Antiretroviral Prophylaxis for HIV Prevention in Heterosexual Men and Women. Baeten, et al. N Engl J Med 2012;367:399-410. Background: Antiretroviral preexposure prophylaxis is a promising approach for preventing human immunodeficiency virus type 1 (HIV-1) infection in heterosexual populations. Methods: We conducted a study of oral antiretroviral therapy for use as preexposure prophylaxis among HIV-1-serodiscordant heterosexual couples from Kenya and Uganda. The HIV- 1-seronegative partner in each couple was randomly assigned to one of three study regimens-- once-daily tenofovir (TDF), combination tenofovir-emtricitabine (TDFFTC), or matching placebo-- and followed monthly for up to 36 months....All couples received standard HIV-1 treatment and prevention services. Results: We enrolled 4758 couples, of whom 4747 were followed: 1584 randomly assigned to TDF, 1579 to TDF-FTC, and 1584 to placebo... Adverse Events among HIV/MDR-TB Co-Infected Patients Receiving Antiretroviral...
Abstract
Background
Significant adverse events (AE) have been reported in patients receiving medications for multidrug- and extensively-drug-resistant tuberculosis (MDR-TB & XDR-TB). However, there is little prospective data on AE in MDR- or XDR-TB/HIV co-infected patients on antituberculosis and antiretroviral therapy (ART) in programmatic settings.
Methods
Mรฉdecins Sans Frontiรจres (MSF) is supporting a community-based treatment program for drug-resistant tuberculosis in HIV-infected patients in a slum setting in Mumbai, India since 2007. Patients are being treated for both diseases and the management of AE is done on an outpatient basis whenever possible. Prospective data were analysed to determine the occurrence and nature of AE.
Results
Between May 2007 and September 2011, 67 HIV/MDR-TB co-infected patients were being treated with anti-TB treatment and ART; 43.3% were female, median age was 35.5 years (Interquartile Range: 30.5โ42) and the median duration of anti-TB treatment was 10 months (range 0.5โ30). Overall, AE were common in this cohort: 71%, 63% and 40% of patients experienced one or more mild, moderate or severe AE, respectively. However, they were rarely life-threatening or debilitating. AE occurring most frequently included gastrointestinal symptoms (45% of patients), peripheral neuropathy (38%), hypothyroidism (32%), psychiatric symptoms (29%) and hypokalaemia (23%). Eleven patients were hospitalized for AE and one or more suspect drugs had to be permanently discontinued in 27 (40%). No AE led to indefinite suspension of an entire MDR-TB or ART regimen.
Conclusions
AE occurred frequently in this Mumbai HIV/MDR-TB cohort but not more frequently than in non-HIV patients on similar anti-TB treatment. Most AE can be successfully managed on an outpatient basis through a community-based treatment program, even in a resource-limited setting. Concerns about severe AE in the management of co-infected patients are justified, however, they should not cause delays in the urgently needed rapid scale-up of antiretroviral therapy and second-line anti-TB treatment

This study examined early outcomes of decentralized care for rifampicin-resistant tuberculosis (RR-TB) in Johannesburg, South Africa. 214 patients with RR-TB were enrolled and followed until 6 months of treatment or a final outcome. Xpert MTB/RIF was used to diagnose 87% of cases. The median time to treatment initiation was 10 days. Interim outcomes at 6 months found 78% still on treatment, 13% had died, and 14% lost from treatment. Patients who started treatment within 1 week of diagnosis had a lower risk of loss from treatment. HIV co-infection occurred in 83% of patients.

This randomized clinical trial compared two post-exposure prophylaxis (PEP) regimens for preventing HIV infection: tenofovir/emtricitabine plus ritonavir-boosted lopinavir versus tenofovir/emtricitabine plus raltegravir. The trial found that while overall PEP non-completion at 28 days was similar between the two regimens, the raltegravir-containing regimen had significantly fewer adverse events and better adherence. The iPrEx trial found that daily oral Truvada (tenofovir/emtricitabine) reduced the risk of HIV infection by 44% among men who have sex with men when combined with comprehensive prevention services. Adherence was high at 95% but effectiveness was greater for those with consistent adherence above 90%. While daily Truvada was generally safe and well tolerated, its long term safety beyond the trial is unknown as is its effectiveness for other populations, routes of transmission, and outside of a clinical trial setting. This randomized controlled trial evaluated the efficacy of isoniazid prophylaxis in preventing tuberculosis in HIV-infected and HIV-exposed uninfected children in South Africa. 548 HIV-infected and 804 HIV-uninfected infants aged 3-4 months were randomized to receive either isoniazid or placebo for 96 weeks. The primary outcomes were tuberculosis-free survival in HIV-infected children and tuberculosis infection-free survival in HIV-uninfected children. There was no significant difference in the primary outcomes between the isoniazid and placebo groups for both HIV-infected and HIV-uninfected children. The burden of tuberculosis remained high among HIV-infected children despite antiretroviral therapy.

HIV treatment and PrEP options have advanced significantly since 2015. Key points:
1) Treatment as prevention is now recommended, with antiretroviral therapy shown to reduce HIV transmission by 96% and dramatically lower prevalence over time if treatment is scaled up.

This randomized controlled trial evaluated the efficacy of isoniazid prophylaxis in preventing tuberculosis in HIV-infected and HIV-exposed uninfected children in South Africa. 548 HIV-infected and 804 HIV-uninfected infants aged 3-4 months were randomized to receive either isoniazid or placebo for 96 weeks. The primary outcomes were tuberculosis-free survival in HIV-infected children and tuberculosis infection-free survival in HIV-uninfected children. There was no significant difference in the primary outcomes between the isoniazid and placebo groups for both HIV-infected and HIV-uninfected children. The burden of tuberculosis remained high among HIV-infected children despite antiretroviral therapy.

- 1. Antiretroviral Prophylaxis for HIV Prevention in Heterosexual Men and Women. Baeten, et al. N Engl J Med 2012;367:399-410. Background: Antiretroviral preexposure prophylaxis is a promising approach for preventing human immunodeficiency virus type 1 (HIV-1) infection in heterosexual populations. Methods: We conducted a study of oral antiretroviral therapy for use as preexposure prophylaxis among HIV-1-serodiscordant heterosexual couples from Kenya and Uganda. The HIV- 1-seronegative partner in each couple was randomly assigned to one of three study regimens-- once-daily tenofovir (TDF), combination tenofovir-emtricitabine (TDFFTC), or matching placebo-- and followed monthly for up to 36 months....All couples received standard HIV-1 treatment and prevention services. Results: We enrolled 4758 couples, of whom 4747 were followed: 1584 randomly assigned to TDF, 1579 to TDF-FTC, and 1584 to placebo... Conclusions: Oral TDF and TDF-FTC both protect against HIV-1 infection in heterosexual men and women.
