Low-frequency ultrasound therapy significantly increases blood flow to wounds in the lower extremities, as measured by skin perfusion pressure (SPP). A study of 17 patients found that those receiving 5 minutes of low-frequency ultrasound treatment saw their peri-wound SPP values increase by an average of 9.11 mmHg, significantly more than the 1.2 mmHg average increase of those receiving only conventional sharp debridement. This data supports using low-frequency ultrasound to promote wound healing through vasodilation and increased skin perfusion in lower extremity wounds.
Three patients with difficult to treat leg wounds were successfully treated with low-frequency ultrasound therapy. A 61-year-old male with rheumatoid arthritis and recurrent leg ulcers experienced complete wound closure and no pain. A 60-year-old female with lymphedema and ankle ulceration had improved pain control and wound healing. An 87-year-old female with a leg wound from a restraint strap had reduced pain and complete wound closure.
Three patients with difficult-to-treat lower extremity wounds were successfully treated with low-frequency ultrasound therapy. The first was a 61-year-old male with rheumatoid arthritis and recurrent pyoderma gangrenosum of the lower leg. With ultrasound therapy, his wound closed completely and his prednisone dose was tapered. The second was a 60-year-old female with lymphedema and ankle ulceration; her wounds became smaller and less painful with ultrasound. The third was an 87-year-old after a leg hematoma; her wound healed after prolonged ultrasound therapy despite other medical issues. All patients experienced reduced pain and improved healing.
An 82-year-old woman with severe third-degree burns on both feet refused transfer to a burn center and was instead treated locally using hyperbaric oxygen therapy (HBOT) and skin substitutes without autologous skin grafting. Her wounds were prepared with debridement and granulation was promoted using vacuum-assisted closure and HBO sessions. Due to her lack of donor skin, a meshed skin substitute was implanted and secured, achieving complete closure with dressings. The case demonstrates that combining HBOT and skin substitutes can heal severe burns without autologous grafts, eliminating pain and risks of traditional harvesting.
