This randomized controlled trial compared the safety and efficacy of isotonic (0.9% saline) versus hypotonic (0.18% saline) intravenous fluids for children. Sixty children between 6 months and 12 years old who required intravenous fluids for 48 hours were assigned to receive either isotonic fluids at 60% of standard volume or hypotonic fluids at standard volume. The primary outcome was incidence of hyponatremia (sodium <130 mmol/L). At 24 hours, hyponatremia occurred in 24% of those receiving isotonic fluids versus 55% of those receiving hypotonic fluids.
