Introduction
The Munchausen Syndrome Article explains about the Munchausen Syndrome, which is a rare fictitious disorder which involves the frequent hospitalization together with an intentional display of signs of sickness and pathological lying. In this regards, the management needs the security history taking with collaboration with the sound clinical processes which entails organicity exclusion in addressing the psychological problems. It is worth noting that a case which is presented having unusual symptoms of same dimensions are as well discussed. The case in this regards brings the finer nuances in the assessment of the entity (Prakash., et al 2014).
Research Question
Based on the abstract of the article, it can be denoted that the research question of the article is the need to understand more on the Munchausen Syndrome as well as the symptoms and therefore the need to ensure that such issues are solved by having a sound clinical process to handle the problem. The problem for the case as well was to find out what caused the 19-year-old housewife to vomit pink substance.
Findings
The findings depict that the 19-year-old housewife was suffering from a factitious disorder, also termed as the Munchausen syndrome. The psychometry performed also showed that there is an elevation of scales of anxiety together with hysteria. Consequently, being managed in an empathetic as well as non-confrontational manner, the psychotherapy was intended to improve the positive coping abilities while at the same time improving the interpersonal relationships which had been imparted (Prakash., et al 2014).
Research Methods Used
The methods used involved observations and clinical assessments. Observations were done by checking regularly the presence of the bloodstained vomits as well as the asthenia and any forms of skin allergy. This was carried out to ensure that the patient had no issues. The observations, as well as little conversation, showed that there were no cases of psychiatric illnesses for the patient in the past. Further, the assessment entails involves the systematic examinations which were performed within the normal limit. The psychiatric evaluation was performed together with ward observations which were intended at revealing the comfortability of the patient while in the hospital (Prakash., et al 2014).
The credibility of the Source of Information
To know the credibility of sources, the authors are scrutinized where their qualifications and their areas of experience assessed to understand whether the information provided is related to the topic at hand. For this article, it can be denoted that all the information provided is credible. This is because all the four authors who contributed to the article have sufficient skills and knowledge pertaining to health-related disorders, and thus, their pieces of information.
Resilience family empowerment
This study examined the role of family empowerment and resilience in predicting recovery from psychosis. Ninety-nine family members of patients with psychotic disorders completed assessments of family empowerment, resilience, and the patients' recovery. Regression analysis found that higher levels of family empowerment and resilience significantly predicted greater recovery in patients, accounting for 8.7% of the variance in recovery outcomes. The findings suggest that strengthening family empowerment and resilience may benefit the treatment of psychotic disorders.

Brief Psychiatric Rating Scale
The Brief Psychiatric Rating Scale (BPRS) was developed in the sixties. It is still one of the most popular behavioral rating scales/instruments use today by clinicians to quickly gather information about the possible presence and severity of various psychiatric symptoms and to assess changes in symptoms in response to medications (Zanello et al., 2013). Originally, the BPRS was a 16-item scale, it was later extended to the standard 18-item version and currently expanded to a 24-item scale to measure additional aspects of schizophrenia symptoms thereby increasing its sensitivity to psychotic and affective disorders and to be used for patients living in the community (Shafer et al., 2017).
The 18-item BPRS assess the following symptoms: somatic concern, anxiety, emotional withdrawal, conceptual disorganization, guilt feelings, tension, mannerisms and posturing, grandiosity, depressive mood, hostility, suspiciousness, hallucinatory behavior, motor retardation, uncooperativeness, unusual thought content, blunted affect, excitement, and disorientation (Yee et al., 2017). The manual of administration of the 24-item BPRS offers a more detailed semi-structured interview with more probe questions for each symptom, and providing supplementary rules for the rating (e.g., delusions) including a well-defined anchor point (Zanello et al., 2013). The recent analysis of the 24-item BPRS produced a four-factor solution: Negative Symptoms, Positive Symptoms, Manic-hostility, and Anxiety–Depression (Zanello et al., 2013). The current BPRS is rated on a seven-point Likert-type scale. A rating of “1” indicates the absence of symptoms, ratings of “2–3” indicate “very mild” to “mild” symptoms that are considered to have nonpathological inte.

For this Assignment, you will document information about a patient that you examined during the last 3 weeks, using the Focused SOAP Note Template provided. You will then use this note to develop and record a case presentation for this patient. Be sure to incorporate any feedback you received on your Week 3 and Week 7 case presentations into this final presentation for the course.
To Prepare
Select a child or adolescent patient that you examined during the last 3 weeks who presented with a disorder for which you have not already created a Focused SOAP Note in Weeks 3 or 7. (For instance, if you selected a patient with anorexia nervosa in Week 7, you must choose a patient with another type of disorder for this week.)
Create a Focused SOAP Note on this patient using the template provided in the Learning Resources..
Please Note:
When you submit your note, you should include the complete focused SOAP note as a Word document and PDF/images of each page that is initialed and signed by your Preceptor.

