Child Interview Project Packet Template
(Adapted from the work of Dr. Ann Hussmann.
Revised January 2017)

this project.
It is a WORD document. The fields will expand as you type.
1. Background Information
IMPORTANT: Add any additional questions that you may find
helpful in writing your summary and conclusion, or delete
questions if you find them in appropriate. If you delete a
question be sure to explain why you deleted it. You may want
to gather this information from the parent or guardian.

2. Interview Questions
Print this out and write in your answers during your interview
with the child. Type the answers into this section before you
submit your final report.Add any questions that you believe will
help you with your summary.

What does a person need to do to be healthy?
What is the least healthy thing you eat? How often to you get
to eat it?

Do you think that you are too fat or too skinny or just right?
What is your bed time? Is it hard for you to fall asleep at
night?

3. PIAGETIAN CONSERVATION TASKS
Conduct the following conservation tasks with the child.
Use actual materials DO NOT just show them this chart.
This information will help you determine the Piagetian Stage
in which this child falls.

with the child that they are the same.
Take one and roll it into a “snake.” Ask the child, “Which
piece has more?

in length. Establish with the child that they are the same.
Move one stick over to the right.
Ask, “Which one is longer.”

Set up two equal lines of checkers (or substitute). Establish that
the two rows have the same amount.
Increase the spacing between the checkers in one of the rows.
Ask the child “Which line has more?”
4. FAMILY PORTRAIT
Give the child a blank piece of paper and colored pencils or
markers. Ask the child to draw a picture of his/her family.
Allow the child to choose who he wishes to include in this
picture. When they are done ask them to “Tell me about your
pictures.” This information will provide you with insights
about ways in which the child views the people in his family
and will help you with your interview. Record the child’s
exact words below:
5. ANIMISM QUESTIONS
Record your answers in the spaces provided and the reason
“why” they answered the way they did. Pay close attention to
the REASON the children give you for each question when you
are writing your summary.

2. Is a cell phone alive? Why or why not or why do you think
so?
3. Is a plant alive? Why or why not ?
4. Is fire alive? How do you know?
5. Is a puppet alive? Why or why not?
6. SUMMARY (60 points) You may delete everything in red
before submit your Project Packet. Wording in red DOES NOT
COUNT toward the minimum 5-page requirement.
Directions: This is a very important part of the project and is
worth 60 points. For the Summary you will combine the
information you learned from reading your textbook and the
data you collected from your interview to formulate conclusions
about the child.
This summary must be a minimum of five (5) full pages, double
spaced, with a 12 point font for full credit. It must include all
of the criteria below. Be sure that you clearly understand what
is required in this summary BEFORE you conduct the interview.

How to Quote and Cite the
Textbook
This is an excerpt (segment) of a summary to illustrate the
proper way to quote and cite the
textbook for this project. This is an example of the APA
(American Psychological Association)
style. As you can see, there are two ways to do this.
1. The first example starts with According to Feldman (2016)
and then the exact quote and
page number are included at the end.
2. The second example starts off with the actual quote. The
author, date, and page number
are listed at the end.
Use the style that makes the most sense and fits with what you
are writing. You can use one
style in one section and the other style in a different section.
Physical Development
Alyssa seems to be a happy, active 12 year old. She is
enrolled in dance and martial arts.
Her parents seems to keep her physically active and she
mentioned that “recess” was her
favorite subject in school because she gets to play four-square
and jump rope with her friends.
This is encouraging as many children are not this active. The
only issue that Alyssa has is
asthma which she keeps under control with her inhalers. The
fact that Alyssa has asthma is not
really surprising. According to Feldman (2016) “more than 7
million U.S. children suffer from
this disorder and worldwide the number is more that 150
million” (p. 286). It is important
that she has it under control so she can participate in sports and
other activities.
Alyssa is a normal height and weight for her age.
In addition to being physically
active, she mentioned that her mom makes her eat healthy foods
and that she only gets treats
once in a while. Unfortunately, not all parents are like Alyssa’s
parents. “Despite their
knowledge that certain foods are necessary for a balanced
nutritious diet, many parents
provide their children with too few fruits and vegetables and
more fasts and sweets than
recommended” (Feldman 2016, p. 284).

