective organisation, is a process that needs time, vision, dedication, expertise and continuity. The most promising location for a new pharmacovigilance centre may depend on the organisation and development of the healthcare system in the country and other local issues. A governmental department (health authority, drug regulatory agency) can be a good host for a pharmacovigilance centre. However, any department in a hospital or academic environment, working in clinical pharmacology, clinical pharmacy, clinical toxicology or epidemiology, may be a suitable starting point for pharmacovigilance. The reporting of adverse drug reactions may start locally, perhaps in one hospital, then extend to other hospitals and family practices in the region, and progress step by step into a national activity. In some countries professional bodies such as the national medical association may be a good home for the centre. When the centre is a country-wide organisation from the start, it should be remembered that much effort, especially in effective communications, will be needed before a substantial proportion of practitioners are contributing. When a centre is part of a larger organisation (for example, a poison control unit, a clinical pharmacology department, or a hospital pharmacy) providing administrative continuity, it can get going as long as there is one professional (e.g. a physician or pharmacist) available who is primarily responsible for pharmacovigilance.
  • 17. Whatever the location of the centre, pharmacovigilance is closely linked to drug regulation. Governmental support is needed for national co-ordination. Pharmacovigilance is nobody’s individual privilege. Good collaboration, co-ordination, communications and public relations are needed for coherent development and for the prevention of unnecessary competition or duplication. Pharmacovigilance in the industry: Pharmaceutical companies are international, hence the number of staff working in this field within the industry, particularly in other European countries and the USA, is far greater. A major pharmaceutical company such as Astra has over 100 permanent, experienced staff in pharmacovigilance within its research and development organisation in Sweden and the UK and a similar number in local operating companies worldwide. This development has been driven by an increased recognition of the role of pharmacovigilance, the investigation and marketing of a wider range of diverse medicinal products and more stringent and detailed regulatory requirements. The number of individual reports of possible adverse drug reactions (ADRs) can be considerable, for key marketed products often more than 1000 case reports a year are received worldwide from health care professionals and other sources. The aims of pharmacovigilance within the industry are essentially the same as those of regulatory agencies; that is to protect patients from unnecessary harm by identifying previously unrecognised drug hazards, elucidating pre-disposing factors, refuting false safety signals and quantifying risk in relation to benefit. Although the perspectives of companies and the regulatory agencies may be different they now work more and more closely together and share information. However, central pharmacovigilance units in major pharmaceutical companies in many instances are far better resourced and have much greater ‘in-house’ expertise on the safety of their particular products. Roles & Responsibilities of a Drug Safety Associate: 1. Needs to ensure that most of the safety reports that are received from the investigational sites or from the post- marketing sector must be identified & reported in accordance to the ICH-GCP standards & also by the standard operational procedures. 2. Reports that are collected must be organized based on the seriousness or effectiveness of the drug initially. 3. An overall consistency in assessing the adverse reaction reports must be accurate & legible. 4. Must be judgmental in taking appropriate decisions by applying their knowledge of FDA & ICH-GCP standard regulations. 5. Must be able to communicate with clinical physicians in drug safety and assess case report forms. 6. Must be able to distinguish & analyze the ratio of risks & benefits of a drug that is marketed.

Licence Partners, Contract Research Organisations (CROs) and Market Authorisation Holders (MAH): Identification of Medicinal Products (IDMP) is a worldwide effort to harmonize medicinal product specifications across regulators and industry as part of continuing efforts towards harmonisation in the pharmaceutical industry. It covers the whole life-cycle of a product from the laboratory onwards. When implemented, IDMP will provide the basis for the unique identification of medicinal products and will facilitate the exchange of information between competent authorities, clinical trial sponsors and marketing authorisation holders (MAH) world-wide. IDMP is based on a set of five ISO standards published in 2012. These standards establish definitions and concepts describing data elements and their structural relationships that support the full description of any medicinal product worldwide in a structured and unique manner. The EU is the first region in the world to start the implementation of IDMP. Implementation is following a phased approach. A major phase has already started, as EMA initiated the process of standardising organisation data i.e. organisation name and location address for relevant stakeholders such as MAHs, sponsors, regulatory authorities, manufacturers and CROs as well as referential data i.e. standard lists of terms such as catalogues of dosage forms, units of measurement, and routes of administration. IDMP implementation will require significant effort particularly from the competent authorities, but also from MAHs, clinical trial sponsors, manufacturers, CROs and other stakeholders.
