You Decide: The Case of Suzanne
This case is presented in the voices of Suzanne and her mother, Sherry. Throughout the case, you will be asked to consider a number of issues and to arrive at various decisions, including diagnostic and treatment decisions. Appendix C reveals Suzanne’s probable diagnosis, the DSM-5 criteria, clinical information, and possible treatment directions.
Suzanne A Sign of Things to Come
I don’t know when I started doing it. I guess I’ve always hated school and I’ve always been really nervous about things. A lot of the time, even before college, I used to play with my hair a lot and pull on it; the more nervous I became, the tighter I pulled. But I didn’t think there was anything unusual about it. You know, everyone has nervous habits that they turn to when they get stressed out, right? My parents were . . . let’s say “difficult.” They were always making me feel like I didn’t do well enough. “Couldn’t you have gotten an A? Couldn’t you play basketball or soccer? Couldn’t you have won the game? What could you have done to prepare for the test better? What can we learn from this? Why don’t you have a boyfriend? Maybe if you dressed differently? You know, we just want you to be happy.”
Many loving parents are described as being “overconcerned” about their children, or “overcontrolling” or “overinvolved.” Where do such patterns of behavior come from? What can parents do to avoid crossing the line in their efforts to guide and protect their maturing children?
I guess this whole thing really started a long time ago. When I was in seventh grade, I used to pluck out my eyelashes. I can’t remember how it started, but I remember that it used to relax me when I was tense. I also got the idea in my head that my eyelashes and my eyes were really irritated. I thought that maybe I had some dirt caught in there, and it was stuck between my eyelashes. So I would loosen it and stop the irritation by pulling the eyelash out. I actually remember thinking that there were microscopic bacteria—like I had seen in a science film—living on the end of the eyelash, wiggling around under my skin, and the bad eyelashes needed to be pulled out. The little pain of pulling the lash out was something I actually looked forward to, like when you have a hanging fingernail that hurts and you need to pull it out: a second of pain and instant relief. Once I started noticing that I was doing it, I would be really nervous right before I would pull it, and I would think that maybe I shouldn’t pull this one. Then, when I was pulling it out, I imagined I could feel the irritating part coming out. After it was over, I felt relieved, all the nervousness gone. I would look at the lash I had pulled, almost trying to see the little bacteria wiggling like a worm on a fishhook. Soon, my eyelids were running out of hair, but no one seemed to notice, so I thought it wasn’t a big deal. Maybe I wasn’t really plucking them all out, I thought. Maybe I was just pluck ...

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

Flu Vaccine Alert in Bangalore Karnatakaaddon Scans
As flu season approaches, health officials in Bangalore, Karnataka, are urging residents to get their flu vaccinations. The seasonal flu, while common, can lead to severe health complications, particularly for vulnerable populations such as young children, the elderly, and those with underlying health conditions.
Dr. Vidisha Kumari, a leading epidemiologist in Bangalore, emphasizes the importance of getting vaccinated. "The flu vaccine is our best defense against the influenza virus. It not only protects individuals but also helps prevent the spread of the virus in our communities," he says.
This year, the flu season is expected to coincide with a potential increase in other respiratory illnesses. The Karnataka Health Department has launched an awareness campaign highlighting the significance of flu vaccinations. They have set up multiple vaccination centers across Bangalore, making it convenient for residents to receive their shots.
To encourage widespread vaccination, the government is also collaborating with local schools, workplaces, and community centers to facilitate vaccination drives. Special attention is being given to ensuring that the vaccine is accessible to all, including marginalized communities who may have limited access to healthcare.
Residents are reminded that the flu vaccine is safe and effective. Common side effects are mild and may include soreness at the injection site, mild fever, or muscle aches. These side effects are generally short-lived and far less severe than the flu itself.
Healthcare providers are also stressing the importance of continuing COVID-19 precautions. Wearing masks, practicing good hand hygiene, and maintaining social distancing are still crucial, especially in crowded places.
Protect yourself and your loved ones by getting vaccinated. Together, we can help keep Bangalore healthy and safe this flu season. For more information on vaccination centers and schedules, residents can visit the Karnataka Health Department’s official website or follow their social media pages.
Stay informed, stay safe, and get your flu shot today!

New Directions in Targeted Therapeutic Approaches for Older Adults With Mantl...i3 Health
i3 Health is pleased to make the speaker slides from this activity available for use as a non-accredited self-study or teaching resource.
This slide deck presented by Dr. Kami Maddocks, Professor-Clinical in the Division of Hematology and
Associate Division Director for Ambulatory Operations
The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.
Target Audience
Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).
Learning Objectives
1.) Identify clinical and biological prognostic factors that can guide treatment decision making for older adults with MCL
2.) Evaluate emerging data on targeted therapeutic approaches for treatment-naive and relapsed/refractory MCL and their applicability to older adults
3.) Assess mechanisms of resistance to targeted therapies for MCL and their implications for treatment selection

