Radio Frequency Skin Tightening is an exciting break through for skin tightening, wrinkle treatments, and for facial rejuvenation. It is one of the few effective and lasting treatments to fight loose skin, wrinkles, and the effects of aging without radical invasive cosmetic surgery. Learn what you need to know about this breakthrough in cosmetic medicine in today's presentation, video, and article brought to you by Bay Area Pelleve.com
HIFU (High-Intensity Focused Ultrasound) has gained popularity in the field of dermatology as a non-invasive cosmetic procedure for skin tightening and rejuvenation. It is often used to address various skin concerns, primarily those related to aging and skin laxity. Here are some key points about HIFU in dermatology:
1. Skin Tightening: HIFU can stimulate collagen production and tighten the skin. It works by delivering focused ultrasound energy to the deep layers of the skin, causing thermal injury to the tissue. This injury prompts the body to produce new collagen, which leads to tighter, firmer skin over time.
2. Wrinkle Reduction: By promoting collagen production, HIFU can help reduce the appearance of fine lines and wrinkles, particularly on the face and neck.
3. Brow Lift: HIFU can lift and define the eyebrows, providing a subtle lifting effect that can open up the eyes and create a more youthful appearance.
4. Jawline Contouring: It is used to sculpt and define the jawline, reducing sagging and jowling.
5. Neck Rejuvenation: HIFU can improve the appearance of the neck by tightening loose skin and reducing the appearance of neckbands (vertical lines on the neck).
6. Décolletage Rejuvenation: It is used to address skin laxity and wrinkles on the chest area, often referred to as the décolletage.
7. Acne Scar Improvement:Some dermatologists use HIFU to improve the appearance of acne scars by stimulating collagen remodeling in scar tissue.
8. Non-Surgical Facelift: HIFU is sometimes referred to as a non-surgical facelift because it provides some of the lifting and tightening effects associated with surgical facelift procedures without the need for incisions or downtime.
One of the advantages of HIFU in dermatology is that it typically requires minimal to no downtime, and the results can gradually improve over several months as collagen production continues. However, the degree of improvement may vary from person to person, and multiple treatment sessions might be needed for optimal results.
It's important to have HIFU treatments performed by a qualified and experienced dermatologist or medical professional who is trained in the use of this technology, as the safety and effectiveness of the procedure depend on the skill of the practitioner. Additionally, individual suitability for HIFU should be assessed during a consultation, and potential risks and benefits should be discussed.

Haemostasis is very important in laparoscopic surgery. Vessel sealing with energy devises play a major role in keeping the surgical field clear. Energy devices are also used for tissue sealing and transection. Despite never types of energy devises electro-surgery is still very popular in gynaecological laparoscopy. Desiccation, dissection, and coagulation are the main effects of electro-surgery that are used for various purposes. Higher thermal injury with monopolar devices lead to the invention of bipolar devices with less tissue damage. Ligasure, pk gyrus, ENSEAL are some of the more advanced bipolar devices. Ultrasonic devices have the capability of coagulation and cutting tissues. During the process it can produce significant thermal injury. Thunderbeat combines bipolar and ultrasonic energy for coagulation and cutting respectively for more precise effects. Laser devices emit a beam of photons with a high degree of spatial and temporal coherence with tissue effects depending on the time of exposure and power density. CO2, Argon, Nd: YAG, KTP-532 are different laser types with different properties. Plasma is the fourth state of matter following solid, liquid and gas. Argon neutral plasma (System 7550TM ABC, Cardioblate) can produce energy in 3 forms including light, heat and kinetic energy. Laser and plasma energy are gaining more popularity for endometriosis surgery due to its localised effects and better preservation of ovarian follicles.
LASER SURGERY
Class 4 > 500 mW Surgical lasers
Introduction
Surgery using a laser to cut tissue instead of a scalpel
Laser scalpel
Highly focused laser beam efficiently ablates (either vaporize or chip away) the living tissue.
At the same time, it seals (welds) capillaries, small blood vessels, lymphatics, and nerve endings, with significant benefits to both patients and surgeons.
MECHANISM OF ACTION
Photovaporolysis
Char – remnant of non-fluid cellular component.
Photoplasmolysis
Crater
Zone of carbonization- the limit of vaporization
Zone of coagulation and thermal necrosis- Slightly farther away from the center. This zone will eventually die.
Zone of hyperthermia- Farthest from the center. Beyond these zones there is no effect Depth of the crater and the diameter of these zones are directly related to power density.
20-watt Carbon Dioxide laser with plume evacuator
Highly absorbed by water, making it perfect for tissue cutting, vaporization and acoustical destruction.
Thermal injury to surrounding tissue is very superficial.
Lateral thermal injury of 0.05 mm to 0.1 mm
Because there is such minimal lateral thermal damage, injury to surrounding tissues is limited to what you see during application of the laser energy.
General parameters for CO2 laser use
Routine incision: Spot Diameter: 0.4 mm
Power Setting: 6 to 10 W
Routine ablation/vaporization Spot Diameter: 0.8 mm
Power Setting: 10 to 20 W
Delicate location incision: Spot Diameter: 0.3 to 0.4 mm
Power Setting: 3 to 6 W
Routine excision: Spot Diameter: 0.8 mm
Power Setting: 8 to 15 W
LASER USES
Less Pain - The laser seals nerve endings as it cuts. So the patient will have less pain.
Less Bleeding - The laser seals small blood vessels during surgery and speeds up surgery by minimizing bleeding.
Less Swelling - No physical contact except the invisible laser beam. The tissue will not be crushed.
Sterilization - The laser sterilizes the surgical site as it cuts. Bacteria and viruses are vaporized by the laser during laser surgery.
Faster Recovery - Less bleeding and swelling will result in faster healing.
Precision - The beam direction and power can be controlled precisely to remove thin layers of tissue and produce minimal side effects on the surrounding healthy tissue.
Reduced hospitalization time - All above factor will greatly reduce the procedure time.
Laser surgery benefits for surgeons
Unique surgical capabilities: Laser surgery improves many surgical procedures by making them simpler and reducing risk. This enables surgeries that are not practical with conventional methods.

