This is a picture that Dr. Thomas Murphy submitted to JAMA that he received from his young patient. The artist (click) is on the examining table. Her older sister is seated nearby in a chair (click), reading a magazine, as is her mother (click), cradling her baby sister. And (click) guess who this is? – yep… that’s the doctor staring at the computer (click), his back to the patient—and everyone else. When he saw the drawing, the physician wrote this caption: “The economic stimulus bill has directed $20 billion to health care information technology, largely funding electronic medical record incentives. I wonder how much this technology will really cost?” The relationship between physicians and patients is at the core of healing. This begins with hearing and understanding. Let’s reimagine healthcare — where physicians can get back to the art of medicine.   • #5 And physicians are in the very eye of the storm. They are being asked to change the way they practice. To use EHRs in ways they never have before. To document many more things in much more detail to ensure that clinical documentation: Reflects whether a patient was admitted with a high risk condition or infection or acquired it at the hospital Provides specific details required for Meaningful Use quality scoring and attestation reporting Support medical necessity with appropriate diagnoses and treatment reasons The information they must capture in clinical documentation to support these major trends and initiatives can be overwhelming. And physicians are on the go and looking to either use their own mobile devices (which may challenge information security systems). In essence, they demand easier access to document care.   • #6 With the overall change in accountable care and reimbursement, the goal is to keep people out of the hospital. Revenue cycle maximization, ensuring billing is timely and accurate with the transition to ICD-10 and cash management are top priorities. The heavy reporting requirements for Meaningful Use, Audits, Reimbursement and Readmission Penalties and the 2 Midnight rule creates even more stress on the organization’s financial viability.