Revised January 2017) Save this document to your desktop and use it as a template for this project. It is a WORD document. The fields will expand as you type. 1. Background Information IMPORTANT: Add any additional questions that you may find helpful in writing your summary and conclusion, or delete questions if you find them in appropriate. If you delete a question be sure to explain why you deleted it. You may want to gather this information from the parent or guardian. What makes you feel happy? 3. PIAGETIAN CONSERVATION TASKS Conduct the following conservation tasks with the child. Use actual materials DO NOT just show them this chart.   • 5. Show the child two identical balls of play dough. Establish with the child that they are the same. Take one and roll it into a “snake.” Ask the child, “Which piece has more? Conservation Of Length Show the child two unsharpened pencils (or a substitute) equal in length. Establish with the child that they are the same. Move one stick over to the right. Ask, “Which one is longer.”
  • 6. Conservation of Number Set up two equal lines of checkers (or substitute). Establish that the two rows have the same amount. Increase the spacing between the checkers in one of the rows. Ask the child “Which line has more?” 4. FAMILY PORTRAIT Give the child a blank piece of paper and colored pencils or markers. Ask the child to draw a picture of his/her family. Allow the child to choose who he wishes to include in this picture. When they are done ask them to “Tell me about your pictures.” This information will provide you with insights about ways in which the child views the people in his family and will help you with your interview. Record the child’s exact words below:
  • 7. 5. ANIMISM QUESTIONS Record your answers in the spaces provided and the reason “why” they answered the way they did. Pay close attention to the REASON the children give you for each question when you are writing your summary. 1. Is a dog alive? Why or why not? 2. Is a cell phone alive? Why or why not or why do you think so? 3. Is a plant alive? Why or why not ? 4. Is fire alive? How do you know? 5. Is a puppet alive? Why or why not? 6. SUMMARY (60 points) You may delete everything in red before submit your Project Packet. Wording in red DOES NOT COUNT toward the minimum 5-page requirement.
  • 8. Directions: This is a very important part of the project and is worth 60 points. For the Summary you will combine the information you learned from reading your textbook and the data you collected from your interview to formulate conclusions about the child. This summary must be a minimum of five (5) full pages, double spaced, with a 12 point font for full credit. It must include all of the criteria below. Be sure that you clearly understand what is required in this summary BEFORE you conduct the interview. Use the BOLDED HEADINGS (TITLES) below for each section or I deduct points. Introduction (Use 12 point font and double-spacing. Delete instructions in red prior to submission.) Begin with a general introduction describing the child and your overall experience with this project. Include the background information here directly from your interview here to support your conclusions. Physical Development (Use 12 point font and double-spacing. Delete instructions in red prior to submission.)Describe the physical development of the child. Explain if their development matched (or does not match) the statistics in your textbook? What other conclusions can you make about his/her physical development and overall health? Justify your conclusions by formally citing information from the textbook and your interview. This is an example of the APA (American Psychological Association) style. As you can see, there are two ways to do this. 1. The first example starts with According to Feldman (2016) and then the exact quote and page number are included at the end. 2. The second example starts off with the actual quote. The author, date, and page number are listed at the end. Use the style that makes the most sense and fits with what you are writing. You can use one style in one section and the other style in a different section. Physical Development
  • 16. Alyssa seems to be a happy, active 12 year old. She is enrolled in dance and martial arts. Her parents seems to keep her physically active and she mentioned that “recess” was her favorite subject in school because she gets to play four-square and jump rope with her friends. This is encouraging as many children are not this active. The only issue that Alyssa has is asthma which she keeps under control with her inhalers. The fact that Alyssa has asthma is not really surprising. According to Feldman (2016) “more than 7 million U.S. children suffer from this disorder and worldwide the number is more that 150 million” (p. 286). It is important that she has it under control so she can participate in sports and other activities. Alyssa is a normal height and weight for her age. In addition to being physically active, she mentioned that her mom makes her eat healthy foods and that she only gets treats once in a while. Unfortunately, not all parents are like Alyssa’s parents. “Despite their knowledge that certain foods are necessary for a balanced nutritious diet, many parents