Introduction
The Munchausen Syndrome Article explains about the Munchausen Syndrome, which is a rare fictitious disorder which involves the frequent hospitalization together with an intentional display of signs of sickness and pathological lying. Research Question
Based on the abstract of the article, it can be denoted that the research question of the article is the need to understand more on the Munchausen Syndrome as well as the symptoms and therefore the need to ensure that such issues are solved by having a sound clinical process to handle the problem. The problem for the case as well was to find out what caused the 19-year-old housewife to vomit pink substance.
Findings
The findings depict that the 19-year-old housewife was suffering from a factitious disorder, also termed as the Munchausen syndrome. The psychometry performed also showed that there is an elevation of scales of anxiety together with hysteria. Consequently, being managed in an empathetic as well as non-confrontational manner, the psychotherapy was intended to improve the positive coping abilities while at the same time improving the interpersonal relationships which had been imparted (Prakash., et al 2014).
Research Methods Used
The methods used involved observations and clinical assessments. Observations were done by checking regularly the presence of the bloodstained vomits as well as the asthenia and any forms of skin allergy. This was carried out to ensure that the patient had no issues. The observations, as well as little conversation, showed that there were no cases of psychiatric illnesses for the patient in the past. Further, the assessment entails involves the systematic examinations which were performed within the normal limit. The psychiatric evaluation was performed together with ward observations which were intended at revealing the comfortability of the patient while in the hospital (Prakash., et al 2014).
The credibility of the Source of Information
To know the credibility of sources, the authors are scrutinized where their qualifications and their areas of experience assessed to understand whether the information provided is related to the topic at hand. For this article, it can be denoted that all the information provided is credible. This is because all the four authors who contributed to the article have sufficient skills and knowledge pertaining to health-related disorders, and thus, their pieces of information.
Resilience family empowerment
This study examined the role of family empowerment and resilience in predicting recovery from psychosis. Ninety-nine family members of patients with psychotic disorders completed assessments of family empowerment, resilience, and the patients' recovery. Regression analysis found that higher levels of family empowerment and resilience significantly predicted greater recovery in patients, accounting for 8.7% of the variance in recovery outcomes. The findings suggest that strengthening family empowerment and resilience may benefit the treatment of psychotic disorders.

Create a Focused SOAP Note on this patient using the template provided in the Learning Resources..
Please Note:
When you submit your note, you should include the complete focused SOAP note as a Word document and PDF/images of each page that is initialed and signed by your Preceptor.
You must submit your SOAP note using SafeAssign. Note: If both files are not received by the due date, faculty will deduct points per the Walden Grading Policy.
Then, based on your SOAP note of this patient, develop a video case study presentation. Take time to practice your presentation before you record.
Include at least five scholarly resources to support your assessment, diagnosis, and treatment planning.
The Assignment
Present the full complex case study. Include chief complaint; history of present illness; any pertinent past psychiatric, substance use, medical, social, family history; most recent mental status exam; current psychiatric diagnosis including differentials that were ruled out; and plan for treatment and management.
Report normal diagnostic results as the name of the test and “normal” (rather than specific value). Abnormal results should be reported as a specific value.

History of Present Illness: Both clients present to the office with concerns regarding their
children being “taken away” by social services as she believes they parent too harshly. They
adamantly proclaim that they are very good, loving parents but believe that when a child is not
following the rules, strict and physical discipline is necessary. They both explain that they were
brought up this way and that physical or emotional abuse is not present. They explain that this is
part of their culture and all they know is right. The social worker referred to the family as “those
Mexicans” and seemed to be bias towards the family. Also, she seemed to not consider any
alternative for the family after they requested it based on their working schedule and after
explaining their cultural belief system.

Developmental history: Parents report that their two sons all have met their milestones
throughout their development and are ages 6 and 8 years old. “Needs to be added to”

Psychosocial history: Male client works full-time and overtime to provide for the family. “Needs

History of abuse/trauma: Based on the genogram created, both clients father and mother have a
history of disciplining their children similarly. They both would require the clients to hold
encyclopedias out in front of them until their arms “felt like they were going to fall off”. Both
clients report this type of discipline as being miserable and that they inherited this from their
parents. No physical or mental abuse of concern.
Review of Systems:
HEENT: No loss of vision or hearing. No sign of congestion, sore throat, or runny nose.
SKIN: Cool and dry skin with no signs of wounds or lesions.
CARDIOVASCULAR: No complaints of chest pain, tightness, discomfort or palpitations. S1,
S2 heard upon auscultation. 2+ pedal and radial pulses. No weight gain and no edema.
RESPIRATORY: Clear lung sounds with no adventitious sounds. No rhonchi or wheezes
auscultated, and chest is symmetrical. No cough present.
GASTROINTESTINAL: No nausea, emesis, or diarrhea. Abdomen soft, no distention, no
discomfort on palpation. Bowel sounds present in all four quadrants.
GENITOURINARY: No burning or discomfort when urinating.
NEUROLOGICAL: No double vision, headaches, seizures. No loss of memory. Alert and
oriented x person, place and time. Agitation present in male client.