A glimpse into the brain of anxiety
1. In C O F F E E C H AT S
A Glimpse into the Brain of Anxiety
Today’s Cup of Coffee is from my favorite place, Hopscotch Coffee. I am drinking a cappuccino made with almond milk. It’s warm,
creamy, bitter, but delicious. Today’s Coffee Chat is focused on the topic of anxiety.
“Step out the front door like a ghost into the fog, where no one notices the contrast of white on white.”
It’s the loneliness, the darkness, and the frustration brought on daily. It’s trying to fix something that’s not broken. It’s feeling
unnoticed, not worthy, and insecure.
When life is going the way you want, you’re always worrying about what could go wrong. Thinking, ” how can I control this? How
can I control what happens next?” The issue with this is that you’ll never control the things you truly want to control. There are
things in this life that bring me constant anxiety and I’m not going to control them no matter how much I would like to. I want to
control things like diseases that effect those closest to my heart. I want to stop him from potentially leaving me one day, while I
can do all the right things at the end of the day you’re not going to stop some one from leaving you if that’s what they decide. I
can’t control someone else’s feelings. You want to give everything you have to make those you love happiest, even if that means
making sacrifices, but sometimes there isn’t anything you can do except just be there for someone. However, I will always worry, I
will always jump to worse case scenarios in my head.
If I haven’t heard from my parents in a while my mind goes straight to the idea that they were in a car accident. If he doesn’t
respond to my text within 15 minutes I worry he’s mad at me. My mind is more pessimistic than optimistic. It’s an everyday uphill
battle. People would argue you can control your attitude and optimism, however, it’s extremely difficult when your mind tries to
fight you in that. If he doesn’t text me that he made it to his destination, I’m calling the people he might be with to make sure he’s
okay.
Anxiety makes me feel worthless. It makes social situations hard. If you’re not invited to something, you feel like they don’t want
to be your friends anymore. You feel as if you’re not as crucial to the friend group compared to others.
When things are going wrong in your life, you might tell one person you are super close with every last excruciating detail. You’re
friends, who may notice something is wrong, will get some of the information, but nothing too detailed. It’s wanting to put on a
front that you have it all together. You’re Instagram is on point, you’re seen as ‘healthy’, or at least that’s what your Instagram
tells people. They don’t see your portion control issues or the slip ups that you beat yourself up over.
It’s being extremely self aware. Anxiety, leads to over thinking every word you say to someone. It’s worrying that you might have
come off too negative to someone, or said something that might have secretly offended them. No matter how irrational it may
seem, these are valid feelings.

Anxiety might make you seem crazy to those who don’t understand it. However, that’s why I’m writing. I want those who don’t
experience this first hand to hopefully get a glimpse into the mind of someone who does. Being the significant other to someone
who has anxiety must be difficult. Dealing with irrational breakdowns, 15 texts in a row making sure you’re doing okay, saying I
love you every time you walk out the door without fail, and having to put your own emotions aside to help your significant other
must be tiring. My anxiety has ruined many things in my life, and you’re constantly learning how to get around that; prevent it
from happening again. I don’t know how Israel deals with me so well, I know it must be frustrating. He doesn’t question it, he just
holds me in his arms, helps guide me through some deep breathing, and wipes away my tears. He’s so patient with me, even
when I’m not always patient with him.
“She parks her car outside of my house, takes her clothes off, says she’s close to understanding Jesus. She knows she just a little
misunderstood. She has trouble acting normal, well I have trouble acting normal.”
Anxiety makes my relationship with God go through ups and downs. It’s a control issue. Sometimes I put my complete and utter
faith in him and allow Him to take control. Other times, I try to take matters into my own hands whether I realize it or not and
leave Him out of it. While I know this is not effective, it definitely is challenging to overcome permanently. What I do know is that
surrendering it all to God is the best thing you can do. Just pray on it. However, I know that is much easier said then done.
I also believe in the power of therapy. My psychologist has seen me all throughout different periods in my life depending what is
happening. I know there is a stigma with therapy, but I encourage you to go if you’re on the fence about it. It truly has helped
equipped me with the tools to help me cope. I also know there is sometimes a chemical imbalance in the brain that causes
anxiety. This is the case for me, as well. I tried various coping mechanisms for the majority of my life, until finally my doctor and I
determined that there must be something else going on, that is partly out of my control. However, you just have to take it one
day at a time.
There will always be days of ups and downs. You’ll feel social and want to be with your friends 24/7 and then you ‘ll have times
where you will hide in your room for days because you don’t want to be a burden to those around you. I get it. I’m right there
with you.
I know we can get through this. I know it’s a struggle but pushing through to the other side is worth it even if it’s not permanent.
Over time you’ll learn methods that will help you cope, and hopefully allow you to diminish the effects of your anxiety on a daily
basis. You got this. I believe in you.
Here are some song lyrics that just have been resonating with me recently. Take what you’d like out of them, and ponder on what
they might mean to you.
“I need a phone call, I need a raincoat, I need a big love, I need a phone call, I need a Sunburn.” – Counting Crows
“She walks along the edge of where the ocean meets the land, just like she’s walking on a wire in the circus, she parks her car
outside of my house and takes her clothes off, says she’s close to understanding Jesus
and she knows she’s more than just a little misunderstood, she has trouble acting normal when she’s nervous” – Counting Crows