1. RF Devices’ Frequency
RF is measured in MHz to determine how quickly it cycles per second.
The higher the MHz, the faster the rotation to create more friction and heat.
The Accent XL is 7x more powerful than our closest competitor.
10. • Bipolar and Monopolar:
– Utilizes passing currents -
displacement of charged particles
• Unipolar:
– Utilizes rotation of water
molecules in a RF generated
electromagnetic field which
translates into friction and heat
generation
2. Electrode Configuration
11. How Bipolar Works?
• The Bipolar handpiece applies both electrodes at the
treatment site. Electrical energy and the heat caused by
the displacement of charged particles, are localized in a
small area. Bipolar energy only penetrates
superficially (up to ~6 mm) and effects only the area
between the electrodes.
12. Monopolar/Bipolar Comparison
Both Monopolar and Bipolar conduct heat in the same fashion and to the same
depth. The differentiation is the return pad is built into the handpiece with Bipolar.
13. How the Accent XL’s Exclusive
Unipolar HP Works
• Polarized molecules try to line themselves up with the electrical
field, like compass needles trying to point at North.
• But because the electrical field is changing 40 million times a
second, the molecules don't quite have time to line up one way
before they have to try to line up the other way!
• The rapidly oscillating electromagnetic field causes movement (a
rotation) of water molecules within the tissue. The resistance to
this movement creates friction and deep heat. This is similar to
your microwave oven at home.

25. Accent: Mechanism of Action
Subcutaneous heat leads to localized
thermally induced cascades of reactions:
1. Q10 effect
2. Vasodilatation and blood flow
3. Oxygen
4. Lymphatic drainage
26. Heat Effect #1: Q10 Effect
•Q10 effect means that
for every 10 degree
Celsius increase in
temperature, we get a
doubling of the
mitochondrial enzyme
activity
•This enhances local fat
metabolism
27. How Fat Cells Break Down and
The Body Absorbs Cell Contents
• The body’s primary source of
energy is glucose.
• When energy is needed, your
body breaks down stored fats
into glycerol and fatty acids to
produce glucose.
• As your body breaks down and
absorbs the contents of the fat
cells, the cells will shrink in size
but the number of fat cells
remains the same.
The deep heat from the Accent increases fat cell metabolism
and causes the release of fat cell contents and reduction in
the cell size.
28. Heat Effect #2 : Vasodilatation
• Heat causes vasodilatation
• Vasodilatation encourages
more blood flow to tissues
• Increased blood flow
results in more nutrient
delivery and cellular waste
removal
29. Heat Effect #3: O2-Hb Dissociation Curve
Effect of Temperature
•Increasing the
temperature denatures
the bond between oxygen
and hemoglobin, which
increases the amount of
oxygen.
•The dissociation curve
shifts to the right which
means better facilitation of
the release of oxygen
from the blood to the
tissue.
30. #4: Lymphatic
Drainage
• The microcirculation is
compressed by all the fatty
tissue resulting in reduced
delivery of nutrients (oxygen,
glucose), and retention of
metabolic wastes and lymphatic
fluid
• The heat generated by the
accent induces profound local
vasodilatation causing flow to
return to the region. Thus,
starved tissue received O2,
glucose and the flow flushes out
the waste products. This results
in a healthier local tissue.

The skin
The septae
The superficial facia
Note: There is no “give” or flexibility to the walls of the compartment
34. Cellulite Begins with Rigid
Compartments
• Heavy women develop cellulite
when they have too much
adipose in a normal space
• In this situation, the excess
adipose compresses the
delicate lymphatics and blood
vessels
• Thus there is poor nutrient
delivery and waste removal
• The septae contract in
response to the toxic
environment
• The contraction of the septae
reduces the size of this already
overfull space, worsening the
problem
• A vicious circle is formed

35. Cellulite Begins with Rigid
Compartments
• Trim women develop cellulite
when they have normal amounts
of adipose in a contracted space
• In this situation, the hormonal and
genetic factors cause the septae
to be short and the normal volume
of adipose compresses the
delicate lymphatics and blood
vessels
• Thus there is poor nutrient delivery
and waste removal
• The septae contract further in
response to the toxic environment
• The additional contraction of the
septae reduces the size of this
already overfull space, worsening
the problem
• A vicious circle is formed

36. Summary: The Accent and Cellulite
1. The Accent causes reduction in
adipose by the Q10 effect, thus
decompressing the
‘compartment’
2. The Accent improves
circulation and drainage by
vasodilatation (thus reducing
toxic effects on septae
3. The Accent improves the
oxygen delivery by the changes
in the oxygen dissociation
curve
4. The Accent’s movement and
the improved circulation
stimulate lymphatic drainage
and toxin reduction
37. In Summery, The Accent:
• Contracts and multiplies collagen fibers to
tighten tissue, improving laxity and
firmness.
• Contracts connective tissue, improving the
skin’s contour, texture and external
architecture.
• Moves retained fluids from subcutaneous
adipose tissue for volume reduction
